Photobiomodulation therapy: management of mucosal necrosis of the oropharynx in previously treated head and neck cancer patients

Necrosis of the oral mucosa following head and neck cancer radiation therapy presents considerable clinical management challenges. We report three cases of symptomatic persisting oral ulcerations where the addition of photobiomodulation therapy resulted in a rapid resolution of the oral lesions and in patient symptoms. These cases suggest that photobiomodulation may represent an adjunct to care of these difficult to manage complications in oncology.

Summary: This study investigated protein differential expression in HPV(+) OPSCC samples and found that functional pathways involved in cell death, apoptosis, WNT signaling, and Rho GTPase signaling play important roles in disease recurrence. Downregulation of certain molecular targets is associated with recurrence and may serve as markers for high-risk disease. Immunohistochemistry staining validated some of the mass spectrometry findings.

Summary: This study aimed to compare the financial hardship of patients with head and neck cancer (HNC) to patients with other cancers, focusing on the domains of coping behaviors and psychologic response. The results showed that patients with HNC had higher levels of hardship in coping behaviors but similar levels in psychologic response compared to patients with other cancers.

Summary: The study confirms that lymphovascular invasion and histologic grade are the strongest predictors of neck lymph node metastasis in cN0 patients with pharynx and larynx cancer. Neck dissection is still necessary for cN0 patients to identify the risk of lymph node metastasis even in the modern era.

Summary: This study investigated the impact of histologic variants and grade on the prognosis of salivary cancer. The results showed that histologic variants had different mortality risks, but these differences were diminished in multivariable analysis. In contrast, worsening grade was strongly associated with deteriorating survival. Integrating grade into stage can improve the predictive ability of the current staging system.

Re-examining predictors of pathologic lymph node positivity in clinically node negative oral cavity cancer

Summary: Elective lymph node dissection (ELND) is commonly performed for early-stage oral cavity squamous cell carcinomas (OCSCC) with clinically negative necks. However, the validity of depth of invasion (DOI) as a predictor of lymph node positivity (pN+) varies in different OC sites. This study found that lymphovascular invasion and histologic grade were strong independent predictors of pN+ in cN0 OCSCC, while DOI was not associated with pN+ in the overall OCSCC population, but showed some predictive value in the oral tongue subset.

They also found that these features co-occur in other cancers originating from the same cell types.
