Using Whole-Exome Sequencing to Identify Genetic Markers for Carboplatin and Gemcitabine-Induced Toxicities

Purpose: Chemotherapies are associated with significant interindividual variability in therapeutic effect and adverse drug reactions. In lung cancer, the use of gemcitabine and carboplatin induces grade 3 or 4 myelosuppression in about a quarter of the patients, while an equal fraction of patients is basically unaffected in terms of myelosuppressive side effects. We therefore set out to identify genetic markers for gemcitabine/carboplatin-induced myelosuppression. Experimental Design: We exome sequenced 32 patients that suffered extremely high neutropenia and thrombocytopenia (grade 3 or 4 after first chemotherapy cycle) or were virtually unaffected (grade 0 or 1). The genetic differences/polymorphism between the groups were compared using six different bioinformatics strategies: (i) whole-exome nonsynonymous single-nucleotide variants association analysis, (ii) deviation from Hardy-Weinberg equilibrium, (iii) analysis of genes selected by a priori biologic knowledge, (iv) analysis of genes selected from gene expression meta-analysis of toxicity datasets, (v) Ingenuity Pathway Analysis, and (vi) FunCoup network enrichment analysis. Results: A total of 53 genetic variants that differed among these groups were validated in an additional 291 patients and were correlated to the patients' myelosuppression. In the validation, we identified rs1453542 in OR4D6 (P = 0.0008; OR, 5.2; 95% CI, 1.8-18) as a marker for gemcitabine/carboplatin-induced neutropenia and rs5925720 in DDX53 (P = 0.0015; OR, 0.36; 95% CI, 0.17-0.71) as a marker for thrombocytopenia. Patients homozygous for the minor allele of rs1453542 had a higher risk of neutropenia, and for rs5925720 the minor allele was associated with a lower risk for thrombocytopenia. Conclusions: We have identified two new genetic markers with the potential to predict myelosuppression induced by gemcitabine/ carboplatin chemotherapy. (C)2015 AACR.

Summary: The TRACERx study reveals the relationship between tissue morphology, the genomic landscape, and relapse risk of lung adenocarcinomas. Higher-grade tumors have increased chromosomal complexity and tend to evolve towards higher-grade patterns. The presence of micropapillary pattern and 'tumor spread through air spaces' are associated with intrathoracic recurrence.

In vitro cannabinoid activity profiling of generic ban-evading brominated synthetic cannabinoid receptor agonists and their analogs

Summary: Following the generic ban in China in 2021, the synthetic cannabinoid market has seen the emergence of compounds with wider structural diversity. This study investigated the cannabinoid receptor activation potential of different analogs, including tail-less compounds and those with a brominated core. The results showed that both tail-less and tailed analogs exhibited cannabinoid activity, albeit with varying potency and efficacy. The study also assessed the CB activity of mixtures of different reference standards and confirmed the presence of ADB-5'Br-INACA and MDMB-5'Br-INACA in seized powders.

Summary: 1-(2,3-Dihydro-1H-inden-5-yl)-2-(piperidin-1-yl)pentan-1-one (3,4-Pr-PipVP), a new synthetic cathinone, was discovered in Germany in 2022. It was initially thought to contain a novel bicyclo[4.2.0]octatrienyl function but was later found to contain a scheduled indanyl ring. It is one of the few SCats with a piperidine ring and showed low potency as a blocker of monoamine transporters.

Using in vitro receptor activity studies of synthetic cannabinoids to support the risk assessment of new psychoactive substances-A Swedish strategy to protect public health from harm

Summary: In the past 15 years, a considerable number of new psychoactive substances (NPS) have been identified in Europe and globally, with limited information on their safety and toxicity. To address this issue, the Public Health Agency of Sweden (PHAS) collaborated with the National Board of Forensic Medicine to establish a strategy using in vitro receptor activity assays to assess the neurological activity of NPS. The first results of this collaboration focused on synthetic cannabinoid receptor agonists (SCRAs). Among the 18 SCRAs selected for characterization, 14 were scheduled as narcotics in Sweden based on their activity on the human cannabinoid-1 receptor.
