Summary: Elevated levels of OLAH in the placenta may be associated with placental dysfunction in pregnancies complicated by preterm preeclampsia and fetal growth restriction. Further research is required to understand the role of OLAH in the placenta, and whether these changes are a maternal adaptation or consequence of disease.

Summary: Patients with systemic autoimmune rheumatic disease (SARD) are more susceptible to SARS-CoV-2 infection. This study compared the characteristics, severity, course, and outcomes of COVID-19 in SARD patients with the non-SARD group. Factors associated with prognosis, including remdesivir therapy efficacy, were also examined. The study found that SARD patients had longer hospital stays, higher mortality rates, and a greater need for oxygen therapy compared to the non-SARD group. Age, the presence of cardiovascular disease, severity of condition on admission, and higher inflammatory marker values were identified as risk factors for death in the SARD group. Remdesivir treatment showed a trend towards improved mortality in SARD patients but was not statistically significant. Overall, SARD patients had worse outcomes and were more likely to have other chronic diseases.

Summary: A retrospective review was conducted to assess the clinical utility of pre-pregnancy planning among female patients with rheumatic diseases. The study found that patients who received pre-pregnancy planning had fewer prenatal disease flares, medication changes during pregnancy, and improved disease control in the first trimester. However, there were no statistically significant differences in adverse pregnancy or fetal outcomes between patients with and without pre-pregnancy planning.

Summary: Calcinosis is a severe complication of autoimmune disorders, including types like calcinosis cutis and calciphylaxis. Calcification of damaged tissues in the presence of normal calcium and phosphate levels is commonly associated with autoimmune diseases. Calciphylaxis, characterized by vascular calcifications and thrombosis, is also related to autoimmune conditions. It is important for physicians to be aware of the clinical presentation and management of calcinosis in order to choose appropriate treatment and prevent long-term complications.

Summary: NGS technologies have played a crucial role in the study and diagnosis of autoimmune diseases, particularly in autoimmune connective tissue diseases, with panels, whole-genome and -exome sequencing, and HLA widely used. Despite the wide scope of research areas, it seems that epigenetic influence on the development of systemic autoimmune diseases may set important trends for future studies.

Summary: Thoracic involvement is a major determinant of morbidity and mortality in patients with autoimmune rheumatic diseases. Interstitial lung disease is the most common pulmonary complication, while other manifestations include pulmonary arterial hypertension, airway disease, and pleural involvement. Treatment mainly focuses on immunosuppression, but further research is needed for a better understanding of the pathogenesis and development of more effective therapies.

Summary: This study aimed to assess the potential of lipid metabolites for detection of late-onset preeclampsia and fetal growth restriction. The study identified phosphatidylinositol 32:1 and cholesterol ester 17:1 as the lipids with the best predictive capacity for the risk of developing preeclampsia or delivering a fetal growth restricted infant. The results suggest that lipid measurements have the potential to improve non-invasive assessment of maternal and fetal health.

Summary: Intima-media thickness (IMT) measurement is a non-invasive method to assess arterial wall, and it is associated with atherosclerosis and endothelial dysfunction. Pre-existing conditions like diabetes, chronic hypertension, and obesity may impair the adaptation to pregnancy and influence fetal IMT. Increased fetal abdominal aorta IMT (aIMT) has been correlated with placental dysfunctions, which may lead to gestational hypertension, preeclampsia, and fetal growth restriction. Understanding the underlying pathomechanism is crucial for developing targeted therapies.

Summary: This study aimed to investigate the association between maternal serum albumin (MAlb) levels and fetal growth as well as the risk of fetal growth restriction (FGR). The findings revealed a reverse U-shaped relationship between MAlb and fetal growth, indicating an increased risk of FGR when MAlb levels were either too low or too high. These findings are important for understanding the relationship between maternal serum albumin and fetal health.

Summary: uPAR is a membrane-bound glycoprotein, and its bioactive form, suPAR, is primarily expressed on the surface of immunologically active cells. Higher levels of suPAR have been associated with disease severity, relapse, and mortality in various inflammatory diseases. This review summarizes the current literature on the potential role of suPAR as a biomarker in autoimmune rheumatic and non-rheumatic diseases.

Summary: Early onset fetal growth restriction (FGR) is often associated with poor neonatal outcomes. It is still unclear whether pregnancies complicated by early FGR plus hypertensive disorders of pregnancy (FGR-HDP) and those complicated by early onset FGR without HDP (normotensive-FGR (n-FGR)) show different prenatal and postnatal outcomes. Recent data suggest that the presence of preeclampsia may increase the risk of poor neonatal outcomes in pregnancy affected by early onset FGR-HDP compared to pregnancy affected by early onset n-FGR. This review aims to compare the clinical characteristics, perinatal outcomes, and potential long-term sequelae of early onset FGR-HDP and early onset n-FGR.

Summary: Malakoplakia is a rare condition characterized by histiocytic cells accumulation, often mimicking neoplasia. Gynecologic involvement is extremely rare, leading to potential diagnostic confusion. It is important to differentiate malakoplakia from other histiocytic disorders to prevent misdiagnosis and overtreatment of this benign condition.

Summary: Universal testing for pregnant women during the COVID-19 pandemic is suggested as a useful strategy. During the study period, 3.1% of pregnant women tested positive for SARS-Cov-2, a significantly higher proportion than estimated by health authorities. Among the positive cases, 44.9% had symptoms or reported close contact with positive patients.

Summary: The study found that in pregnancies complicated by intrauterine growth restriction, both maternal and fetal vascular malperfusion are associated with an increased risk of abnormal infant neurodevelopmental outcomes at 2 years of age. Specifically, fetal vascular malperfusion has a more significant impact on infant neurodevelopment, highlighting the importance of attention to this issue in clinical practice.

Summary: This study found that among HIV-positive pregnant women, those with a CD4/CD8 ratio less than 0.3 were more likely to have insufficient HIV-RNA suppression in the third trimester and to deliver preterm compared to women with a ratio of 1 or higher. Additionally, women with a baseline CD4/CD8 ratio between 0.3 and 0.45 were also at increased risk for preterm delivery.

Summary: Antisynthetase syndrome (ASSD) is an autoimmune disease characterized by the presence of autoantibodies against aminoacyl transfer RNA (tRNA) synthetases. In this study, researchers identified differentially expressed miRNAs between ASSD patients with and without interstitial lung disease (ILD). Two miRNAs, miR-30a-5p and miR-29c-3p, were found to be upregulated in ASSD-ILD patients compared to those without lung involvement. These miRNAs play a role in regulating inflammation and fibrosis, and their levels in exosomes could be useful in identifying patients at risk of ILD.
