Post-COVID-19 paediatric inflammatory multisystem syndrome: association of ethnicity, key worker and socioeconomic status with risk and severity

Summary: This population-based retrospective cohort study found that children from ethnic minority groups, living in more deprived areas, and from key worker families are over-represented among cases of Paediatric Inflammatory Multisystem Syndrome Temporally associated with SARS-CoV-2 (PIMS-TS). Children from black ethnic groups had longer admissions and were more likely to require ventilation.

Multisite Veno-Venous Cannulation for Neonates and Nonambulatory Children*

Jon Lillie, Alison Pienaar, Jenny Budd, Simone Speggiorin, Anne Gordon, Paul James, Jo Perkins, Benedict Griffiths, Miriam Fine-Goulden, Conal Austin, Caner Salih, Nicola Agnew, Andrew G. Nyman

Summary: This study reported on the use of VV extracorporeal membrane oxygenation in neonates and children under 10 kg, accessing the jugular and returning to the femoral vein. The procedure was technically successful and post-decannulation follow-up showed no clinical implications.

Systemic Inflammation Is Associated With Neurologic Involvement in Pediatric Inflammatory Multisystem Syndrome Associated With SARS-CoV-2

Mario Sa, Luwaiza Mirza, Michael Carter, Lalani Carlton Jones, Vasantha Gowda, Jennifer Handforth, Tammy Hedderly, Julia Kenny, Karine Lascelles, Jean-Pierre Lin, Daniel Lumsden, Marilyn McDougall, Owen Miller, Thomas Rossor, Vinay Shivamurthy, Ata Siddiqui, Rahul Singh, Shan Tang, Marie White, Susan Byrne, Ming Lim

Summary: Pediatric inflammatory multisystem syndrome temporally associated with SARS-CoV-2 (PIMS-TS) is a severe immune-mediated disorder. Neurologic involvement was found in 12% of children with PIMS-TS, presenting with various symptoms such as altered consciousness, behavioral changes, focal neurology deficits, headaches, hallucinations, excessive sleepiness, and focal seizures. Higher systemic inflammatory markers were identified in children with neurologic involvement and in those who had not fully recovered after 3 months of follow-up.

Contemporary surgical outcome and symptomatic relief following vascular ring surgery in children: effect of prenatal diagnosis

Parinita Swarnkar, Simone Speggiorin, B. Conal Austin, Andrew Nyman, Caner Salih, Vita Zidere, John M. Simpson, Trisha V. Vigneswaran

Summary: This study aimed to describe the postoperative complications and outcomes in children with isolated double aortic arch or right aortic arch with left arterial ligament and assess the impact of fetal diagnosis on outcomes. The most common complication was vocal cord palsy, and older age at surgery was the only predictor for postoperative complications. Prenatal diagnosis was associated with a lower rate of postoperative symptoms.

In vitro measurements of ultrafiltration precision in hemofiltration and hemodialysis devices used in infants

Jean Crosier, Mike Whitaker, Heather J. Lambert, Paul Wellman, Andrew Nyman, Malcolm G. Coulthard

Summary: In vitro testing showed that conventional hemotherapy devices in the UK do not regulate ultrafiltration control well enough for use in small infants, except for the volumetrically controlled NIDUS circuit.

Lower Risk of Multisystem Inflammatory Syndrome in Children With the Delta and Omicron Variants of Severe Acute Respiratory Syndrome Coronavirus 2

Jonathan M. Cohen, Michael J. Carter, C. Ronny Cheung, Shamez Ladhani

Summary: Little is known about the risk of multisystem inflammatory syndrome in children (MIS-C) with different severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variants. In southeast England, MIS-C rates per confirmed SARS-CoV-2 infections in children aged 0-16 years were 56% lower during prevaccine Delta, 66% lower during postvaccine Delta, and 95% lower during the Omicron period.

Summary: This study compared the precision, biochemical clearances, clinical efficacy, outcomes, and safety profile of a new non-Conformite Europeenne-marked hemodialysis device (NIDUS) with peritoneal dialysis (PD) and continuous venovenous hemofiltration (CVVH) for small babies. The results showed that NIDUS provides accurate, controllable fluid removal and adequate clearances, indicating its important potential alongside other modalities for infant renal replacement therapy.

Impact on the trachea in children with prenatally diagnosed vascular ring formed by a right aortic arch

Christopher Oakley, Alice Hurn, Claire Suckling, Grace Moriarty, John M. Simpson, Simona Turcu, Jane Heraghty, Vita Zidere, Andrew Nyman, Trisha V. Vigneswaran

Summary: This study investigated the relationship between prenatal detection of right sided aortic arch (RAA) with left-sided arterial duct (LD) and symptoms and severity of tracheobronchial compression assessed by bronchoscopy. The study found that the absence of symptoms did not exclude significant tracheal compression.

Catheter directed thrombolysis for a pulmonary arterial thrombus on VA ECMO in a child - a case report

Livia Procopiuc, Elizabeth Boot, Paul James, Narayan Karunanithy, Chris Meadows, Andrew Nyman, Jon Lillie

Summary: This is a case report on the high-risk strategy of using V-A ECMO support in a pediatric heart transplant patient. The patient developed a massive pulmonary embolus during V-A ECMO treatment, but it was successfully resolved using ultrasound accelerated catheter directed thrombolysis. The patient went on to receive a heart transplant and had a favorable outcome.

Postnatal impact of a prenatally diagnosed double aortic arch

Trisha V. Vigneswaran, Milou P. M. Van Poppel, Benedict Griffiths, Paul James, Haran Jogeesvaran, Zehan Rahim, John M. Simpson, Simone Speggiorin, Vita Zidere, Andrew Nyman

Summary: This study retrospectively reviewed consecutive cases seen at two fetal cardiology units from 2014 to 2019, finding that prenatal diagnosis of double aortic arch (DAA) may lead to postnatal symptoms of airway compression, with significant tracheal compression present regardless of morphology type.
