Summary: TC response to adjustment of anti-TNFa treatment can be predicted for adalimumab but not for infliximab. Decreasing interval for adalimumab results in increased TC, while increasing interval for adalimumab and dose increase for infliximab both lead to decreased TC.

Summary: The study evaluated the relationship between serum infliximab concentrations during induction and short-term clinical remission in children with ulcerative colitis. Higher infliximab concentrations prior to the second dose were associated with week 8 clinical remission. Young age and low body weight impact infliximab concentration, prompting the need for proactive adjustment in pediatric patients with ulcerative colitis.

Summary: In patients with steroid-refractory ASUC responding to second-line medical therapy, the process of endoscopic remission started with bleeding remission and was not achieved in half the patients at day 98 for vascular pattern. Infliximab provided a higher endoscopic remission rate than cyclosporine at day 98.

Summary: ASUC, a complication of ulcerative colitis associated with high levels of circulating tumor necrosis factor alpha, can be treated with intensified infliximab regimen to avoid colectomy. This case study demonstrated successful treatment of ASUC and megacolon using a combination of hydrocortisone and accelerated infliximab infusions, leading to positive clinical response without surgical intervention. Longitudinal studies are needed to determine the optimal therapeutic monitoring and treatment for ASUC patients.

An adalimumab level of >= 8.25 mu g/mL is associated with deep remission, providing additional data to guide therapeutic drug monitoring with adalimumab.

Summary: This study investigated the relationship between expirium air carbon monoxide (E-CO) levels and disease severity in patients with ulcerative colitis (UC) and Crohn's disease (CD). It was found that the severity of UC decreased with higher E-CO levels and the mean number of cigarettes smoked, while the severity of CD increased in line with the mean number of cigarettes smoked.

Summary: This study aimed to investigate the diagnostic accuracy of UCEIS and MES in predicting response to biological therapy and the need for colectomy in patients with ulcerative colitis. The results showed that UCEIS was more accurate in predicting therapy unresponsiveness, and when UCEIS was >= 7, patients had a higher risk of undergoing colectomy within a shorter time frame.

Summary: Loss of response to infliximab or adalimumab in ulcerative colitis occurs frequently, with an annual rate of 10% for infliximab and 13% for adalimumab. Dose escalation rates were higher than loss of response, with 72% and 52% regaining clinical benefit for infliximab and adalimumab, respectively. Uniform definitions are needed for more robust evaluations.

Summary: This review aims to analyze the population PK models of infliximab (IFX) in patients with ulcerative colitis (UC) and acute severe UC (ASUC), and assess the impact of disease activity on IFX PK. The results showed that there are still many gaps in the current research, such as a limited number of models specifically developed for UC patients, a lack of studies on severe UC patients, and a lack of evaluation of predictive performance for the models. Therefore, more comprehensive research and adequate reporting and evaluation of the models are needed for clinical application.

Summary: This study investigated the incidence, severity, and risk factors of post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) diagnosed according to the diagnostic criteria and classification for acute pancreatitis (AP), revealing a higher incidence of PEP and a tendency towards severe cases compared to the Cotton criteria. Female sex, naive papilla, surgically-altered gastrointestinal anatomy, procedure time after reaching the papilla, pancreatic duct injection, and intraductal ultrasonography were identified as independent risk factors for PEP.
