Summary: We report a case of a 14-year-old female who ingested an overdose of bupropion, resulting in seizures and cardiotoxicity. The patient responded well to supportive care and lidocaine drip, and the QT interval was eventually narrowed. This successful treatment of ventricular tachycardia and ventricular fibrillation with lidocaine bolus and drip, leading to shortening of the QT interval, has not been previously reported.

Summary: Drug-induced liver injury (DILI) is a major cause of acute liver failure (ALF). DDX3X plays a role in pro-survival stress granule (SG) assembly, but its role in DILI is unknown. This study revealed that DDX3X expression is decreased in DILI patients and DDX3X deficient mice. DDX3X deficiency aggravated liver injury in multiple DILI models by enhancing oxidative stress, hepatocyte death, and inflammatory responses.

Summary: This study aimed to predict outcomes of bupropion overdose using a decision tree approach. The results showed that multiple seizures, conduction disturbance, intentional exposure, and confusion were the most influential factors to predict the outcome of bupropion exposure. Coma and seizure, including single, multiple and status, were the most important factors to predict major outcomes.

Females are more likely to utilize mental health services and be on a provincial disability program. Age, income, mental health diagnoses, and prescribed opioid dosage were associated with persistent opioid use in both genders.

Summary: Cardiorenal syndrome (CRS) is a complex and heterogeneous spectrum of symptoms that has posed challenges for cardiologists for many years. The coexistence of heart failure (HF) and deteriorated renal function adds to these challenges and is a strong predictor of poor outcomes. However, with the development of new renal assessment methods, we now have the opportunity to treat CRS more effectively than ever before. This review focuses on explaining the pathophysiology, recent advances, and current practices of monitoring renal function in patients with acute CRS, aiming to improve patient care and guide treatment strategies.

Summary: Cardiorenal syndrome is a broad spectrum disorder affecting the heart and kidneys. India has a high burden of acute cardiorenal syndrome, with 46.1% of patients diagnosed in India. Acute cardiorenal syndrome involves sudden kidney deterioration in acute heart failure patients. The pathophysiology includes hyperactivation of the sympathetic nervous system and the renin-angiotensin-aldosterone system. Complications increase the risk of mortality, making it a global healthcare burden. Current biomarkers have limitations in early detection, highlighting the need for novel proteomic biomarkers to prevent progression.

Summary: Critically ill patients with sepsis or septic shock have a worse prognosis due to a higher mortality rate and longer hospital stay. The proper use of antibiotics is crucial as it can affect the pharmacokinetics and pharmacodynamics in these patients. About 50% of septic patients develop acute renal failure, requiring continuous renal replacement therapy, which greatly affects the elimination rate of antibiotics. Adjustments in antibiotic doses are necessary in patients with acute renal failure based on glomerular filtration rate, while dosages should be increased in those undergoing continuous renal replacement therapy.

The developed probe can accumulate in the kidneys and respond to specific indicators, enabling sensitive detection of renal failure at an early stage. Additionally, the probe can be excreted in urine, allowing for remote detection and prediction of renoprotective efficacy.
