Summary: Patients with COPD and PAH treated with bosentan combined with sildenafil showed improved PAH, HR, cardiac function, pulmonary function, and blood gas indexes compared to those treated with bosentan combined with iloprost. They also had lower levels of inflammatory factors, fewer adverse reactions, higher ORR, and greater satisfaction levels.

Three patients died within 48-72 hours. Overall, oral Sildenafil was effective in treating PPHN, and the combination with Bosentan showed better results in some patients.

The Kids Mod PAH Trial aims to investigate the efficacy of upfront combination therapy in improving PH outcomes in children and establish a clinical trials network to pave the way for future research.

Summary: This study compared the effects of different medications for pulmonary arterial hypertension (PAH) through a network meta-analysis. The findings suggest that combination therapy with endothelin receptor antagonist (ERA) and phosphodiesterase-5 inhibitors (PDE5i), as well as monotherapy with riociguat, can reduce clinical worsening in PAH patients. Additionally, PDE5i, intravenous/subcutaneous prostanoids, and riociguat may decrease mortality compared to placebo. Combination therapy with ERA+PDE5i and riociguat may also increase 6-min walk distance in PAH patients.

Summary: The study demonstrated that SIL can revert established cardiac hypertrophy in SHR by inhibiting NHE1 activity and reducing its phosphorylation level through modulation of the PKG-p38 MAP kinase-PP2A signaling pathway. Additionally, SIL was found to decrease oxidative stress and the pro-hypertrophic phosphatase calcineurin levels, thereby alleviating myocardial hypertrophy and interstitial fibrosis.

Summary: Pulmonary arterial hypertension (PAH) is a progressive disease with a 7-year survival rate of approximately 50%, despite advancements in therapy. Risk factors for developing PAH include methamphetamine use, scleroderma, human immunodeficiency virus, portal hypertension, and genetic predisposition. PAH is characterized by impaired vasodilation, enhanced vasoconstriction, and proliferation in the pulmonary vasculature, involving pathways such as nitric oxide, prostacyclin, thromboxane A2, and endothelin-1. This paper discusses novel drugs targeting new and alternative pathways for treating PAH, in addition to the traditional pathways targeted by established PAH medications.
