A new protocol for Enterobacteriaceae identification and detection of carbapenemase-producing isolates from blood cultures by combining MALDI-TOF MS and the Carba NP test has been evaluated. Bacterial identification was correct in 129 of the 130 isolates tested while the Carba NP detected 28 out of the 29 carbapenemase producers. (C) 2016 Elsevier B.V. All rights reserved.

The findings show an emerging trend of RMTs among clinical CPE isolates, posing a threat to the clinical utility of aminoglycosides and plazomicin.

Summary: This study aimed to describe the characteristics of patients with community-onset bloodstream infections (CO-BSI) caused by Pseudomonas aeruginosa, identify predictors, and develop a predictive score. The study found that male gender, hematological malignancy, obstructive uropathy, source of infection other than urinary tract, biliary tract or intra-abdominal, and healthcare-associated BSI are independent predictors of Pseudomonas aeruginosa CO-BSI. The developed predictive score showed a negative predictive value of 89%.

Clostridioides difficile recovered in pleural fluid: Contamination or infection? A case report of a proven empyema and a literature review

Transcriptomic clustering of critically ill COVID-19 patients

Summary: In this study, researchers identified two transcriptomic clusters in critically ill COVID-19 patients based on peripheral blood transcriptomes. One cluster was characterized by interferon-related expression and the other promoted B-cell activation. These clusters had different ICU outcomes, suggesting different pathogenetic mechanisms. The findings demonstrate the potential of transcriptomics in identifying patient endotypes for personalized therapies in severe COVID-19.

Summary: The rise of AMR bacteria, especially VRE Enterococcus faecium, is a major health concern. Phage therapy has emerged as a novel alternative for AMR infections, but it requires a large collection of phages. This study explored human feces as a source of new E. faecium-infecting phages and isolated and characterized vB_EfaH_163 phage. It has a dsDNA genome, infects a wide range of E. faecium and E. faecalis strains, including some linezolid-resistant ones, and can control the growth of VRE isolates in culture and in an animal model, demonstrating its potential therapeutic application.

Evaluation of a Shotgun Metagenomics Approach for Detection of ESBL- and/or Carbapenemase-Producing Enterobacterales in Culture Negative Patients Recovered from Acute Leukemia

Summary: Patients diagnosed with acute leukemia often have a weakened immune system, making infections a major concern. This study used shotgun metagenomics to screen the gut of recovered leukemia patients for ESBL- and/or carbapenemase-producing Enterobacterales. The results showed no presence of these bacteria, indicating successful decolonization and positive implications for future clinical management. Further studies are needed to validate the utility of metagenomics in routine screening of multidrug-resistant bacteria in onco-hematological patients.

Clostridium paraputrificum bacteremia in a 64-year-old woman with colon carcinoma

Summary: We report a case of bacteremia caused by Clostridium paraputrificum in a 64-year-old woman with colon carcinoma and gastrointestinal disease. The isolate was susceptible to metronidazole and vancomycin, but resistant to benzylpenicillin, meropenem, and clindamycin based on the new EUCAST 2022 clinical breakpoints. Therefore, treatment with metronidazole should be considered in all patients with Clostridium bacteremia until antibiotic susceptibility is determined to minimize the risk of treatment failure.

Clinical Utility of the FilmArray® Blood Culture Identification (BCID) Panel for the Diagnosis of Neonatal Sepsis

Summary: This study aimed to evaluate the clinical utility of the FilmArray((R)) blood culture identification (BCID) panel in diagnosing neonatal sepsis. The results of the FilmArray((R)) BCID panel were compared with blood culture results in neonates with suspicion of early-onset and late-onset sepsis over a two-year period. The FilmArray((R)) BCID panel showed high specificity and negative predictive values, making it useful for decision-making regarding empirical antimicrobial treatment in neonates with negative blood cultures.

Enterococcal Phages: Food and Health Applications

Summary: Enterococcus is a diverse genus of Gram-positive bacteria found in various environments, including the human gut and fermented foods. This microbial genus has both beneficial effects and safety concerns. While it plays an important role in the production of fermented foods and can serve as probiotics, it is also responsible for the accumulation of toxic compounds and has emerged as important hospital-acquired pathogens due to antimicrobial resistance. Bacteriophages have re-emerged as a promising tool for controlling enterococcal infections, especially those involving antimicrobial-resistant strains.
