Summary: Community-level vulnerability is associated with poor fidelity in the Living Donor Navigator Program, and factors such as community unemployment rate, single-parent households, minority population, and households without vehicles are associated with increased risk of self-advocacy.

Summary: In October 2021, the American Society of Transplantation (AST) held a virtual consensus conference to address barriers preventing the expansion of living donor liver transplantation (LDLT) in the United States. A multidisciplinary group of experts identified financial implications on donors, crisis management, policies and regulations, and ethical considerations as significant barriers. They proposed strategies to overcome these barriers and highlighted the importance of donor safety and the need for appropriate crisis management plans in transplant programs.

Summary: Minimally invasive laparoscopic nephrectomy for living kidney donors resulted in shorter surgical duration, decreased length of hospital stay, and higher postoperative quality of life compared to open nephrectomy. The complication rates were similar, with a slight difference in surgical complications between the two groups.

Summary: Nearly 10,000 people are removed from the kidney transplant waiting list each year due to illness or death. Live donor kidney transplant (LDKT) provides better outcomes and survival benefit, but the number of LDKT has decreased in recent years. It is important to use the best available data and avoid biases when evaluating potential living kidney donors.

Summary: This study surveyed living kidney donors to understand their perspectives on follow-up laboratory testing. The completion of 6-month testing was strongly associated with the completion of 12-month testing. Donors who did not prioritize testing and forgot to get labs drawn until reminded were more likely to complete the testing.

Summary: This article conducts an in-depth ethical analysis of living donor uterus transplantation by integrating clinical, psychological, and qualitative study data. The concept of living organ donors as patients in their own right is applied to living uterus donation, grounded in the principles of research ethics and considering eight potential vulnerabilities of living donors. It also emphasizes the importance of special responsibilities in the relationship between living donor advocates and potential donors. Additionally, ethical analyses need to be conducted separately for directed and non-directed uterus donors due to their different relationships with recipients.

Summary: Based on data from Australia and New Zealand, females constitute a larger proportion of kidney donors to male recipients. The relationship between recipient and donor is a factor that modifies the association between recipient gender and donor gender. Older recipients residing in remote areas are more likely to receive kidneys from female donors.

Summary: This study investigated the experiences and attitudes towards anonymity among kidney donors and recipients participating in non-directed altruistic kidney donation and the UK living kidney sharing scheme. The results showed that some participants broke anonymity after transplantation and were satisfied with contact or meeting with the other party. Another group of participants remained anonymous, but a portion of them also expressed a desire to meet post-transplant. The study supports the expansion of conditional anonymity to other countries that allow anonymous kidney transplantation.

Summary: There is a lack of consensus on how to assess steatosis in potential donor livers for transplantation, leading to wide variations in definitions and approaches used by pathologists. A study by the Banff Working Group on Liver Allograft Pathology aimed to address this issue by establishing criteria for defining large droplet fat (LDF) and developing an algorithm for determining fat droplet size and percentage of hepatocytes involved. The proposed guidelines intend to improve standardization in steatosis assessment of donor liver biopsies, with the calculated percentage of LDF to be provided to the surgeon.

Summary: The study compared the outcomes of kidney transplant patients receiving belatacept versus tacrolimus for maintenance immunosuppression with rapid steroid withdrawal over a period of 2 years. The results showed similar patient and graft survival rates between the groups, but a significantly lower incidence of eGFR <45 ml/min per 1.73 m(2) in the belatacept group and a significantly higher incidence of biopsy-proven acute rejection.

Summary: Solid organ transplantation is a life-saving procedure that has made significant progress in the past 70 years, particularly in T-cell-targeted immunosuppression and organ allocation systems. However, humoral alloimmune responses remain a major challenge, especially for patients with preexisting anti-HLA antibodies. Therapies to target plasma cells, the source of antibody production, have had limited effectiveness in reducing antibody levels.
