Diagnosis can be challenging, particularly in distinguishing between idiopathic pulmonary fibrosis, fibrotic hypersensitivity pneumonitis, and connective tissue disease-associated ILD. Antisynthetase syndrome (ASyS) can present a diagnostic challenge, especially when lung involvement is predominant or extrapulmonary manifestations are subclinical. Coordination between pulmonologists and rheumatologists is essential for accurate diagnosis and management.

Summary: This study aimed to characterize the lung microbiome in the bronchoalveolar lavage fluid (BALF) of patients with Antisynthetase Syndrome (ASSD) and evaluate its correlation with differential cell count and other bacterial genera in BALF. The results showed that patients with anti-Jo1 autoantibodies had a lower abundance of Veillonella genus, which was strongly correlated with levels of eosinophils, lymphocytes, and Prevotella genus, and negatively correlated with the percentage of macrophages.

Age and CT progression were associated with reduced survival, while the presence of ground-glass opacities and predominant lower lobe distribution were associated with increased survival.

Summary: MRI evaluation of patients with antisynthetase syndrome (ASS), dermatomyositis (DM), and immune-mediated necrotizing myopathy (IMNM) revealed frequent muscle and myofascial edema in ASS patients. Compared to IMNM, ASS and DM were associated with more adductor-muscle sparing and subcutaneous-tissue edema. While similar edema patterns were observed in ASS and DM, there were subtle differences between the two groups, including less symmetry and more myofascial edema of the tensor fasciae latae in the ASS group.

Summary: Dysphagia is a known symptom of dermatomyositis, but it can be challenging to diagnose and treat sudden-onset dysphagia without other symptoms. Two patients without severe muscle weakness experienced sudden inability to swallow solids and liquids. Muscle biopsy results confirmed the diagnosis of dermatomyositis, and videofluoroscopy revealed pharyngeal contraction and upper esophageal sphincter opening issues. Both patients showed low tongue pressures. Treatment with methylprednisolone and immunoglobulin led to improvement in swallowing function, with tongue pressure improvement preceding subjective and objective measurements of dysphagia.

Summary: Visual dysfunction is a significant nonmotor symptom of Parkinson's disease (PD), and visual hallucinations and visuospatial dysfunctions are common in PD patients. This study used electroencephalographic coherence analysis to evaluate neuronal synchronization in PD patients with and without visual hallucinations or visuospatial dysfunctions. The findings suggest that reduced coherence in the frontal regions of the brain may be involved in visuospatial dysfunctions in PD patients.
