Prevalence and Incidence of Osteoporosis and Osteoporotic Vertebral Fracture in Korea Nationwide Epidemiological Study Focusing on Differences in Socioeconomic Status

Study Design.A cross-national study.Objective.To determine the prevalence and incidence of osteoporosis (OP) and osteoporotic vertebral fracture (OVF) in Korea and to investigate if socioeconomic status has an effect.Summary of Background Data.As life expectancy increases, OP and related fragility fractures are also increasing. This presents a serious challenge, not only for health authorities but also for individuals, their families, and society overall. Determining the prevalence and incidence of OP and related fragility fractures is the first step in developing strategies to reduce their increasing disease burdens. Concurrently, we need to confirm whether people with low socioeconomic status are more susceptible to these diseases.Methods.Using the Health Insurance Review and Assessment Service (HIRA) database from 2008 to 2012, we estimated the annual prevalence and incidence of OP and OVF and investigated the differences according to socioeconomic status by National Health Insurance (NHI) beneficiaries and Medical Aid (MA) recipients.Results.In 2012, the standardized prevalence of OP in the NHI and MA groups was 3968 and 6927 per 100,000, respectively (odds ratio, 3.83). The standardized incidence of OP in the MA group was significantly higher than in the NHI group in 2011 and 2012 (odds ratios, 2.34 and 2.19, respectively). In addition, the standardized incidence of OVF in the MA group in 2011 and 2012 was 408 and 389 per 100,000, respectively, and the incidence in the MA group was significantly higher than in the NHI group (odds ratios, 4.13 and 4.12, respectively; P<0.001).Conclusion.We confirmed that the prevalence and incidence of OP and OVF in the MA group were higher than those in the NHI group. It showed that low income might be a significant factor related to OP and OVF.Level of Evidence: 3

Does Graft Position Affect Subsidence After Anterior Cervical Discectomy and Fusion?

Summary: This retrospective cohort study investigated the effect of graft position on subsidence after anterior cervical discectomy and fusion (ACDF). The study found that placing the graft in the anterior position of the surgical-level endplate reduces the risk of subsidence. Smaller graft footprint size and pseudarthrosis were also found to be associated with subsidence.

The general public's perspectives on telemedicine during the COVID-19 pandemic in Korea: analysis of a nationwide survey

Predictive risk factors for mechanical complications after multilevel posterior cervical instrumented fusion

Summary: This study investigated the incidence of mechanical complications following multilevel posterior cervical instrumented fusion surgery. The significant risk factors for mechanical complications were low bone mineral density, a large number of fused vertebrae, a large preoperative C2-7 sagittal vertical axis, and low preoperative C2-7 lordosis. The findings of this study could be valuable for preoperative counseling and surgical planning.

Prevalence and severity of COVID-19 among children and adolescents with autism spectrum disorders in the Republic of Korea

Summary: This study investigates the trends of COVID-19 among children and adolescents with and without autism spectrum disorder, and evaluates the differences in prevalence, severity, and case fatality rate. The study finds that while the COVID-19 prevalence is lower among individuals with autism spectrum disorder, they have higher hospitalization rates and severity. However, the length of hospital stay is not different between the two groups.

Trends in National Pharmaceutical Expenditure in Korea during 2011-2020

Summary: This study analyzed the trends in pharmaceutical expenditure, the share of pharmaceutical expenditure in health expenditure, and the expenditure by pharmacological groups in Korea. The results showed an increase in pharmaceutical expenditure over the 10-year period, indicating the need for macroscopic management. The findings on expenditure by pharmacological groups can assist in future policy-making.

Clinical Effect of Transverse Process Hook with K-Means Clustering-Based Stratification of Computed Tomography Hounsfield Unit at Upper Instrumented Vertebra Level in Adult Spinal Deformity Patients

Summary: This study investigated the efficacy of the UIV hook system in preventing screw pullout in adult spinal deformity surgery using the pedicle Hounsfield unit (HU) stratification based on K-means clustering. The results showed that the UIV hook system can significantly decrease the pullout rate within a certain range of pedicle HU, and the postoperative clinical outcomes improved significantly with the implementation of the hook system.

National Trends in Hospitalization for Ambulatory Care Sensitive Conditions among Korean Adults between 2008 and 2019

Summary: This study examines the trends in hospitalization rates for ambulatory care sensitive conditions (ACSC) and identifies factors affecting hospitalization over a 12-year period. The results show differences in hospitalization rates among different conditions and insurance types. Policy reforms are needed to reduce hospitalization for heart failure, pneumonia, and urinary tract infection, especially among Medical Aid patients.

Comparison of Whole Spine Sagittal Alignment in Patients with Spinal Disease between EOS Imaging System versus Conventional Whole Spine X-ray

Summary: The study compared the sagittal alignment differences between the biplanar whole body imaging system (EOS) and conventional whole spine X-ray (WSX). In sagittally imbalanced patients, EOS showed smaller pelvic tilt (PT), greater sacral slope (SS), and more upright posture compared to WSX. However, in sagittally balanced patients, these differences were less pronounced. Surgeons should take these differences into account when planning surgical interventions.

Endplate-specific fusion rate 1 year after surgery for two-level anterior cervical discectomy and fusion(ACDF)

Summary: This study found that in double-level ACDF procedures, the nonfusion rate was higher at the caudal fusion levels, especially at the lower endplates of the caudal fusion levels. There was no difference in radiological parameters and clinical outcomes between the fusion and pseudarthrosis groups.

Reimbursement of Digital Therapeutics: Future Perspectives in Korea

Summary: Digital health is rapidly growing worldwide, and digital therapeutics (DTx) are expanding their scope from wellness to treatment. DTx provides evidence-based therapeutic interventions and is generally the same internationally. It receives support in the form of state subsidies and national health insurance coverage in other countries. The pricing and coverage of DTx are determined based on clinical effects and existing technology. It is expected that DTx for various diseases will be developed in the near future, increasing the demand and need for proper support systems and health insurance coverage listings.

Preliminary Results of Teleconsultations Temporarily Allowed during the COVID-19 Pandemic
