Summary: A 79-year-old male patient with chronic coronary syndrome and complex coronary artery disease was enrolled in a clinical trial assessing the use of coronary computed tomography angiography (CCTA) and CCTA-derived fractional flow reserve (FFRCT) for surgical revascularization. Discordance between CCTA and initial FFRCT results required further analysis using invasive and non-invasive coronary angiography. The findings revealed that the stenosis in the proximal left anterior descending artery (LAD) was physiologically significant in angiography-derived fractional flow reserve (FFR), while it remained functionally negative in the second assessment of FFRCT, possibly due to extensive calcification.

Summary: This study assessed the long-term utilization of beta blockers and its association with outcomes in patients undergoing coronary artery bypass grafting (CABG). The results showed that ongoing treatment with cardioselective beta blockers after CABG was associated with a reduction in major adverse cardiovascular events (MACEs), primarily due to a reduced risk of myocardial infarction (MI). This association was consistent in patients with and without previous MI, heart failure, atrial fibrillation, or reduced left ventricular ejection fraction (LVEF).

Summary: The association between graft patency and clinical outcomes is more complex and variable than commonly thought, influenced by factors such as graft type, occlusion timing, myocardium at risk, and patient characteristics. Graft occlusion is often linked to non-fatal events, with graft failure from competitive flow being benign while occlusion in graft-dependent circulation causing clinical symptoms.

Summary: This article describes a rare case of Kawasaki disease-related coronary artery aneurysms in an elderly patient. The case involves multiple giant coronary artery aneurysms, which is less common and associated with ischemic symptoms, making treatment challenging.

Summary: A systematic review and pooled analysis of high-quality individual patient data from CABG trials found that women have worse outcomes than men in the first 5 years after surgery, with higher rates of cardiac events and repeat revascularization. However, this difference is not significant in patients aged over 75 years and is not affected by the surgical technique.

Summary: This study compared the postoperative surgical outcomes of high-complexity surgery (coronary artery bypass grafting) in HPSA and non-HPSA designated areas. The results showed that patients living in HPSAs had comparable surgical outcomes but had a significantly greater travel burden.

Summary: Obesity does not predict increased all-cause in-hospital mortality in patients undergoing CABG procedure. Therefore, CABG is a safe procedure for overweight patients.

Summary: Comparative outcomes for patients with reduced ejection fraction undergoing CABG vs PCI showed that CABG was associated with improved long-term survival, reduced MACCE events, and fewer repeat revascularization procedures.

Summary: This study aimed to evaluate the incidence and risk factors for acute kidney injury (AKI) following postoperative coronary angiography in patients who underwent coronary artery bypass grafting (CABG). The multivariable analysis identified postoperative peak values of CK-MB, EuroSCORE I, and AKI before re-angiography as independent predictors for AKI, while delayed angiography was found to be protective against AKI. Patients with post-angiography AKI had a significantly higher mortality rate than those without AKI.

Summary: New-onset postoperative atrial fibrillation (POAF) after coronary artery bypass grafting is associated with increased risks of ischemic stroke, thromboembolism, heart failure hospitalization, and recurrent atrial fibrillation, but not all-cause mortality. Early initiation of oral anticoagulation (OAC) does not reduce the risk of ischemic stroke or thromboembolism, but instead increases the risk of major bleeding.

Summary: Coronary angiography is crucial for diagnosing coronary heart disease, but has limitations in identifying hemodynamic changes. FFR technique, when combined with coronary angiography, allows for both functional and anatomical assessment of heart disease. The use of FFR in PCI and CABG may impact treatment outcomes and surgical collateralization.

Summary: This retrospective cohort study aimed to determine the influence of reexploration for bleeding and blood product requirement after coronary artery bypass grafting (CABG) on long-term mortality. The results showed that reexploration for bleeding was significantly associated with higher 30-day mortality but not with long-term mortality. Poorer long-term mortality was attributed to patient characteristics and higher use of postoperative blood products.
