Summary: In patients receiving chronic hemodialysis, infective endocarditis (IE) is a common and serious complication characterized by higher mortality and relapse rates compared to non-HD patients. Additionally, cardiac surgery is less frequently performed in HD patients.

Summary: This study aimed to assess periprocedural hemostasis in IE patients undergoing urgent SVR and evaluate the association between hemostatic disorders and early bleeding as well as thromboembolic events. The results showed that hemostatic capacity of IE patients was significantly impaired before and after SVR, and it was related to bleeding and blood product transfusion.

Summary: Infective endocarditis in heart transplant recipients presents with specific characteristics, with non-valvular involvement being common and valve involvement not having a predominant location. Three fifths of cases have a nosocomial or health care-related origin. The major pathogens include staphylococci, enterococci, and Aspergillus, with an in-hospital mortality rate of 17%.

Summary: This study describes the clinical features, diagnosis, and treatment outcome of patients with infective endocarditis in patients with a valve-tube ascending aortic graft. Male patients with staphylococcal infection were predominantly affected, and multimodality imaging played an important role in diagnosis and management. Conservative therapy may be a valid alternative in selected patients.

Summary: Infective endocarditis (IE) carries a high risk of vascular complications, which are correlated with increased mortality. The use of anticoagulation in IE patients remains controversial and challenging. An appropriate anticoagulation strategy is crucial for improving outcomes and requires a good understanding of the indication, timing, and regimen of anticoagulation in IE.

Summary: This article reviews the current literature on the use of oral transitional therapy for the treatment of bacteraemia and infective endocarditis, and compares it to the traditional IV-only approach. The study found that oral therapy is not inferior to IV therapy in terms of treatment outcomes, and it can reduce hospital stay and decrease the occurrence of adverse events.

Summary: This systematic review evaluates all available risk scores for infective endocarditis (IE) and finds that some scores perform well in the derivation cohorts but poorly in new cohorts. The development of current IE risk scores is limited by small sample size, retrospective data collection, short-term outcomes, and lack of external validation, highlighting the need for future population studies and large registries to address this clinical need.

Summary: By comparing the metabolic and immunologic similarities and differences between ICR mice and NOD mice, it was found that NOD mice have differences in circulating proinsulin levels, infiltration of cells into and near pancreatic islets, gene expression in islets, and protein abundance in islets compared to ICR mice. Therefore, ICR mice are an excellent genetically similar and MHC-matched control choice for NOD mice in studies exploring mechanisms relevant to autoimmune-mediated diabetes onset and novel therapeutic interventions.

Summary: Congenital heart disease (CHD) patients with infective endocarditis (IE) have better survival rates compared to non-CHD patients. CHD patients are younger and have fewer comorbidities. Positive blood cultures for Streptococcus viridans and a higher prevalence of dental procedures were observed in CHD patients.

Summary: This study assessed the distribution of amoxicillin in infected heart valves and the achievement of related pharmacokinetic/pharmacodynamic (PK/PD) targets in IE patients undergoing heart valve surgery. The results showed significant penetration of amoxicillin into the infectious site and supported the need for combination antibiotic therapy for enterococcal IE.

Summary: In patients with infective endocarditis, platelet reactivity is increased in those with Staphylococcus aureus (SA) infection. However, the use of aspirin can reduce this increased reactivity without increasing bleeding risk. More research is needed to confirm the potential benefits of aspirin in SA-positive infective endocarditis.

Summary: This study evaluated the prognosis of patients with infective endocarditis who did not undergo surgery but received long-term antibiotic treatment. The results suggest that a long-term antibiotic treatment strategy, including long-term oral suppressive antibiotic treatment, may be considered for these patients. During follow-up, some patients experienced infection relapse and mortality, but only a small number of deaths were related to infective endocarditis.

Summary: This study investigated the clinical characteristics and outcomes of patients with infective endocarditis (IE) after transcatheter aortic valve implantation (TAVI). The study found no significant difference in in-hospital mortality and 1-year all-cause mortality between patients treated with antibiotics alone and patients treated with cardiac surgery and antibiotics. The high mortality in patients with TAVI-IE was strongly associated with patients' characteristics, pathogens, and complications.

Summary: The current data does not support the routine practice of prehospital intubation, and the effect of prehospital intubation on mortality might depend on the experience of EMS clinicians. Therefore, when intubation is required, it is advisable to involve prehospital personnel who are proficient in prehospital intubation.

Summary: Chronic postsurgical pain affects 10% of patients after surgery. Transitional Pain Service (TPS), a multidisciplinary patient-tailored intervention, has been developed and implemented to improve perioperative pain management and prevent chronic postsurgical pain. A satisfaction survey was conducted among healthcare providers to analyze their satisfaction and learn from their experiences on TPS implementation. The survey showed that 81.0% of staff members strongly felt that patient care had improved with the introduction of TPS, and 86.8% wanted to continue using TPS in the future. Overall, healthcare provider satisfaction improved after the implementation of TPS in our hospital.

Summary: This study aimed to determine whether low-dose dobutamine stress echocardiography (DSE) during transcatheter edge-to-edge mitral valve repair (TMVR) can predict residual mitral regurgitation (MR) at discharge. The results showed that an increase of MR during DSE can predict the presence of >moderate MR at discharge.

Summary: This study investigated the accuracy of (H)EMS assessment of unstable pelvic ring injuries and the application rate of NIPBD. The results showed that the prehospital sensitivity of (H)EMS for assessing unstable pelvic ring injuries and applying NIPBD is low. Roughly half of all unstable pelvic ring injuries were not recognized by (H)EMS. Further research is recommended to aid the routine use of NIPBD.

Summary: The study assessed whether VOCs in exhaled breath after COMEX-30 treatment are early markers of hyperoxic stress and/or POT. Results showed that the identified VOCs are associated with inflammatory responses or pulmonary diseases and most subjects reported pulmonary symptoms reflecting early-stage POT. Therefore, these VOCs are likely markers of POT, not just indicators of hyperbaric hyperoxic exposure.

Summary: This study compared the effects of adductor canal block (ACB) and femoral nerve block (FNB) on readiness for discharge in patients undergoing outpatient ACL reconstruction. The results showed no difference in readiness for discharge between the two groups, but patients in the FNB group were more likely to experience motor blocks and (near) falls. However, the FNB group consumed less opioids in the post-anesthesia care unit compared to the ACB group.
