Alternative physical treatments for deep venous thrombosis prophylaxis in surgical patients: a systematic review

Summary: In addition to early mobilisation and graduated compression stockings, various non-pharmacological physical therapies have shown prophylactic benefits for preventing lower limb deep venous thrombosis (DVT) after surgery and trauma. These include electrostimulation, portable automatic pneumatic compression, and manual calf massage. Our findings indicate that these interventions only have a statistically significant benefit when used in combination with low molecular weight heparin (LMWH). There is no evidence to support the use of these interventions alone for DVT prophylaxis, especially in patients with a higher risk of bleeding who may not be able to use LMWH.

Summary: Fibrosis of visceral organs such as the lungs, heart, kidneys, and liver is a major cause of morbidity and mortality, and is also associated with other disorders like cancer and metabolic disease. Collagen VI, particularly in conditions like IPF and chronic liver and kidney diseases, has elevated expression and its chain fragments are emerging as important biomarkers for fibrotic conditions.

A vasculature niche orchestrates stromal cell phenotype through PDGF signaling: Importance in human fibrotic disease

Summary: Fibrosis, characterized by excessive accumulation of matrix protein, has significant impact on health in the Western world. However, there is a lack of effective antifibrotic treatments due to the difficulty in identifying targets for human fibrosis. In this study, researchers used Dupuytren's disease as a model to analyze the vasculature niche of human fibrosis at a single-cell resolution. The results showed distinct populations of endothelial and pericyte cells, and revealed a complex topological organization in the fibrotic microenvironment. It was found that endothelium promotes immune regulatory fibroblast phenotype through platelet-derived growth factor signaling, leading to an immune cell-enriched perivascular niche. Additionally, pericytes were found to house a putative myofibroblast precursor. These findings provide valuable insights for fibrosis research and highlight the therapeutic potential of targeting blood vessel signaling in human fibrosis.

Treatments for early-stage Dupuytren's disease: an evidence-based approach

Summary: Current treatments for Dupuytren's disease are limited to late-stage disease when patients have developed flexion contractures and have impaired hand function. These treatments have limitations and the use of treatments for early-stage disease lack clear biological basis or evidence of effectiveness. A detailed understanding of the cellular landscape and molecular signalling in nodules of early-stage disease would permit the identification of potential therapeutic targets.

Dupuytren's disease: a localised and accessible human fibrotic disorder

Summary: We discuss the biology of Dupuytren's disease (DD), a common fibrotic disorder of the hand. The disease develops through the interaction of genetic and environmental factors, as well as epigenetic signaling. The cellular and molecular pathways in DD can be easily studied due to the accessibility of tissue in affected patients, and this research may have implications for understanding fibrotic tissues in other parts of the body.

Summary: Clinical practice guidelines aim to provide evidence-based care and standardize treatment to improve patient outcomes. However, there are limited national guidelines for the management of open fractures, especially in low- or middle-income countries. International collaboration may be a solution to adapt guidelines to different resources and healthcare systems across borders.

Repeated courses of adalimumab are likely to be a cost-effective treatment option for progressive early-stage Dupuytren's disease.
