Summary: This study aimed to elucidate the roles of Tregs and their subgroups in primary Sjo & BULL;gren syndrome (pSS). The results showed that the percentage of rTregs was increased in pSS patients and decreased after treatment, while the percentage of aTregs increased after treatment. In patients with higher disease activity, the percentage of rTregs decreased after treatment. Compared to the control group, the percentage of rTregs among Tregs was increased in pSS patients and decreased after treatment.

Summary: The study aimed to determine characteristics and prognosis of pSS-ILD patients, and to identify risk factors for mortality. Among the pSS-ILD patients, majority were women, with NSIP being the most common HRCT pattern. Impairment in diffusion capacity was the most common pulmonary function test abnormality. Age, smoking, and severity of lung involvement were identified as risk factors for mortality.

Summary: This case report presents a 72-year-old male patient with salivary gland involvement in ANCA-associated vasculitis, which mimics Sjogren syndrome. The patient presented with parotid and submandibular gland pain and swelling, accompanied by dry mouth and eyes. Laboratory tests showed positive ANCA, hematuria, and proteinuria, but negative Anti-Ro and -La. The patient was treated with corticosteroids and cyclophosphamide for acute kidney injury, but unfortunately, he passed away a few months later.

Summary: In 1933, the Swedish ophthalmologist Hendrik Sjbgren proposed that the dryness symptoms his patients were experiencing could be related to a systemic disease and abnormal immune response. Over the past 20 years, significant progress has been made in understanding the epidemiological determinants and pathogenic mechanisms of Sjbgren's syndrome (SjS), leading to the development of personalized therapeutic strategies. SjS is now recognized as one of the most common systemic autoimmune diseases, and the identification of prognostic factors allows for personalized follow-up and early interventions to prevent severe outcomes.

The clinical course of cryoglobulinemic vasculitis is usually mild, but severe organ damage and life-threatening manifestations can occur.

Summary: This survey aimed to understand the future work model of general practitioners and the changes in primary healthcare centers due to the COVID-19 pandemic. The results showed that a majority of family doctors felt they spent too much time on administrative tasks, opposed call center systems, believed in their responsibility for home care, supported continuity of care as a fundamental value, and advocated for primary healthcare to provide complementary tests and first hospital visits. The survey responses provide important insights for the future development of primary healthcare.

Summary: This study evaluated the humoral and cellular immune response in systemic lupus erythematosus (SLE) patients treated with Belimumab after receiving three doses of SARS-CoV-2 mRNA vaccines. The results showed that all patients had a humoral response after the first dose, but the antibody titer significantly declined after the second dose. However, the third dose led to a significant increase in antibody titer, restoring it to the level before vaccination. The cellular-mediated response declined after the second dose but did not further decline after the third dose. There were no significant differences in humoral and cellular responses between patients and controls.

Summary: This study investigated the prevalence and outcome of COVID-19 in a large series of MCs patients, as well as the safety and immunogenicity of COVID-19 vaccines. The results showed a significantly higher prevalence of COVID-19 in MCs patients compared to the general population, and the use of immunomodulators was associated with an increased risk of infection. Additionally, MCs patients with COVID-19 had a higher mortality rate, and older age was correlated with worse COVID-19 outcomes. The vaccination immunogenicity was impaired in MCs patients, and certain immunomodulators hindered the vaccine-induced immune response.

Summary: This article provides an updated overview of the hematological involvement associated with sarcoidosis, including the management approach for cytopenias and the association with hematological malignancies. The etiology of cytopenias in sarcoidosis can be attributed to infiltration of hematopoietic organs and autoimmune-mediated cytopenias. The association between sarcoidosis and hematologic involvement is best understood as a pathogenic continuum, where cytopenias and hematologic neoplasms are intertwined due to various etiopathogenic mechanisms.
