Transradial access in acute myocardial infarction complicated by cardiogenic shock: Stratified analysis by shock severity

Summary: This study demonstrates that transradial access is a safe and feasible option for patients with acute myocardial infarction complicated by cardiogenic shock, reducing coronary access-site bleeding. Additionally, ultrasound-guided transfemoral access can also reduce bleeding and vascular complications. Efforts should be made to incorporate vascular access protocols into existing algorithms in dedicated shock care centers.

Summary: The COVID-19 pandemic led to a decrease in hospitalizations for acute coronary syndromes and a resurgence of structural complications of myocardial infarctions. Delay in VSR repair is associated with declining mortality, but patients may develop cardiogenic shock while waiting for surgery. Limited options are available for patients with VSR complicated by right ventricular failure and cardiogenic shock, with a very low survival rate for those undergoing surgical or percutaneous VSR repair.

Summary: The study found significant variability in discharge medication patterns for atrial fibrillation patients undergoing transcatheter aortic valve replacement in the US, including significant underuse of oral anticoagulants (OACs). Among patients receiving OAC+APT, there was no lower risk of stroke or mortality compared to OAC alone or APT alone, but an increased risk of bleeding complications at 1 year compared to APT alone.
