Summary: A 75-year-old woman with persistent pancytopenia and fever nine years after a kidney transplant was diagnosed with cutaneous, mucocutaneous, and visceral leishmaniasis. This is the first report in Portugal of a kidney transplant recipient presenting with the full spectrum of leishmaniasis. High clinical suspicion is required for diagnosis of this rare infection, especially in endemic regions where visceral leishmaniasis can be life-threatening.

Summary: This study aims to examine the incidence, associations, and outcomes of posttransplant parathyroidectomy and calcimimetic use in a cohort of Medicare-insured US kidney transplant recipients. The study found that the rate of posttransplant parathyroidectomy remained stable over time, while the use of calcimimetic increased during the study period. Long dialysis vintage and pre-transplant calcimimetic use were strongly associated with posttransplant parathyroidectomy and calcimimetic use. Roughly 1 in 4 patients were hospitalized within 90 days of posttransplant parathyroidectomy, with hypocalcemia-related diagnoses being the most common complication. Parathyroidectomy was not associated with an increase in acute kidney injury compared to calcimimetic initiation.

Access to transplantation for older adults can be increased through interventions targeting frailty and reducing waiting times through optimized organ use. Focused study and educational interventions are needed to improve outcomes for this vulnerable group.

However, there are numerous challenges and uncertainties associated with genetic testing for asymptomatic living kidney donors. Not all transplant practitioners are aware of the limitations of genetic testing or have access to specialized genetic counseling or expertise in renal genetics.

Summary: This study compared the long-term outcomes of kidney transplantation in patients with and without congenital anomalies of the kidney and urinary tract (CAKUT). It was found that patients with CAKUT had a higher risk of urinary tract infections compared to those without CAKUT, but the rates of graft rejection were similar between the two groups. The graft survival rate was higher in patients with CAKUT, suggesting the importance of providing support and encouragement for CAKUT patients with kidney failure to seek transplantation.

Summary: This study compared the clinical outcomes and outcomes of living donation in recipients with aHUS and C3G to a control group. It found that donors for complement-related kidney diseases did not experience major cardiac events or thrombotic microangiopathy, and the occurrence of new-onset hypertension was similar. Although there were two cases of cancer and death among related donors, the overall results suggest that kidney transplantation is relatively safe.
