While alcohol abuse is harmful, moderate alcohol consumption, especially of wine, is associated with health benefits. This review evaluates the relationship between the type and dose of alcoholic drink and the risk of various diseases, and emphasizes the unique benefits of moderate wine consumption, particularly in a Mediterranean diet.
Although it is clearly established that the abuse of alcohol is seriously harmful to health, much epidemiological and clinical evidence seem to underline the protective role of moderate quantities of alcohol and in particular of wine on health. This narrative review aims to re-evaluate the relationship between the type and dose of alcoholic drink and reduced or increased risk of various diseases, in the light of the most current scientific evidence. In particular, in vitro studies on the modulation of biochemical pathways and gene expression of wine bioactive components were evaluated. Twenty-four studies were selected after PubMed, Scopus and Google Scholar searches for the evaluation of moderate alcohol/wine consumption and health effects: eight studies concerned cardiovascular diseases, three concerned type 2 diabetes, four concerned neurodegenerative diseases, five concerned cancer and four were related to longevity. A brief discussion on viticultural and enological practices potentially affecting the content of bioactive components in wine is included. The analysis clearly indicates that wine differs from other alcoholic beverages and its moderate consumption not only does not increase the risk of chronic degenerative diseases but is also associated with health benefits particularly when included in a Mediterranean diet model. Obviously, every effort must be made to promote behavioral education to prevent abuse, especially among young people.

Could ketogenic diet starve cancer? Emerging evidence

Luigi Barrea, Massimiliano Caprio, Dario Tuccinardi, Eleonora Moriconi, Laura Di Renzo, Giovanna Muscogiuri, Annamaria Colao, Silvia Savastano

Summary: Cancer cells predominantly utilize aerobic glycolysis for metabolism. This characteristic has led to the hypothesis that dietary manipulation, such as a ketogenic diet, can influence tumor treatment and prognosis. The effectiveness of the ketogenic diet in cancer management may be attributed to various mechanisms, beyond the originally proposed glucose/insulin signaling inhibition. A qualified nutritionist is essential in managing the potential complications of this dietary therapy.

Impact of COVID-19 on the Destiny of Bariatric Patients

Paola Gualtieri, Marco Marchetti, Laura Di Renzo, Gemma Lou De Santis, Roselisa Palma, Carmela Colica, Giulia Frank, Antonino De Lorenzo, Nicola Di Lorenzo

Summary: Obese patients with COVID-19 face worse outcomes due to prothrombotic and low-grade inflammation status. The postponement of bariatric surgery during the SARS-CoV-2 outbreak has caused delays in obesity treatment and severe consequences. This umbrella review highlights the need to prioritize bariatric surgery as obesity is a risk factor for severe COVID-19 and the economic costs of the pandemic are substantial.

Exploring the Sustainable Benefits of Adherence to the Mediterranean Diet during the COVID-19 Pandemic in Italy

Paola Gualtieri, Marco Marchetti, Giulia Frank, Rossella Cianci, Giulia Bigioni, Carmela Colica, Laura Soldati, Alessandra Moia, Antonino De Lorenzo, Laura Di Renzo

Summary: This study aimed to investigate the adherence to the Mediterranean diet and its impact on health, environmental sustainability, and socioeconomic factors during the COVID-19 pandemic in Italy. The study found that adherence to the Mediterranean diet affected carbon and water footprints as well as food expenditure. The results showed that individuals with lower and medium adherence to the Mediterranean diet had higher carbon and water footprints. There were also significant differences in food expenditure among different levels of adherence to the Mediterranean diet.

Summary: The Mediterranean Diet is associated with beneficial effects against chronic non-communicable diseases. A study on 24 subjects showed that 2-week supplementation with mixed apple and bergamot juice had a positive impact on body composition, biochemical profile, and oxidative and inflammatory gene expression. The supplementation resulted in increased lean mass, reduced total cholesterol/HDL index, and improved gene expression related to CNCDs.

Summary: Supplementation has a positive impact on vaccination response, enhancing immune defenses and reducing side effects. Our study analyzed the relationship between vaccine side effects and supplement intake among the Italian population. The results showed a significant correlation between supplement consumption and side effects at the end of the vaccination cycle, as well as associations with specific side effects such as diarrhea and nausea.

The Pharmaceutical Formulation Plays a Pivotal Role in Hydroxytyrosol Pharmacokinetics

Laura Di Renzo, Antonella Smeriglio, Mariarosaria Ingegneri, Paola Gualtieri, Domenico Trombetta

Summary: Current evidence supports the use of EVOO and its minor components to improve cardiovascular and metabolic health. This study investigated the pharmacokinetics of DOPET, a bioactive compound in EVOO, on 20 healthy volunteers. Results showed higher bioavailability of DOPET compared to literature, suggesting the role of pharmaceutical formulation in its bioavailability and pharmacokinetics.

Pancreatic Ductal Adenocarcinoma and Nutrition: Exploring the Role of Diet and Gut Health

Paola Gualtieri, Rossella Cianci, Giulia Frank, Erica Pizzocaro, Gemma Lou De Santis, Silvia Giannattasio, Giuseppe Merra, Giovanni Butturini, Antonino De Lorenzo, Laura Di Renzo

Summary: The incidence of pancreatic cancer, particularly pancreatic ductal adenocarcinoma (PDAC), is increasing worldwide and is associated with chronic inflammation. Imbalance in the gut microbiota composition, known as dysbiosis, can lead to chronic inflammation and potentially promote PDAC development. Nutrition plays a modulatory role in the immunological processes of PDAC, influencing the gut microbiota composition and inflammation levels, thereby affecting the pathogenesis and progression of PDAC.

Modified Mediterranean-Ketogenic Diet and Carboxytherapy as Personalized Therapeutic Strategies in Lipedema: A Pilot Study

Laura Di Renzo, Paola Gualtieri, Samanta Zomparelli, Gemma Lou De Santis, Silvia Seraceno, Claudia Zuena, Giulia Frank, Rossella Cianci, Domenico Centofanti, Antonino De Lorenzo, Rita Businaro

Summary: This study evaluated the use of a modified Mediterranean ketogenic diet (MMKD) combined with carboxytherapy as a treatment for lipedema patients. The results showed that the ketogenic diet effectively induced weight and fat loss, including in the limbs. However, the best results were obtained from the combination of the ketogenic diet and carboxytherapy, which showed improvements in body composition, skin texture, pain reduction, and sleep quality.

Valentina Tassinari, Antonella Smeriglio, Virgilio Stillittano, Domenico Trombetta, Romano Zilli, Roberta Tassinari, Francesca Maranghi, Giulia Frank, Daniele Marcoccia, Laura Di Renzo

Summary: Endometriosis is a chronic inflammatory disease that is estrogen-dependent and characterized by the presence of extrauterine endometrial tissue, causing pelvic pain and fertility impairment. The exact cause of endometriosis is not fully understood, and conventional therapies have shown effectiveness but with considerable side effects. There is a growing interest in the use of herbal medicine for endometriosis treatment, particularly polyphenols that have anti-endometriotic and anti-inflammatory properties. This review aims to provide an overview of the anti-inflammatory activity of polyphenols in the treatment of endometriosis.
