Summary: This study describes the program structure, educational benefits, graduate specialty choices, and career impacts of the Pathology Medical Student Fellowship (PSF) at the research institution. The results indicate that many PSF graduates subsequently enter pathology careers, regardless of their initial uncertainty in specialty choice, and have a high rate of pursuing academic medical careers.

Summary: This study used a modified Delphi approach to develop a framework of 47 competencies for global neurology training. These competencies cover eight domains, including cultural context, medical knowledge, team-based practice, and healthcare systems. Summary: This study aims to provide data-driven benchmarks for defining accreditation criteria for foregut fellowships. Analysis of case log data of fellows trained from 2009 to 2019 shows a significant increase in the number of foregut cases and endoscopies over the past decade, with hiatal/paraoesophageal hernia repair having the most notable growth. Additionally, the proportion of robotic cases has been increasing year by year, while open cases and laparoscopic/thoracoscopic cases have been decreasing annually.

Summary: This study examines the reasons for pursuing endocrinology as a subspecialty among current fellows/trainees in the United States. The majority of respondents decided to pursue endocrinology during residency, and their endocrinology rotation and experiences with clinical mentors were identified as the most influential factors. Limited exposure to endocrinology during medical school highlights the need for improved integration of endocrinology experiences between medical school and residency.

Summary: This study revealed the importance of training future pediatric tertiary care specialists to care for children with medical complexity, as well as the need for improvement in certain high importance knowledge themes. The results from qualitative interviews and quantitative survey showed a high correlation, highlighting the importance of further training and development to improve patient safety, especially for vulnerable patient groups like CMC in tertiary care pediatrics.

Lack of funding and competing clinical responsibilities are major obstacles to completing the scholarly activity. Optimizing the alignment between fellow scholarly activity and their postgraduate positions can better support career development.

Summary: Patients with membranous nephropathy associated with malignancy were found to have unique NELL1 immune complexes in kidney biopsy tissue, with characteristic histopathology of incomplete capillary loop staining and IgG1 predominance. These NELL1-positive cases also showed a higher association with concurrent malignancy compared to other known types of membranous nephropathy.

Summary: Breast cancer remains one of the leading causes of mortality in women worldwide, and there is a need for better predictive and therapeutic targets. This study identifies ABI1 as a prognostic biomarker for breast cancer metastasis and demonstrates that genetic disruption of the ABI1 gene reduces lung metastases in mice. The study also shows that aberrations in ABI1 gene expression are associated with lung metastasis in primary breast cancer tumors. Targeting ABI1 may provide a therapeutic advantage for breast cancer patients.

Summary: Pro-inflammatory immune system development, metabolomic defects, and deregulation of autophagy are interconnected in driving the pathogenesis of systemic lupus erythematosus (SLE), specifically lupus nephritis (LN). Defects in autophagy and mitophagy lead to the accumulation of oxidative stress-generating mitochondria and necrotic cell death. The activation of mTOR and aberrant autophagy promote the expansion of pro-inflammatory cells and compromise end-organ resistance in LN. mTOR activation has been identified as a biomarker for disease activation and prognosis in SLE patients.

Summary: Studies have shown that correcting metabolic acidosis in chronic kidney disease with sodium bicarbonate can delay disease progression and improve survival rates. However, there may be challenges in correcting metabolic acidosis in chronic liver disease due to concerns about increased sodium and fluid load. This study aims to explore the association between bicarbonate therapy and the need for paracentesis in individuals with CKD-related metabolic acidosis and liver cirrhosis.

Summary: Obesity is associated with renal cell carcinoma and increases the complexity of surgical outcomes. This study compared the effectiveness of minimally invasive partial nephrectomy (MIPN) and open partial nephrectomy (OPN) in preserving renal function in obese patients. The results showed no statistically significant difference in renal function preservation between MIPN and OPN at 3-6 months post-surgery in obese patients, but MIPN had long-term benefits compared to OPN. In non-obese patients, MIPN was favored over OPN.
