Summary: Frailty is strongly associated with time in hospital and mortality among patients undergoing transcatheter aortic valve replacement. Higher frailty levels are linked to longer hospital stays and increased risk of death within one year. Strategies aimed at preventing hospitalization among frail patients could be beneficial in reducing the burden of care.

Summary: This study retrospectively evaluated imaging features of right heart failure on pre-TAVR CT for 505 patients and found that characteristics such as pericardial effusion size and main pulmonary artery size were associated with increased mortality within 1 year after the procedure. Multivariate logistic regression analysis showed that these features were independent predictors of death, even after adjusting for the Society of Thoracic Surgeons score.

Summary: Despite the rapid adoption of transcatheter aortic valve replacement (TAVR), there are limited data on aortic valve reintervention after initial TAVR. A retrospective study reviewed 1487 patients who underwent TAVR and found that 24 (1.6%) required aortic valve reintervention. Most patients were deemed unsuitable for repeat TAVR due to concurrent cardiac procedures, unfavorable anatomy, or endocarditis. Therefore, careful assessment of TAVR repeatability should be considered in the initial TAVR evaluation, especially for younger patients who may require aortic valve reintervention.

However, TAVR-related infective endocarditis (TAVR-IE) is expected to be more common in younger patients. Therefore, it is important for clinicians to be familiar with common presentations, major risk factors, diagnostic pitfalls, therapeutic approaches, and prevention of TAVR-IE.

Summary: The objective of this study was to explore the relationship between spectral computed tomography (CT) measured iodine concentration and strength of aortic valvular calcification (AVC) in patients with aortic valve stenosis (AVS). The study found that iodine concentration was independently associated with the reduction percentage of AVC. Additionally, when an optimal iodine concentration cutoff point was set at 4 mg/mL, it could accurately assess fragile AVC with a reduction percentage greater than 18.5%.

Summary: In a large series of ViV-TAVR procedures, the incidence of periprocedural PPI was relatively low and decreased with the use of new-generation THV systems. Older age, larger THV size, and previous right bundle branch block were associated with an increased risk of PPI. Although there were no significant differences in 30-day mortality between the PPI and no-PPI groups, PPI patients exhibited a trend toward higher mortality risk at follow-up.

Summary: The study compared outcomes of transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) in patients with rheumatic aortic stenosis (AS), as well as TAVR in nonrheumatic AS. The results showed similar mortality rates between TAVR and SAVR for rheumatic AS patients, suggesting that TAVR could be a viable alternative to traditional surgery in this patient population.
