Summary: This study compared the outcomes of immediate autologous breast reconstruction and delayed-immediate reconstruction. The study found that the immediate reconstruction group had more complications such as mastectomy skin flap necrosis, while the delayed-immediate reconstruction group had more complications such as infection and readmission.

Summary: Delayed-immediate autologous breast reconstruction is a safe and flexible option for patients undergoing postmastectomy radiation therapy. This study compared the outcomes of tissue expander placement strategies in these patients and found that using subpectoral placement resulted in a higher need for skin and flaps.

Summary: This study compared the postoperative complication rates between immediate (IBR), delayed immediate (DIBR), and delayed (DBR) breast reconstruction modalities. The results showed that DIBR and DBR were associated with lower overall complication rates compared to IBR. Physicians should consider patients' unique circumstances when deciding upon the appropriate timing modality.

Summary: This study presents a novel technique for nipple-sparing mastectomy and immediate breast reconstruction in patients with large ptotic breasts. The technique involves using the pectoral muscle and an autologous dermal flap to cover the implant/tissue expander, eliminating the use of acellular dermal matrix. The results showed successful outcomes with no major complications and only minor complications that were managed conservatively.

Summary: Postmastectomy radiation therapy (PMRT) is crucial in treating locally advanced breast cancer. The timing of therapy in relation to autologous breast reconstruction (ABR) remains controversial. Immediate ABR (I-ABR) patients had higher rates of complications such as fat necrosis and skin necrosis, while delayed-immediate ABR (DI-ABR) patients experienced complications following tissue expander placement, with a significant percentage requiring reoperation.

Summary: The study reviewed flap outcomes in women who underwent simultaneous mastectomy-autologous reconstruction with PMRT, showing that patients who received surgery within 3 months of PMRT had higher rates of complications and reoperations. IMN radiation was associated with increased risk.

Summary: This study evaluated patients who chose DIEP flap as the reconstructive method during initial consultation and compared characteristics of surgery in relation to their final reconstructive choice. Delayed-immediate DIEP reconstruction has advantages over immediate DIEP flap, including shorter free flap operative times. Patients commonly change their preference for second stage reconstruction.

Further research is needed to determine the role of PMRT and reconstruction surgical technique in the incidence of complications between immediate and delayed reconstructions.

Summary: This study aimed to compare the effects of different crosslinking methods on autologous fat grafting. It was found that ex situ crosslinking using 3D bioprinting was more effective in maintaining the shape stability of the grafts, while in situ crosslinking, although improving fat cell distribution and survival, had poorer shape retention.

Summary: The oncological safety of different types and timings of postmastectomy breast reconstruction (PMBR) remains controversial. This study found that delayed autologous PMBR leads to similar (loco)regional recurrence rates as immediate autologous PMBR. Current evidence suggests that oncological concerns should not be a reason to withhold patients from certain reconstructive timings or techniques, and patients should be offered all technically qualified reconstructive options equally.

Summary: The study aimed to estimate the cumulative risks of all cancers in women aged 50 to 75 with a BRCA1 or BRCA2 gene mutation. The results showed that women with BRCA1 or BRCA2 mutations have a high risk of cancer, particularly breast and ovarian cancer, between the ages of 50 and 75. Therefore, appropriate counseling should be provided to these women.

Summary: The objective of this study was to evaluate the pathogenic variant yield for nine cancer predisposition genes by testing criteria. Results showed that of the 1006 women studied, 100 women (9.9%) were found to have a pathogenic variant in one of the nine genes tested. The highest prevalence of pathogenic variants was found in women with triple-negative breast cancer (23%).

Summary: This study aimed to evaluate the effect of a theory-based behavioral intervention delivered by genetic counselors on the uptake of risk-reducing salpingo-oophorectomy (RRSO) by women with BRCA1 or BRCA2 pathogenic variants (PVs) compared to usual care. Findings showed that the intervention significantly reduced decisional conflict and increased knowledge at one-year, and resulted in a higher proportion of RRSO at two years compared to usual care.

Summary: Individuals with hereditary cancer syndrome have increased risk of ovarian, breast, and other types of cancers. However, most high-risk individuals in Canada are unaware of their risk. To address this issue, the British Columbia Gynecologic Cancer Initiative organized a research summit to explore the potential of population-based genetic testing in preventing hereditary cancer and reducing morbidity and mortality.

Summary: This review evaluates the psychological and health behavior outcomes associated with multi-gene panel testing for hereditary breast and ovarian cancer (HBOC) risk. Individuals with a pathogenic variant (PV) reported higher levels of testing-related concerns, cancer-specific distress, and uptake of prophylactic surgery compared to those with variant of uncertain significance (VUS) or negative result. The impact of gene penetrance on patient-reported and clinical outcomes remains limited and conflicting.

Summary: This study is the first to assess the psychological impact of reflex BRCA1/2 tumor genetic testing in women newly diagnosed with high-grade serous ovarian cancer (HGSOC). The results show that increased dispositional optimism provides a protective effect, while increased cancer-related distress increases the psychological impact of tumor genetic testing. Educational resources are needed to help improve patient understanding and recall of tumor results.
