Kidney Pathology Education for Nephrology Fellows: Past, Present, and Future

Kidney pathology education is crucial for nephrology fellows and practicing nephrologists. The use of digital learning, especially during the pandemic, has made nephrology a leader in online medical education. Resources such as medical blogs, journal clubs, interactive quizzes and games, online conferences, podcasts, and mentorship opportunities can supplement nephropathology training.
Kidney pathology education is a critical component in training of nephrology fellows, as well as for continuing medical ed-ucation for practicing nephrologists. Kidney pathology images are included on nephrology fellow board exams, and clinico-pathologic correlation of kidney biopsy findings is critical in everyday clinical practice. Nephropathology training is a requirement by the American College of Graduate Medical Education within nephrology fellowship curricula. However, greater than one-third of fellowship program directors believe that nephropathology training for their fellows is not suffi-cient. During the Coronavirus Disease-19 pandemic, the use of digital learning has become commonplace with virtual confer-ences (local, national, and international) and online meetings becoming the norm for education. Nephrology has become a leader in free open-access online medical education, both prior to and, to even a greater extent, during the pandemic. Here, we review available resources to nephrology fellows and other learners to supplement nephropathology training, which in-cludes medical blogs, journal clubs, interactive quizzes and games, online conferences, podcasts, and mentorship opportu-nities. These resources are archived and provide durable content to learners of all stages of training, even beyond the pandemic.(c) 2022 by the National Kidney Foundation, Inc. All rights reserved.

A case of immune complex mediated tubulointerstitial disease and nephrotic syndrome: anti LRP-2 Nephropathy with diffuse podocyte effacement

Summary: This case presents a rare form of kidney disease known as Anti-LDL Receptor-Related Protein 2 (Anti-LRP2) nephropathy, characterized by acute kidney injury and immune complex deposits in the proximal tubules. The patient exhibited features of acute kidney injury and nephrotic syndrome, with significant overlap in morphology with minimal change disease and anti-LRP2 nephropathy, highlighting the complexity of diagnosing and treating such conditions.

NELL1 is a target antigen in malignancy-associated membranous nephropathy

Summary: Patients with membranous nephropathy associated with malignancy were found to have unique NELL1 immune complexes in kidney biopsy tissue, with characteristic histopathology of incomplete capillary loop staining and IgG1 predominance. These NELL1-positive cases also showed a higher association with concurrent malignancy compared to other known types of membranous nephropathy.

ABI1-based expression signature predicts breast cancer metastasis and survival

Summary: Breast cancer remains one of the leading causes of mortality in women worldwide, and there is a need for better predictive and therapeutic targets. This study identifies ABI1 as a prognostic biomarker for breast cancer metastasis and demonstrates that genetic disruption of the ABI1 gene reduces lung metastases in mice. The study also shows that aberrations in ABI1 gene expression are associated with lung metastasis in primary breast cancer tumors. Targeting ABI1 may provide a therapeutic advantage for breast cancer patients.

Summary: Pro-inflammatory immune system development, metabolomic defects, and deregulation of autophagy are interconnected in driving the pathogenesis of systemic lupus erythematosus (SLE), specifically lupus nephritis (LN). Defects in autophagy and mitophagy lead to the accumulation of oxidative stress-generating mitochondria and necrotic cell death. The activation of mTOR and aberrant autophagy promote the expansion of pro-inflammatory cells and compromise end-organ resistance in LN. mTOR activation has been identified as a biomarker for disease activation and prognosis in SLE patients.

Summary: This study analyzed the serum of patients with THSD7A-associated membranous nephropathy and found that predominantly IgG4 anti-THSD7A antibodies can activate the complement system, but at high IgG4 density and through the alternative pathway.

Summary: Studies have shown that correcting metabolic acidosis in chronic kidney disease with sodium bicarbonate can delay disease progression and improve survival rates. However, there may be challenges in correcting metabolic acidosis in chronic liver disease due to concerns about increased sodium and fluid load. This study aims to explore the association between bicarbonate therapy and the need for paracentesis in individuals with CKD-related metabolic acidosis and liver cirrhosis.

Short-Term and Long-Term Renal Outcomes in Patients With Obesity After Minimally Invasive Versus Open Partial Nephrectomy for the Treatment of Renal Cancer: Retrospective Study

Summary: Obesity is associated with renal cell carcinoma and increases the complexity of surgical outcomes. This study compared the effectiveness of minimally invasive partial nephrectomy (MIPN) and open partial nephrectomy (OPN) in preserving renal function in obese patients. The results showed no statistically significant difference in renal function preservation between MIPN and OPN at 3-6 months post-surgery in obese patients, but MIPN had long-term benefits compared to OPN. In non-obese patients, MIPN was favored over OPN.
