Summary: Neurovascular compression causing optic neuropathy is rare but potentially treatable. This report discusses a case of optic nerve compression within the orbital apex due to an ectatic internal carotid artery in a patient with congenital absence of the contralateral internal carotid artery, highlighting the complementary roles of advanced neuroimaging and neuro-ophthalmologic examination in diagnosis.

Summary: Lower cranial nerve palsies are rare and often caused by tumors. A 49-year-old woman presented with progressive right-sided atrophy of the tongue, sternocleidomastoid and trapezius, dysarthria, and dysphagia for 3 years. Imaging revealed a circular lesion adjacent to the lower cranial nerves, which was confirmed to be an unruptured aneurysm in the C1 segment of the right internal carotid artery. Partial symptom improvement was achieved after endovascular treatment.

Summary: This review discusses the medical and surgical management approaches for patients with acute, subacute, and chronic internal carotid artery occlusion (ICAO). Revascularization through surgery, endovascular procedures, or a combination of both has shown to improve functional outcomes in patients with ICAO compared to systemic thrombolysis or medical therapy alone. Further research is needed to determine the superiority of revascularization methods for patients with ICAO.

Summary: Cervical artery dissection, a rare but important diagnosis in young stroke patients, showed an extremely rare association with thyrotoxicosis in a previously healthy 43-year-old female. By treating the underlying hyperthyroidism and managing the bilateral internal carotid artery dissections conservatively, the patient achieved a complete recovery with resolution of neurological symptoms.

Summary: In this study, we investigated the clinical significances of the relationships among sphenoid sinus aeration, intersphenoid sinus septum (ISS), and internal carotid artery (ICA). The results showed that patients with presellar pneumatization exhibited less prominent ICAs and more ISS attached to or near the paraclival ICA. Therefore, caution should be taken when using the endoscopic endonasal transsphenoidal approach to treat patients with poor sphenoid sinus aeration.
