Summary: This study evaluated the rate of non-sentinel lymph node (NSLN) positivity and factors influencing this in patients with a positive sentinel lymph node (SLN) following neoadjuvant chemotherapy (NAC). The results showed a high rate of nodal positivity on completion axillary lymph node dissection (cALND) in the setting of positive SLN after NAC, supporting the current standard of routine cALND. In patients with cN+ disease, NSLN positivity varied based on tumor biology, multicentricity/multifocality, number of positive SLNs, and SLN metastasis size.

Summary: This study investigated the sentinel lymph node and lymphatic drainage pattern of breast cancer using contrast-enhanced ultrasound. The most common lymphatic drainage pattern observed was one lymphatic channel originating from the upper quadrant of the areola and one sentinel lymph node. Contrast-enhanced ultrasound can identify the lymphatic drainage pattern before surgery, reducing the false negative rate of sentinel lymph node biopsy.

Summary: The purpose of this study was to determine the correlation between the biological subtype of breast cancer and the risk of metastasis to a sentinel lymph node. In the analyzed group of 1018 women with breast cancer, 57% were classified as Luminal A subtype. A positive sentinel lymph node was found in 26.5% of the women.

Summary: The SENOMIC trial found that omitting axillary lymph node dissection in patients with breast cancer and sentinel node micrometastases had excellent 3-year event-free survival rates. However, patients who underwent mastectomy had worse tumor characteristics and higher risk of recurrence, especially without adjuvant radiotherapy. Long-term follow-up and enrollment of mastectomy patients are crucial.

Summary: Sentinel lymph node biopsy (SLNB) appears to be a safe and effective technique for breast cancer during pregnancy, according to this systematic review. A total of 63 articles were included, with 47 strongly in favor of SLNB in pregnancy-related breast cancer (PABC), 4 partially in favor, 10 strongly against, and 2 partially against. Sub-categorization based on study type showed that the majority of studies in favor had higher levels of evidence. In total, 382 women with PABC underwent SLNB, and full data were reported for 237 cases. The overall live birth rate was 95.8%, while the overall neonatal complication rate was 3.4%. No cases of maternal side effects or anaphylactic reaction, maternal death, stillbirth, or neonatal death were reported (0%).

Summary: In recent years, there has been a trend towards less aggressive surgical management of axillary lymph nodes in breast cancer. This study evaluated the effectiveness of evaluating axillary sentinel lymph nodes and found that serial sectioning at 2-mm intervals reduced false negatives due to macrometastatic disease. Discordant cases were mostly false negatives detected on the first permanent section level due to sampling errors.

Summary: Studies have shown significant heterogeneity in lymphatic drainage patterns of different cutaneous melanomas, emphasizing the need for multiple imaging evaluations in critical skin areas. The use of fluorescence lymphography with indocyanine green has greatly improved visualization at a microscopic level. It is crucial to combine SPET-CT scans with lymphoscintigraphy preoperatively and perform additional evaluations with indocyanine green intraoperatively to accurately identify sentinel nodes and reduce false negative rates.

Summary: The local treatment of axilla in early breast cancer patients without clinically evident lymph node metastases is a subject of debate. Various prospective randomized surgical trials are currently being conducted to evaluate the safety of omitting sentinel lymph node biopsy in upfront breast-conserving surgery and neoadjuvant setting. These trials aim to determine whether it is possible to make therapeutic decisions without pathologic evaluation of nodal status. Additionally, ongoing trials are investigating the possibility of de-escalating surgery based on the response to neoadjuvant systemic therapy.

Summary: The added value of repeat sentinel lymph node biopsy (rSLNB) in clinically node-negative (cN0) patients with ipsilateral breast tumor recurrence (IBTR) who underwent optimal clinical staging with FDG-PET/CT was assessed. The study found that the incidence of a tumor-positive rSLNB in patients with a negative FDG-PET/CT is low and does not change survival. Therefore, rSLNB can be omitted in these cN0 patients.

Summary: This study aimed to evaluate the rate of positive non-sentinel lymph nodes in breast cancer patients after neoadjuvant systemic therapy following positive sentinel lymph node biopsy. Positive non-SLNs were found in 62.3% of cases and were significantly associated with older age, clinically positive lymph nodes, SLN extracapsular extension, and a higher ratio of positive SLNs/total SLNs. ECE and higher nodal ratio were independent predictors of III axillary level positivity.

Summary: This study aimed to describe the surgical technique and outcomes of single-incision laparoscopic clockwise continuous total abdominal colectomy with end ileostomy in patients with ulcerative colitis. The study included 52 patients, with a median operation time of 100 minutes and a postoperative complication rate of 25%. There were no intraoperative complications or conversions to open surgery. The results showed that single-incision laparoscopic surgery is a safe and effective treatment option.
