Summary: Menopausal hormone therapy (HT) aims to improve quality of life by treating menopausal symptoms, but it also comes with adverse effects on breast tissue, including increased breast density and breast cancer risk. Healthcare professionals need to be aware of HT's impact on breast tissue to provide appropriate counseling for informed decision-making.

Summary: The study found specific methylation differences in response to hormone therapy among women who were 10 or more years postmenopause. Further research is needed to determine the functional consequences of these changes on atherosclerosis progression and the protective effects of hormone therapy.

Summary: Postmenopausal symptoms are systemic symptoms associated with estrogen deficiency after menopause. Hormonal therapy (HT) and non-HT are currently used to treat these symptoms, but the best regimen for balancing the benefits and risks is still unclear. This article reviews the characteristics, regimens, and side effects of drugs used in HT and non-HT, providing valuable information for clinical drug selection.

Summary: This retrospective cohort study in Sweden investigated the use of menopausal hormone therapy (MHT) in premenopausal women after bilateral oophorectomy. The study found that only 69% of women without malignancy underwent MHT within 1 year after surgery, and the duration of treatment was limited. The authors suggest further research is needed to understand the reasons behind the low dispensation rate in this group and improve adherence to treatment guidelines.

Summary: Hormone therapy is an effective treatment for menopausal symptoms, especially for younger women close to menopause. The benefits and risks of hormone therapy differ based on age and clinical characteristics, highlighting the importance of individualized treatment approaches. It is crucial to consider factors such as timing of initiation, type and route of administration, and patient-specific considerations when prescribing hormone therapy.

Summary: This study examines the association between menopausal hormone therapy (MHT) and cause-specific mortality. The results suggest that self-reported MHT use after early natural menopause, surgical menopause, or premenopausal hysterectomy is associated with a lower risk of breast cancer mortality but not consistently associated with mortality from cardiovascular disease or other causes.

Summary: The link between survival and reproductive function is influenced by both long-term evolutionary pressures and short-term environmental pressures. Loss of reproductive function in mammals is associated with increased disease risks, especially in females. Women experience a more abrupt and severe change in disease rates related to reproduction, and restoring young ovarian tissue in postreproductive-aged females can significantly increase survival.

Summary: Menopausal hormone therapy has varying effects on cardiometabolic markers and cardiovascular disease risk depending on formulation, dose, and route of delivery. Oral estrogens without progestogens have positive effects on lipoprotein levels, glycemia, insulin, and CVD risk, while adding progestogens blunts the lipid-related effects. Transdermal estrogens have less impact on coagulation, inflammation, and lipids compared to oral estrogens, potentially posing a lower risk of venous thromboembolism and stroke.

Summary: In this cohort study, ever use of estrogen-based hormone therapy was associated with less progression of subclinical atherosclerosis over time in women with and without HIV. This suggests that hormone therapy may confer a similar cardiovascular benefit for both groups.

Summary: We discussed the CV safety and efficacy of subcutaneous testosterone therapy (STT) in postmenopausal women. Our proposed innovative criteria (IDEALSTT) for recommending STT include total testosterone level, carotid artery intima-media thickness, and calculated SCORE for 10-year CVD risk. While hormone replacement therapy (HRT) with testosterone has gained popularity in treating pre and postmenopausal women, the long-term safety and CV risk of STT in women are still controversial.

Summary: Globally, cognitive decline and dementia pose a major challenge to public health. The causes of these conditions can be genetic or triggered by various factors. The association between estrogen decline after menopause and cognitive decline/dementia remains inconclusive. This article explores the prevention of cognitive decline and dementia in postmenopausal women, focusing on pharmacological intervention for primary prevention and diet/weight reduction for secondary prevention. Further research is needed to determine the role of menopausal hormone therapy (MHT) in dementia.

Summary: This Danish cohort study investigated the association between hormone therapy (HT) and disease activity and disability accrual among women with multiple sclerosis (MS). The study found no significant association between HT and disability accrual, although there was a trend for increased risk with longer duration of use. However, current users of HT had a 20% increased risk of recurrent relapse compared to non-users.

Summary: This study found that women living with HIV are rarely prescribed menopausal hormone therapy (MHT) despite a high indication. MHT uptake was associated with discussions about menopause with healthcare providers, emphasizing the need for education among healthcare providers on managing menopause within HIV care settings.

Summary: This study investigated the association between different types of hormone replacement therapy (HRT) and atrial fibrillation (AF). The findings showed that current HRT users, except those using estradiol-only HRT, had significantly elevated AF risk, while past users who used estradiol plus progestin HRT had a reduced AF risk.

Summary: This case illustrates the successful use of oocytes obtained through controlled ovarian hyperstimulation in a gestational carrier for a patient with ovarian cancer to achieve a clinical pregnancy. The findings suggest that in certain clinical situations, fertility preservation through controlled ovarian hyperstimulation may be a viable option for women with ovarian cancer who desire future fertility.

Summary: This systematic review analyzed the effectiveness of minimally invasive approaches to treat fibroids and compared their impact on quality of life and symptoms. The results showed that radiofrequency ablation and ultrasound-guided sclerotherapy significantly improved quality of life, while uterine artery embolization and ultrasound-guided high-intensity frequency ultrasound had similar effects as surgical interventions. Minimally invasive approaches were effective alternatives to surgery for improving quality of life and fibroid-associated symptoms.

Summary: This study aimed to determine the impact of telehealth on high-risk obstetrics patients during the COVID-19 pandemic. By conducting a retrospective chart review, it was found that there were different patterns of telehealth and in-person clinic visits among pregnant women. Factors associated with choosing telehealth included private insurance, anxiety, obesity, and twin pregnancies. The study ultimately concluded that choosing telehealth did not have a significant impact on delivery mode and pregnancy outcomes, suggesting that it may be a beneficial option for patients with certain complications.

Summary: The study identified gaps in Mid-South healthcare providers' awareness of the boxed warning on low-dose vaginal estrogen and their comfort level in providing counseling regarding the risks, benefits, and alternatives to vaginal estrogen. Collaboration between physicians and Advanced Practice Registered Nurses can benefit patients experiencing menopause. Gaps in knowledge and practice for optimal outcomes for patients experiencing genitourinary syndrome of menopause were revealed, providing opportunities for continuing education for providers and improved treatment for patients.
