Artificial Intelligence (AI) systems provide valuable support in decision-making, especially in the medical field. The interaction between humans and AI, known as human-AI collaboration, can achieve better outcomes by integrating the opinions of both parties.
Artificial Intelligence (Al) systems are precious support for decision-making, with many applications also in the medical domain. The interaction between mos and Al enjoys a renewed interest following the increased possibilities of deep learning devices. However, we still have limited evidence-based knowledge of the context, design, and psychological mechanisms that craft an optimal human-Al collaboration. In this multicentric study, 21 endoscopists reviewed 504 videos of lesions prospectively acquired from real colonoscopies. They were asked to provide an optical diagnosis with and without the assistance of an Al support system. Endoscopists were influenced by Al (or = 3.05), but not erratically: they followed the Al advice more when it was correct (or = 3.48) than incorrect (or = 1.85). Endoscopists achieved this outcome through a weighted integration of their and the Al opinions, considering the case-by-case estimations of the two reliabilities. This Bayesian-like rational behavior allowed the human-Al hybrid team to outperform both agents taken alone. We discuss the features of the human-Al interaction that determined this favorable outcome.

Anastomotic Leaks following Esophagectomy for Esophageal and Gastroesophageal Junction Cancer: The Key Is the Multidisciplinary Management

Raquel Ortigao, Brigitte Pereira, Rui Silva, Pedro Pimentel-Nunes, Pedro Bastos, Joaquim Abreu de Sousa, Filomena Faria, Mario Dinis-Ribeiro, Diogo Libanio

Summary: This study identified older age and nodal disease as independent risk factors for anastomotic leakage after esophagectomy. Given the high mortality rates and in-hospital mortality rates in patients with EAL, endoscopic therapy may be an effective treatment approach.

Endoscopic Resection of Gastrointestinal Neuroendocrine Tumors: Long-Term Outcomes and Comparison of Endoscopic Techniques

Pedro Pimentel-Nunes, Raquel Ortigao, Luis Pedro Afonso, Rui Pedro Bastos, Diogo Libanio, Mario Dinis-Ribeiro

Summary: This study analyzed the short and long-term outcomes after endoscopic resection (ER) of gastric, duodenum, and rectal gastrointestinal neuroendocrine tumors (GI-NETs). The results showed that ER is a safe and highly effective treatment, especially for tumors smaller than 12 mm. Endoscopic mucosal resection with a cap (EMRc) was associated with a higher complication rate and should be avoided. Standard EMR (sEMR) was a simple and safe technique that is the best therapeutic option for most luminal GI-NETs.

Summary: The I-CARE study aims to evaluate the benefit-risk ratio of current therapies in IBD patients. It collected data from 10,206 patients and observed the treatment risks of anti-tumor necrosis factor and other biologic therapies, particularly the risks of cancer/lymphoma and serious infections. The results showed that different treatment regimens had varying effects on patients, and further research is needed to assess specific risks.

Summary: This study evaluated the effectiveness of biologic agents in Crohn's disease complicated with internal fistulizing disease. It found that biologic therapy is beneficial for approximately three-quarters of patients, achieving a fistula closure rate of 24%, but around one-third of patients still require surgery. Patient- and lesion-related factors can identify patients who will obtain more benefit from these drugs.

An enriched mixture model for functional clustering

Tommaso Rigon

Summary: This paper introduces a novel enriched Dirichlet mixture model for clustering functional data, incorporating functional constraints and bounding the model complexity. The prior process is characterized through a urn scheme, enhancing the interpretability of clustering. Variational Bayes approximation is employed for tractable posterior inference, overcoming computational bottlenecks.

A generalized Bayes framework for probabilistic clustering

Tommaso Rigon, Amy H. Herring, David B. Dunson

Summary: In this article, a generalized Bayes framework is proposed to bridge loss-based clustering methods and model-based clustering methods. The proposed method allows for quantification of uncertainty in clustering and provides a way to characterize uncertainty in clustering using Gibbs posteriors. Efficient deterministic algorithms and sampling algorithms are developed based on Bregman divergence and pairwise similarities. Furthermore, existing clustering algorithms can be interpreted as generalized Bayes estimators in this framework, allowing for uncertainty quantification.

Summary: Serrated polyposis syndrome (SPS) is a common polyposis syndrome that increases the risk of colorectal cancer (CRC). A germline frameshift variant in the POLD1 gene was identified in an SPS patient with CRC. The variant also appeared in three other SPS-affected family members. Organoid models were generated to study the variant's impact, but no significant differences were observed compared to the control group. Further research is needed to understand the functional consequences of POLD1 alterations. This study provides the first organoid model for germline POLD1 variants, establishing a foundation for studying SPS, CRC, and other malignancies.

Gastric Cancer: A Practical Review on Management of Individuals with Hereditary or Familial Risk for Gastric Cancer

Marisa Linhares, Claudia Marques Pinto, Diogo Libanio, Manuel R. Teixeira, Mario Dinis-Ribeiro, Catarina Brandao

Summary: Gastric adenocarcinoma is a common and deadly cancer worldwide. Screening the general population is recommended in eastern countries, but may not be cost-effective in low to intermediate-risk countries. Therefore, it is important to identify high-risk populations for adequate screening and surveillance. This review summarizes the data required to identify individuals at risk and highlights the importance of hereditary syndromes and screening protocols.
