Gender differences among patients with primary ankylosing spondylitis and spondylitis associated with psoriasis and inflammatory bowel disease in an iberoamerican spondyloarthritis cohort

The aim of the study was to compare clinical manifestations, disease activity, functional capacity, spinal mobility, and radiological findings between men and women from a multicenter, multiethnic Ibero-American cohort of patients with Spondyloarthritis (SpA). This observational cross-section study included 1264 consecutive SpA patients who fulfilled the modified New York criteria for ankylosing spondylitis (AS). Demographic, clinical, and radiologic data were evaluated. Categorical data were compared by chi(2) or Fisher's exact tests and continuous variables by ANOVA with post-hoc tests. Primary AS was diagnosed in 1072 patients, psoriatic spondylitis in 147, and spondylitis associated to inflammatory bowel disease (IBD) in 45 patients. Overall, male patients were significantly younger, had longer diagnostic delay, lower disease activity, worse spinal mobility, better quality of life, and more severe radiologic damage. Dactylitis and enthesitis, as well as swollen joint count, were significantly more common among women. In primary AS, there was a marked male predominance (76.2%). Among patients with psoriatic spondylitis, male predominance was lower (57.8%), but was also associated with worse spinal mobility and more severe radiologic damage. In the total population, male patients with primary AS referred higher permanent work disability (13.2% vs 6.9%; P<0.05), although no difference was observed in psoriatic or IBD spondylitis according to the gender. Among Ibero-American SpA patients, there are some differences in clinical and radiological manifestations, men showing more structural damage, whereas women more active disease. These data suggest that the phenotype of SpA differs between genders. This can influence the subsequent diagnostic approach and therapeutic decisions.

Health-related quality of life among spondyloarthritis and chronic low back pain patients: results from a nationwide population-based survey

Summary: The study found that both spondyloarthritis and chronic low back pain have similar impacts on health-related quality of life (HRQoL), including mental, physical, and social domains. Patients with spondyloarthritis and chronic low back pain reported significantly lower HRQoL compared to those without rheumatic diseases. Modifiable factors such as multimorbidity, active disease, obesity, and regular physical exercise were associated with HRQoL in these conditions.

Ultrasound findings and prognosis of shoulder pain: A role for Doppler signal?

Summary: This study aimed to identify ultrasound prognostic factors for shoulder pain. The findings showed that a positive Doppler signal on shoulder tendons and absent tendon heterogeneity were associated with improvement reported by the patients. Poorly defined and sparsely distributed calcifications were also indicators of a better disease course.

Symptomatic fractures in systemic sclerosis: A case-control study

Summary: The study identifies the determinants of health-related quality of life and global functioning and health in patients with axial SpA, peripheral SpA, and PsA. Disease activity and physical function were found to be major factors influencing HRQoL and GH across different types of SpA. Clinical characteristics and sociodemographic factors also play an important role, highlighting the importance of a holistic approach for individual patients.

Safety of intra-articular glucocorticoid injections state of the art

Summary: Intra-articular glucocorticoid injection (IAGCI) is commonly used for treating joint pain and inflammation, but detailed descriptions of its safety are lacking. This review summarizes the complications of IAGCI in the short-term, immediate and long-term, as well as their predictors. Most studies primarily report mild adverse events with a similar incidence to placebo. However, reported incidences vary significantly and are mostly based on retrospective data. Septic arthritis is the most concerning complication due to its high mortality rate. Other local complications include injection site pain, post-injection flare, skin hypopigmentation and atrophy, and tendon rupture. Systemic side effects are also common, including vasovagal reactions, flushing, increased appetite and mood changes, hyperglycemia in diabetic patients, and bleeding in high-risk patients. There is limited evidence on predictors of complications. Overall, more detailed knowledge of adverse events and complications is needed given the routine use of IAGCI.

Summary: Recent studies have shown that immunocompromised individuals may inadvertently contribute to the mutations of the virus that lead to new variants. This aspect should be carefully considered as immunocompromised individuals remain at risk of COVID-19 despite vaccination, experience more severe disease, and can be contagious for longer. The European Reference Network on Rare and Complex Connective Tissue and Musculoskeletal Diseases (ReCONNET) addressed this issue by forming a multidisciplinary Task Force to develop guidelines for treating immunocompromised patients with COVID-19. The final principles and points-to-consider were agreed upon by the Task Force.
