Prevention of cardiorenal damage: importance of albuminuria

Luis M. Ruilope, Alberto Ortiz, Alejandro Lucia, Blanca Miranda, Gloria Alvarez-Llamas, Maria G. Barderas, Massimo Volpe, Gema Ruiz-Hurtado, Bertram Pitt

Summary: Chronic kidney disease (CKD) is a leading cause of death that requires early detection for effective management. The current diagnostic criteria for CKD only identify advanced stages, leaving a 'blind spot' in detection where kidney injury is present but undetectable. Elevated urinary albumin-excretion rate can predict future cardiovascular events. Implementing opportunistic or systematic albuminuria screening and therapy has the potential to improve cardiorenal outcomes and mitigate the projected burden of CKD and cardiovascular diseases in the future.

Endocrine functions of the heart: from bench to bedside

Massimo Volpe, Giovanna Gallo, Speranza Rubattu

Summary: The heart has an endocrine function, producing biologically active substances such as natriuretic peptides (NPs). NPs play important roles in regulating cardiovascular and renal systems through their interaction with guanylate cyclase-A coupled receptors. They also have potential therapeutic applications in treating heart failure and hypertension, and are used as biomarkers in cardiovascular diseases. Furthermore, research on the regulation of NP synthesis and secretion by microRNAs and epigenetics, as well as their interactions with other endocrine systems, is ongoing.

Cardiovascular risk and the COVID-19 pandemic: A retrospective observational study in a population of healthcare professionals

G. Solfanelli, D. Giaccio, A. Tropea, V. Bucicovschi, G. Gallo, G. Tocci, B. Musumeci, L. Marcellini, E. Barbato, M. Volpe, A. Battistoni

Summary: To contain the spread of COVID-19, many countries implemented restrictive measures, which led to significant changes in daily behaviors. Healthcare workers, who faced increased risk of infection, might have experienced additional stress and developed unhealthy habits. This study investigated the impact of the pandemic on cardiovascular risk in a population of healthy healthcare workers using the SCORE-2 assessment.

An Expert Opinion on the Role of the Rosuvastatin/Amlodipine Single Pill Fixed Dose Combination in Cardiovascular Prevention

Giovanna Gallo, Riccardo Sarzani, Arrigo Francesco Giuseppe Cicero, Stefano Genovese, Matteo Pirro, Luca Gallelli, Andrea Faggiano, Massimo Volpe

Summary: Current cardiovascular disease prevention strategies focus on managing individual cardiovascular risk profiles, often requiring patients to take multiple drugs. Single pill fixed dose combinations can improve blood pressure and cholesterol control compared to separate drug administration, due to better adherence and simplicity. This paper discusses the use of the Rosuvastatin-Amlodipine combination for managing hypertension/hypercholesterolemia and emphasizes the importance of early and effective cardiovascular risk management.

The Emerging Role of Prediabetes and Its Management: Focus on l-Arginine and a Survey in Clinical Practice

Massimo Volpe, Armando Ferrera, Roberto Piccinocchi, Carmine Morisco

Summary: The worldwide increase in metabolic disorders has led to interest in early identification and management of high-risk patients, especially prediabetes. Lifestyle modification and pharmacotherapy, including metformin, vitamin D, and l-arginine, have shown potential benefits in preventing the progression to type 2 diabetes. This review focuses on the role of l-arginine in prediabetes, including its molecular mechanisms and ongoing trials, and reports interesting data from clinical practice in Italy.
