Mandibular Class II Furcation Defect Treatment: Effects of the Addition of Platelet Concentrates to Open Flap: A Systematic Review and Meta-Analysis of Randomized Clinical Trials

Background: To improve the efficacy and outcomes of regenerative therapy for furcation defects, the use of platelet concentrates (PCs) in addition to open flap debridement (OFD) has been investigated. The aim of this systematic review is to evaluate whether mandibular Class II furcation defects treated with the addition of PC to OFD heal with a significant improvement in the following: 1) horizontal clinical attachment level (HCAL); 2) vertical clinical attachment level (VCAL); 3) probing depth (PD); and 4) level of gingival margin (LGM). Methods: Electronic databases (MEDLINE, EBSCO Library, Cochrane Database of Systematic Reviews, and SCOPUS) were searched for randomized clinical trials to address the use of PCs in combination with OFD compared with a control group without PCs for the treatment of mandibular Class II furcation defects in humans and to provide data on the above reported outcome measures. The results of selected studies were converted to mean difference and standard error and interpolated using the inverse of variance test. Heterogeneity was investigated using both the Higgins index and the Q test. Results: Of 254 articles screened, only 11 were read in full text, and three of these were included in the meta-analysis. The addition of PCs to OFD revealed a small improvement in the following: 1) HCAL (mean difference, 1.36 mm; 95% confidence interval [CI] = 1.07 to 1.65); 2) VCAL (mean difference, 1.54 mm; 95% CI = 1.23 to 1.85); and 3) PD (mean difference, 1.83 mm; 95% CI = 1.36 to 2.29). No differences were found for the LGM. Heterogeneity across the studies was high, and all the three included studies were performed in one country. Conclusions: Adding PCs to OFD for the treatment of mandibular Class II furcation defects may lead to slight improvements in clinical parameters. Nonetheless, because of the high heterogeneity of the very small number of reported studies and the small effect size, no definitive conclusion can be achieved about the clinical application of such a treatment option.

Role Played by Paraoxonase-2 Enzyme in Cell Viability, Proliferation and Sensitivity to Chemotherapy of Oral Squamous Cell Carcinoma Cell Lines

Summary: Oral squamous cell carcinoma is the most aggressive and frequent form of head and neck cancer. Paraoxonase-2 enzyme has been identified as a potential molecular target for effective oral cancer treatment. Silencing paraoxonase-2 gene led to a reduction in cell proliferation and viability, enhanced sensitivity to cisplatin, and activation of the apoptosis pathway. These findings suggest that paraoxonase-2 could be a promising target for molecular treatment of oral cancer.

Cost/effectiveness analysis of treatment options for the rehabilitation of the total edentulous mandible

Summary: This study conducted a cost-effectiveness analysis of different rehabilitative solutions for edentulous mandibles. The decision tree models showed that fixed dentures supported by 2 implants were the most cost-effective option, followed by overdentures retained by 2 implants. Probabilistic sensitivity analysis confirmed the dominance of these two strategies.

CAD/CAM Abutments in the Esthetic Zone: A Systematic Review and Meta-Analysis of Soft Tissue Stability

Summary: Computer-aided design and computer-aided manufacturing customized abutments are commonly used in clinical practice, but there is a lack of scientific evidence on their advantages in terms of soft tissue stability. This systematic review compared prefabricated and customized (CAD/CAM) abutments in terms of soft tissue outcomes. The analysis of six clinical trials showed no significant differences in midfacial mucosal recession, interproximal papillae, and pink aesthetic score after 12 months. The potential benefits of CAD/CAM abutments on soft tissues should be further investigated.

Location and Gender Differences in Osteonecrosis of the Jaws in Patients Treated with Antiresorptive and Antineoplastic Drugs Undergoing Dentoalveolar Surgical, Systematic Review with Meta-Analysis and Trial Sequential Analysis

Summary: Antiresorption drugs like bisphosphonates and monoclonal antibodies are used in the treatment of osteoporosis and other bone-related diseases. These drugs have been associated with cases of osteonecrosis of the jaws (MRONJ). Risk factors include extraction procedures in oral surgery, particularly in the mandible and maxilla. A systematic review and meta-analysis was conducted to determine if there is a greater risk of MRONJ in male or female subjects and to assess the involvement of osteonecrosis in different extraction sites. After analyzing 24 studies, the results showed a trend of higher risk in males and in the posterior mandibular sector, but further studies are needed to confirm these findings.

Mandibular Reconstruction after Resection of Ameloblastoma by Custom-Made CAD/CAM Mandibular Titanium Prosthesis: Two Case Reports, Finite Element Analysis and Discussion of the Technique

Summary: Virtual surgical planning for CAD/CAM mandibular reconstruction using titanium prosthesis has been recently reported. However, there may be difficulties in TMJ function due to prosthesis contamination from oral mucosa dehiscence. Two cases of mandibular reconstruction after ameloblastoma resection using custom-made CAD/CAM titanium prosthesis were presented, aiming to preserve TMJ function and avoid intraoral incisions. Finite elements analysis was performed to ensure the efficacy of the prosthetic device. Although successful, there are still drawbacks and risks, and limitations and side effects must be considered when using this technique.

Summary: Circulating cell-free DNA (cfDNA) plays an important role in cell apoptosis or necrosis, including in the development and evolution of tumors and inflammatory diseases. Periodontitis, a chronic inflammatory disease, may be linked to various systemic inflammatory diseases. A possible correlation between periodontal disease and cfDNA has been discovered, offering new perspectives for diagnosis and therapy.

Summary: The present study suggests that LGALS3BP is highly expressed in oral squamous cell carcinoma (OSCC) and may serve as a potential diagnostic biomarker and therapeutic target for ADC-based therapy. The study also found a correlation between high LGALS3BP expression levels and aggressive histological features of OSCC. Additionally, OSCC cell lines with high LGALS3BP expression showed high sensitivity to the ADC-payload DM4.

High-risk TP53 mutations predict poor primary treatment response of patients with head and neck squamous cell carcinoma

Summary: This study explores the predictive value of TP53 mutations in treatment outcomes for HNSCC patients. It demonstrates that high-risk-of-death mutations are associated with a higher risk of stable/progressive disease, particularly in older patients with a history of alcohol consumption. Additionally, it suggests the potential usefulness of two classification systems for TP53 mutational landscapes.

Effect of Fabrication Technology on the Accuracy of Surgical Guides for Dental-Implant Surgery

Summary: The study investigated the impact of milling and 3D printing technologies on the accuracy of surgical guides. The results showed that the milled guides had smaller deviations in the contact surface with the mucosa compared to the 3D-printed guides. Additionally, the milling technology performed better in terms of axial and linear deviations of the sleeves' housings. Therefore, milling technology can provide more accurate surgical guides.
