Macular versus Retinal Nerve Fiber Layer Parameters for Diagnosing Manifest Glaucoma A Systematic Review of Diagnostic Accuracy Studies

Topic: Macular parameters have been proposed as an alternative to retinal nerve fiber layer (RNFL) parameters to diagnose glaucoma. Comparing the diagnostic accuracy of macular parameters, specifically the ganglion cell complex (GCC) and ganglion cell inner plexiform layer (GCIPL), with the accuracy of RNFL parameters for detecting manifest glaucoma is important to guide clinical practice and future research. Methods: Studies using spectral domain optical coherence tomography (SD OCT) and reporting macular parameters were included if they allowed the extraction of accuracy data for diagnosing manifest glaucoma, as confirmed with automated perimetry or a clinician's optic nerve head (ONH) assessment. Cross-sectional cohort studies and case-control studies were included. The QUADAS 2 tool was used to assess methodological quality. Only direct comparisons of macular versus RNFL parameters (i.e., in the same study) were conducted. Summary sensitivity and specificity of each macular or RNFL parameter were reported, and the relative diagnostic odds ratio (DOR) was calculated in hierarchical summary receiver operating characteristic (HSROC) models to compare them. Results: Thirty-four studies investigated macular parameters using RTVue OCT (Optovue Inc., Fremont, CA) (19 studies, 3094 subjects), Cirrus OCT (Carl Zeiss Meditec Inc., Dublin, CA) (14 studies, 2164 subjects), or 3D Topcon OCT (Topcon, Inc., Tokyo, Japan) (4 studies, 522 subjects). Thirty-two of these studies allowed comparisons between macular and RNFL parameters. Studies generally reported sensitivities at fixed specificities, more commonly 0.90 or 0.95, with sensitivities of most best-performing parameters between 0.65 and 0.75. For all OCT devices, compared with RNFL parameters, macular parameters were similarly or slightly less accurate for detecting glaucoma at the highest reported specificity, which was confirmed in analyses at the lowest specificity. Included studies suffered from limitations, especially the case-control study design, which is known to overestimate accuracy. However, this flaw is less relevant as a source of bias in direct comparisons conducted within studies. Conclusions: With the use of OCT, RNFL parameters are still preferable to macular parameters for diagnosing manifest glaucoma, but the differences are small. Because of high heterogeneity, direct comparative or randomized studies of OCT devices or OCT parameters and diagnostic strategies are essential. (C) 2016 by the American Academy of Ophthalmology.

Posterior stromal ripples increase risk of Descemet's membrane endothelial keratoplasty graft detachment worsening over time

Summary: By evaluating the features of anterior segment optical coherence tomography (AS-OCT), it was found that posterior stromal ripples and detachment axial extension are the main risk factors for graft detachment worsening in Descemet's membrane endothelial keratoplasty (DMEK). Patients showing these features should be closely monitored to identify the need for re-bubbling at an early stage, thus improving surgical outcomes.

Presumed sympathetic ophthalmia following scleral buckling surgery: A case report and review of the literature

Effectiveness and Safety of XEN45 in Eyes With High Myopia and Open Angle Glaucoma

Targeting immunoproteasome in neurodegeneration: A glance to the future

Summary: The immunoproteasome, initially discovered for its role in MHC class I antigen processing and immune system modulation, has been found to have additional functions unrelated to antigen presentation. Its expression and activity alterations are important in understanding neuroinflammation dynamics and the pathogenesis of neurological disorders. Furthermore, these new functions suggest the potential of immunoproteasome as a therapeutic target for neurodegeneration. This review provides an overview of the structure-function relationships of immunoproteasome and its observed neuro-modulatory functions.

Heterogeneity of Ocular Hemodynamic Biomarkers among Open Angle Glaucoma Patients of African and European Descent

Summary: This study examined the differences in ocular hemodynamic biomarkers between early open angle glaucoma (OAG) patients and healthy controls of African and European descent. The results showed that vascular densities were significantly lower in European OAG patients, while central macular vascular density was lower in African OAG patients. Macular and parafoveal thickness were also significantly lower in African OAG patients compared to European patients. Additionally, there was a negative correlation between intraocular pressure (IOP) and visual field index in African OAG patients, while a slightly positive relationship was observed in European patients.

Summary: This study aimed to evaluate the risk of intraocular pressure increase (IOPi) among nondiabetic patients using bevacizumab, aflibercept, and ranibizumab. The results showed that compared with aflibercept, bevacizumab and ranibizumab users had a higher risk of IOPi.

Adherence to Oral Antidiabetic Drugs in Patients with Type 2 Diabetes: Systematic Review and Meta-Analysis

Summary: Poor adherence to oral antidiabetic drugs (OADs) in patients with type 2 diabetes (T2D) can lead to therapy failure and risk of complications. This study aimed to determine the adherence to OADs in patients with T2D and estimate the association between good adherence and glycemic control. The systematic review and meta-analysis included 156 studies and showed that the proportion of adherent patients was 54%. There was a significant association between good glycemic control and good adherence, indicating that improving adherence could reduce the risk of complications.

Circulating Levels of Hydrogen Sulfide (H2S) in Patients with Age-Related Diseases: A Systematic Review and Meta-Analysis

Summary: Hydrogen sulfide (H2S) is an endogenous gasotransmitter that plays a role in various biological effects. Imbalanced biosynthesis of H2S has been observed in animal models of age-related diseases, but human studies have inconsistent results. This systematic review and meta-analysis found that circulating levels of H2S were significantly reduced in patients with progressive, chronic, and degenerative diseases compared to healthy individuals. Stratified analysis also showed a significant reduction in circulating H2S levels in patients with vascular disease or kidney disease. These findings suggest that targeting the H2S system may have potential therapeutic benefits for slowing the progression of age-related diseases.

Adherence to riluzole therapy in patients with amyotrophic lateral sclerosis in three Italian regions-The CAESAR study
