Purpose Several variations of the arterial blood supply of the orbit have been reported over the years. This review is aimed to provide an update focusing on three important issues: (a) variations of the ophthalmic artery origin; (b) contribution of the external carotid artery to the orbital blood supply; (c) orbital hemodynamic. Methods A PubMed and Google search was carried out with the following keywords: ophthalmic artery origin, ophthalmic artery anastomoses and ophthalmic artery anatomy. Results The site of origin of the ophthalmic artery displays a limited number of variations. However they are important as they are also associated with course variations. Anastomoses between the ophthalmic artery and the external carotid artery are numerous and many of them can acquire clinical relevance. Records on their anatomic frequency are limited. Orbital hemodynamic variations are a poorly studied subject. Recent investigations in children have unveiled unexpected variability and instability in the way the blood flows through the orbit. Conclusions The orbit shows several possible arterial variations. Some of them have a profound influence on its hemodynamic at least in children. More studies are required to ascertain if the hemodynamic variability observed in children can be pinpointed also in adults.

Summary: There is no consensus on the most effective endovascular technique for patients with acute ischemic stroke due to terminal internal carotid artery occlusion. This study compared the safety and efficacy of aspiration technique (AT) and combined technique (CT) as first-line approach, revealing that CT yielded better technical and functional outcomes.

Lymphatic Collecting Vessels in Health and Disease: A Review of Histopathological Modifications in Lymphedema

Elisabetta Weber, Margherita Agliano, Eugenio Bertelli, Guido Gabriele, Paolo Gennaro, Virginia Barone

Summary: Secondary lymphedema is a common side effect of oncological treatments, affecting the daily lives of patients and the healthcare system. Treatment involves surgical techniques to connect lymphatic vessels and veins, but successful results depend on the functional structure of the vessel. This article reviews the structure of lymphatic collecting vessels in health and in lymphedema, focusing on histopathological changes that affect the success of lymphatic-venular anastomosis.

Untangling the Extracellular Matrix of Idiopathic Epiretinal Membrane: A Path Winding among Structure, Interactomics and Translational Medicine

Laura Bianchi, Annalisa Altera, Virginia Barone, Denise Bonente, Tommaso Bacci, Elena De Benedetto, Luca Bini, Gian Marco Tosi, Federico Galvagni, Eugenio Bertelli

Summary: Idiopathic epiretinal membranes (iERMs) are fibrocellular sheets of tissue that develop at the vitreoretinal interface. The extracellular matrix (ECM) composed of various proteins plays a crucial role in matrix biology in iERMs. This study used a bioinformatics approach to analyze the interactome of key proteins in iERM, revealing interactions among ECM molecules, cell receptors, and intra- or extracellular proteins. The modulation of matrix deposition by biomarkers found in the vitreous humor of iERM patients is also discussed, along with the proposed techniques for pharmacologic vitreolysis in iERM treatment.

Summary: This study investigated the outcomes of AIS patients with tandem occlusion (TO) treated with carotid artery stenting (CAS) during endovascular thrombectomy (EVT). The results showed that CAS was associated with lower 3 months' mortality and higher rates of successful recanalization among TO-AIS patients.

Summary: Epiretinal membranes (ERMs) are pathological tissues that form in the vitreoretinal interface, leading to vision loss. Understanding the molecular dysfunctions that contribute to ERM development is crucial. Our bioinformatics study identified CD44 as a central regulator of ERM dynamics, and its interaction with PDPN could promote migration in epithelial cells. PDPN, overexpressed in various cancers, plays a relevant role in fibrotic and inflammatory pathologies, modulating signaling pathways involved in ERM formation. Understanding the role of PDPN may open new therapeutic options for fibrosis.

Mechanical thrombectomy for in-hospital stroke: data from the Italian Registry of Endovascular Treatment in Acute Stroke

Andrea Naldi, Giovanni Pracucci, Roberto Cavallo, Valentina Saia, Andrea Boghi, Piergiorgio Lochner, Ilaria Casetta, Fabrizio Sallustio, Andrea Zini, Enrico Fainardi, Manuel Cappellari, Rossana Tassi, Sandra Bracco, Guido Bigliardi, Stefano Vallone, Patrizia Nencini, Mauro Bergui, Salvatore Mangiafico, Danilo Toni

Summary: This study aimed to evaluate the outcomes, safety, and time intervals of mechanical thrombectomy (MT) in patients with in-hospital stroke (IHS) compared with out-of-hospital stroke (OHS) patients. The results showed that IHS patients had worse functional outcomes and higher mortality at 3 months. However, the recanalization rates and risk of symptomatic intracranial hemorrhage were similar. Improvements in the management and shorter time intervals are needed for IHS patients.

Summary: In this study, a sensitive and reproducible LC-MS/MS method was developed and validated for the quantification of short-chain fatty acids (SCFAs) in human, rat, and mouse plasma. The method showed good performance in terms of recovery (> 80%), precision (RSD <14%), accuracy (RE < ± 10%), and sensitivity (LOQ of 0.01 μM for acetic, butyric, propionic, and isobutyric acid). The results highlighted the correlation between plasma concentrations of SCFAs and age, emphasizing the importance of developing a reliable method for their quantification to study their biological role.

Clinical anatomy of the spina musculi recti lateralis: A frequently overlooked variation of the greater wing of the sphenoid

Denise Bonente, Virginia Barone, Vitaliano Francesco Muzii, Sara Ottolenghi, Miriam Durante, Sandra Bracco, Claudio Nicoletti, Eugenio Bertelli

Summary: This study characterizes the morphology and topography of the spina musculi recti lateralis (SMRL) and compares it to known bony landmarks. The results show that the SMRL is a process of the sphenoidal orbital plate and serves as a reliable landmark for the insertion of the tendinous ring and lateral rectus. This is an important variation for orbital surgeons to be aware of.
