Summary: Left atrial (LA) remodeling is closely associated with cerebral stroke. However, the relationship between early-stage impaired deformability of the left atrium and stroke/transient ischemic attack (TIA) remains unclear. This study used speckle tracking echocardiography to evaluate changes in LA deformability and found that LA strain rate during left ventricular early diastole (SRE) was lower in the stroke/TIA group compared to the non-stroke/TIA group. Multiple regression analysis showed that SRE had a linear dependence on E/A ratio, LA volume index (VI), and body mass index, and SRE and sex ratio were independent risk factors for stroke/TIA.

Summary: This study aimed to compare the risk of stroke recurrence between known atrial fibrillation before stroke (KAF) and atrial fibrillation detected after stroke (AFDAS) in patients with anticoagulation, and to investigate the impact of pre-existing anticoagulation on this risk difference. The study found that only pre-existing anticoagulation, not KAF, was independently associated with a higher risk of stroke recurrence. Therefore, future research should focus on the causes of stroke despite anticoagulation to develop improved preventive treatments.

Improvements were seen in stroke diagnostics and secondary prevention treatments, as well as a decrease in discharge against medical advice and in-hospital mortality. Disparities between rural and urban hospitals narrowed, but differences in management and outcomes persisted.

Summary: This review examines the effects of omega-3 fatty acids (n3-FAs) on cardiovascular risk reduction. It has been found that high-purity EPA ethyl ester has significant benefits in reducing cardiovascular events and atherosclerosis progression compared to mixed EPA/DHA treatments. Mechanistic differences between n3-FAs, including membrane interactions, metabolic products, cholesterol efflux, antioxidant properties, and tissue distribution, have been explored.

Summary: Mild behavioral impairment (MBI) is a syndrome that uses neuropsychiatric symptoms (NPS) to identify individuals at high risk for dementia. The study used rs-fMRI to investigate the functional connectivity (FC) within the default mode network (DMN) and salience network (SN) in MBI. The results showed reduced FC within the DMN and SN in MBI patients, providing insights into the early mechanisms of the disease.

Summary: Hybrid ablation is a novel therapy that combines minimally invasive surgical and percutaneous catheter-based techniques for the management of atrial fibrillation (AF). Observational studies have shown approximately 70% success rates but higher risks compared to catheter-based ablation. Hybrid ablation is typically offered to patients with persistent or longstanding persistent AF, requiring two procedures. While a randomized trial demonstrated its superiority over catheter-based ablation, it also showed higher complication rates. Contemporary studies have shown a decrease in the incidence of serious complications. Additional randomized trials are needed to determine the risks, benefits, and cost effectiveness of hybrid ablation in clinical practice.

Summary: The burden of heart failure is increasing, and there is geographic variation in the quality of care for heart failure with reduced ejection fraction. This study evaluates the prescription of guideline-directed medical therapies (GDMT) within the Veterans Health Administration and finds significant variation across regions. Targeted strategies are needed to improve GDMT prescription in lower-performing regions, especially those affected by income inequality and poverty.
