The Effect of Direct Oral Anticoagulant Therapy (DOACs) on oral surgical procedures: a systematic review

Summary: This study aims to determine the risk of perioperative and postoperative bleeding during oral surgical procedures in patients on direct oral anticoagulants (DOACs). The results demonstrate that minor oral surgical procedures are safe for patients on DOAC therapy, but there is still controversy regarding the continuation or discontinuation of anticoagulant drugs during oral surgery.

Assessment of exposure to direct oral anticoagulants in elderly hospitalised patients

Summary: The study found that there is significant variability in drug exposure among elderly patients receiving direct oral anticoagulants (DOACs), with factors such as dosage, renal function, and gender contributing to a third of the variability in DOAC plasma concentrations. Further investigation is needed to determine whether the observed increase in DOAC exposure in older patients is the cause of their increased risk of bleeding, which could potentially be alleviated through dosing titration.

Most studies lack detailed information on the concepts and methods used to assess nonadherence.

Summary: This study found a significant temporal association between invasive dental procedures (IDPs), particularly extractions and oral-surgical procedures, and subsequent infective endocarditis (IE) in individuals at high-risk for IE. The use of antibiotic prophylaxis (AP) was associated with a significant reduction in IE incidence following these procedures.

Coagulation assays and direct oral anticoagulant levels among patients having an elective surgery or procedure

Summary: Coagulation testing is not a reliable method for predicting DOAC concentrations. There is a weak correlation between coagulation assay results and DOAC levels, except for a moderate correlation between thrombin time and dabigatran concentrations. Prothrombin time and activated partial thromboplastin time have modest sensitivity but poor specificity in predicting DOAC levels.

Summary: The study aimed to evaluate prescriber compliance to DOAC labels and the impact of dosing appropriateness on clinical outcomes. Results showed high compliance to approved indications but 1/3 of prescriptions did not adhere to approved dosing, and dosing deviation was associated with an increase in adverse clinical outcomes.

Summary: This study successfully developed a bleeding risk prediction model for AF patients on oral anticoagulants, with factors such as age over 75 years, history of cancer, prior major hemorrhage, and arterial hypertension included in the final prediction model. The model showed good calibration and discrimination with a Brier score of 0.23 and a c-statistic of 0.71 at 12 months follow-up.

Direct-acting oral anti-coagulants in dental practice: A Retrospective Observational Study (Part 1)

Summary: This retrospective observational study aimed to determine the incidence of direct-acting oral anti-coagulant (DOA) use in patients undergoing invasive dental procedures. The study found that despite the increase in DOA usage, there is no consistent approach to preoperative laboratory testing and DOA discontinuations. Most cases of excessive bleeding during procedures were locally controlled with hemostatic agents, with a low incidence of postoperative complications.

Summary: This study demonstrates that transradial access is a safe and feasible option for patients with acute myocardial infarction complicated by cardiogenic shock, reducing coronary access-site bleeding. Additionally, ultrasound-guided transfemoral access can also reduce bleeding and vascular complications. Efforts should be made to incorporate vascular access protocols into existing algorithms in dedicated shock care centers.

Summary: The COVID-19 pandemic led to a decrease in hospitalizations for acute coronary syndromes and a resurgence of structural complications of myocardial infarctions. Delay in VSR repair is associated with declining mortality, but patients may develop cardiogenic shock while waiting for surgery. Limited options are available for patients with VSR complicated by right ventricular failure and cardiogenic shock, with a very low survival rate for those undergoing surgical or percutaneous VSR repair.

Summary: The study found significant variability in discharge medication patterns for atrial fibrillation patients undergoing transcatheter aortic valve replacement in the US, including significant underuse of oral anticoagulants (OACs). Among patients receiving OAC+APT, there was no lower risk of stroke or mortality compared to OAC alone or APT alone, but an increased risk of bleeding complications at 1 year compared to APT alone.
