Advances in telecommunications technology have facilitated telementoring initiatives that virtually link a mentor and mentee in the operating room and have shown significant promise in conferring high rates of safe procedure adoption. Recognizing that telementoring has the potential to be one solution to the global demand for what is sure to be a constant need for surgical retraining and safe incorporation of new technologies, the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES) convened the Project 6 Summit. Content experts in minimally invasive surgery, surgical mentoring and telementoring, surgical education, business development, healthcare innovation, and regulation were invited to attend a two-day summit to outline the current state of surgical telementoring and chart the challenges and opportunities going forward. Five opportunity buckets consisting of: Legal and Regulatory, Business Development and Proving Value, Effective Communication and Education Requirements, Technology Requirements and Logistics were the subject of focused working groups and subsequent review and consensus by summit attendees. The current state-of-the-art and guiding principles are presented herein. Telementoring activities are poised for exponential growth but will require a coordinated effort by stakeholders working through and around a healthcare system not yet suited for this paradigm shift.

Hemodilution in pediatric trauma: Defining the expected hemoglobin changes in patients with liver and/or spleen injury: An ATOMAC plus secondary analysis

Rachael L. Stottlemyre, David M. Notrica, Aaron S. Cohen, Lois W. Sayrs, Jessica Naiditch, Shawn D. St Peter, Charles M. Leys, Daniel J. Ostlie, R. Todd Maxson, Todd Ponsky, James W. Eubanks, Amina Bhatia, Cynthia Greenwell, Karla A. Lawson, Adam C. Alder, Jeremy Johnson, Erin Garvey

Summary: This study analyzed initial and follow-up measured hemoglobin values for children with BLSI with and without evidence of intra-abdominal bleeding. The results showed that decreases in hemoglobin are expected after trauma, even if not bleeding. Initial hemoglobin levels correlate with failure of non-operative management and transfusion, but subsequent hemoglobin levels do not guide the need for transfusion or correlate with failure of non-operative management.

Robert Baird, Pramod Puligandla, Steven Lopushinsky, Christopher Blackmore, Sanjay Krishnaswami, Benedict Nwomeh, Cynthia Downard, Todd Ponsky, Muhammad O. Ghani, Harold N. Lovvorn

Summary: The impact of a virtual Pediatric Surgery Bootcamp curriculum was evaluated in terms of resource utilization, learner engagement, knowledge retention, and stakeholder satisfaction. The virtual format reduced expenses, interfered less with schedules, reached a wider audience, and facilitated content archiving. However, learners and program directors still preferred in-person education.
