Background: Surgery for benign disease is associated with a low-risk of developing venous thromboembolism (VTE). Despite a relatively low incidence of postoperative VTE in patients after elective cholecystectomy and abdominal hernia repair there are data proving hypercoagulability in the early postoperative period. We focused on assessment of the systemic inflammatory response and coagulation status in these surgical patients after hospital discharge. Methods: Prospectively, patients who underwent surgery for benign disease were included. Two hundred sixteen patients were enrolled - 90 patients in laparoscopic cholecystectomy (LC) group and 126 patients in hernia surgery (HS) group. Risk assessment of VTE according to the Caprini risk assessment model was performed in all patients. Prevalence of VTE in postoperative period was observed. Markers of systemic inflammatory response (IL-6, CRP, alpha-1-acid glycoprotein, transferrin) and coagulation markers (PLT, fibrinogen, prothrombin fragment F1 + 2 and D-dimer) were measured before surgery, on 7-10th postoperative day and on 28-30th postoperative day. Results: Clinically apparent deep vein thrombosis was diagnosed in only one patient - 0.46 %. Statistically significant elevation of inflammatory markers IL-6, CRP and alpha-1-acid glycoprotein (p < 0.001; all) were proved in both groups of patients on 7-10th postoperative day. Statistically significant elevation of coagulation markers PLT, fibrinogen, prothrombin fragment F1 + 2 and D-dimer (p < 0.001; all) were proved in LC and HS groups on 7-10th postoperative day. No statistical difference was observed in IL-6, CRP and alpha-1-acid glycoprotein levels a month after surgery as compared with preoperative levels within each group. Statistically significant elevation of fibrinogen and prothrombin fragment F1 + 2 levels (p < 0.001; both) persisted on 28-30th postoperative day in both groups. Persisted elevation of D-dimer levels was proved only in HS group (p < 0.001), not in LC group (p = 0.138), a month after surgery. Conclusions: Activated systemic inflammatory response and hypercoagulable condition were verified in patients after laparoscopic cholecystectomy and hernia surgery after their hospital discharge. Hypercoagulability persisted even a month after surgery. Nevertheless, we observed very low prevalence of clinically apparent VTE in patients with in-hospital postoperative VTE prophylaxis.

Plasma Hsp90 levels in patients with systemic sclerosis and relation to lung and skin involvement: a cross-sectional and longitudinal study

Hana Storkanova, Sabina Oreska, Maja Spiritovic, Barbora Hermankova, Kristyna Bubova, Martin Komarc, Karel Pavelka, Jiri Vencovsky, Joerg H. W. Distler, Ladislav Senolt, Radim Becvar, Michal Tomcik

Summary: According to the study, plasma levels of Hsp90 are increased in patients with systemic sclerosis compared to healthy controls, and are associated with increased inflammatory activity, worsened lung function, and extent of skin involvement in patients with diffuse cutaneous systemic sclerosis. In patients with systemic sclerosis-associated interstitial lung disease treated with cyclophosphamide, baseline Hsp90 levels can predict changes in DLCO over 12 months of therapy.

The Periphery of Salivary Gland Carcinoma Tumors Reveals a PD-L1/PD-1 Biomarker Niche for the Evaluation of Disease Severity and Tumor-Immune System Interplay

Martin Kuchar, Zuzana Strizova, Linda Capkova, Martin Komarc, Jiri Skrivan, Jirina Bartunkova, Daniel Smrz, Jan Plzak

Summary: This research found that PD-1 and PD-L1 expression in salivary gland cancer patients primarily cluster in the tumor periphery, potentially predicting the response to immune checkpoint inhibitor therapy. Additionally, PD-1 positive tumor cells were mainly found in the center of high-grade tumors, and the presence of lymph node metastases and primary tumor stage significantly correlated with PD-L1 positive tumor cells in the tumor periphery.

Fas-Fas Ligand Interplay in the Periphery of Salivary Gland Carcinomas as a New Checkpoint Predictor for Disease Severity and Immunotherapy Response

Summary: Salivary gland carcinomas (SGCs) exhibit high morphological heterogeneity and limited treatment options. Immunotherapy with immune checkpoint inhibitors (ICIs) is a promising approach for SGCs, but their resistance to this therapy may be modulated by the Fas-FasL interplay. The study found that SGC cells exhibit resistance to FasL-induced apoptosis by tumor-infiltrating immune cells, potentially impacting immunotherapy outcomes in high-stage tumors.

Summary: The study revealed stable uncorrected distance visual acuity in myopic patients 12 years after iris-fixated phakic intraocular lenses (IF pIOLs) implantation, but also a certain proportion of cataract formation and endothelial cell loss postoperatively. Factors such as preoperative corneal thickness and re-enclavation surgery were closely associated with patient outcomes.

Thrombin Decrease in Thrombin Generation after Heparin Administration in a Homozygous Type II Heparin Binding Site Antithrombin-Deficient Pregnant Woman

Ivana Malikova, Martina Husakova, Jana Bilkova, Radka Brzezkova, Tomas Kvasnicka

Summary: This study aimed to determine the effectiveness of heparin by monitoring changes in thrombin generation in vitro, so that pregnant women are not exposed to stress in vivo. The results showed significant differences in the degree of inhibition among different heparins, with nadroparin being the most effective. Therefore, monitoring thrombin generation should be supplemented with routine anti-Xa activity monitoring to estimate thrombophilic risk during pregnancy.

Oncostatin M and Nivolumab Affect the Cytotoxic T-Cell Proportions and the Susceptibility to TRAIL-Induced Death in Non-Leukocyte Cell Subpopulations in Soft Tissue Sarcomas

Giulia Toffanin, Andrej Ozaniak, Robin Bartolini, Martin Komarc, Rene Novysedlak, Michal Rataj, Jitka Smetanova, Antonio Rosato, Robert Lischke, Jirina Bartunkova, Zuzana Strizova

Summary: This study aimed to investigate the effects of Oncostatin M (OSM) on immune cells derived from liposarcoma, leiomyosarcoma, and myxofibrosarcoma, and the potential cooperation between OSM and nivolumab in treating these soft tissue sarcomas (STSs). The results suggest that OSM may have a role in the treatment of leiomyosarcoma, myxofibrosarcoma, and liposarcoma. However, further histotype-tailored studies are needed to fully comprehend the functions of OSM in STSs.
