Lung transplantation in patients who have undergone prior cardiothoracic procedures

BACKGROUND: Patients who have undergone prior cardiothoracic procedures offer technical challenges that may affect post-transplant outcomes and be a reason to decline listing. Data are currently limited regarding the indication for lung transplantation among recipients who have had prior cardiothoracic procedures. METHODS: Of 453 lung transplants performed at Cleveland Clinic from January 2005 to July 2010, 206 recipients (45%) had undergone prior cardiothoracic procedures: 157 lung only, 15 cardiac only, 10 cardiac + lung, 10 pleurodesis + lung, and 14 other. Chest tube placement was performed in 202 patients. Survival, post-transplant length of intensive care unit and hospital stays, primary graft dysfunction, and pulmonary function outcomes were compared with outcomes of patients not having prior procedures using propensity score adjustment. RESULTS: Short-term and long-term survival was similar between the 2 groups. Survival at 30 days, 1 year, and 5 years was 94%, 83%, and 55% for the prior cardiothoracic procedure group and 96%, 84%, and 53% for the no prior cardiothoracic procedure group (log-rank p = 0.9). Intensive care unit stay was longer (6 days vs 5 days, p = 0.02) in the prior cardiothoracic procedure group; this was particularly true for pleurodesis + lung (10 days, p = 0.05), although post-transplant hospital stay was similar (16 days, 16 days, and 22 days; p = 0.13). Primary graft dysfunction was not increased in the prior cardiothoracic procedure group. Forced expiratory volume in 1 second was similar for both groups but lower for thoracotomy and lung procedures using a bilateral chest tube (p < 0.05 each). CONCLUSIONS: A prior cardiothoracic procedure is not a contraindication for lung transplantation. (C) 2016 International Society for Heart and Lung Transplantation. All rights reserved.

Summary: This study analyzed the relationship between coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) in the revascularization of left main coronary artery (LMCA) disease, as well as the co-existing non-coronary atherosclerosis (NCA). The results showed that PCI had better clinical outcomes in terms of major adverse cardiovascular and cerebrovascular events (MACCE) and bleeding in patients with NCA compared to CABG. However, no significant difference was observed in patients without NCA.

Extracorporeal membrane oxygenation for postcardiotomy cardiogenic shock after valve replacement

Summary: This study compared the outcomes of ECMO support in patients with mechanical versus bioprosthetic valves after cardiac surgery. The results showed no significant differences between the two groups in terms of ECMO duration, inotropic support, intra-aortic balloon pump, stroke, duration of ICU and hospital stay. Thrombosis can occur in both valve types and is associated with high mortality. The long-term durability of bioprosthetic valves may be affected by ECMO.

Summary: This study evaluated the association between pre-V-A-ECMO arterial lactate level and in-hospital mortality in postcardiotomy patients. The results showed that increased arterial lactate level before V-A-ECMO initiation was associated with higher in-hospital mortality. Therefore, arterial lactate level may be useful in guiding the decision-making process and timing of initiation of postcardiotomy V-A-ECMO.

Summary: This study compared the outcomes of postcardiotomy extracorporeal membrane oxygenation (ECMO) initiation in the operating room (OR) or the pediatric cardiac intensive care unit (PCICU). The results showed that ECMO insertion in the OR had a comparable mortality rate to insertion in the PCICU. Pre-ECMO low pH and high lactate during ECMO were associated with mortality.

Does preoperative screening with computed tomography of the chest decrease risk of stroke in patients undergoing coronary artery bypass grafting

Summary: This retrospective case-control study analyzed 405 patients who underwent primary isolated coronary artery bypass grafting. The study found that the only significant predictor of postoperative stroke was a history of preoperative stroke or transient ischemic attack. However, further prospective studies are needed to investigate the role of computed tomography in preventing postoperative stroke.

Long-Term Effects of Postoperative Atrial Fibrillation following Mitral Valve Surgery

Summary: This study evaluated the incidence, short-term and long-term outcomes of postoperative atrial fibrillation (PoAF) in patients who underwent mitral valve surgery. The overall incidence of PoAF was 16.8%, and it was associated with higher mortality, stroke, and dialysis rates. ICU and hospital stays were significantly longer in patients with PoAF. PoAF was also significantly associated with increased risk of mortality, heart failure rehospitalization, and stroke.

Summary: This study compared the outcomes of PCI and CABG in patients with impaired and normal renal function. The results showed that PCI had a lower risk of in-hospital MACCE in patients with impaired renal function, but there were no significant differences in follow-up MACCE between PCI and CABG.

Applying machine learning methods to predict operative mortality after tricuspid valve surgery

Summary: This study aimed to evaluate the effect of obesity on anticoagulation outcomes and survival in non-valvular atrial fibrillation (AF) patients. The results showed that obesity was not associated with anticoagulation treatment, stroke, or bleeding. However, predictors of mortality were age, heart failure, creatinine clearance, and the type of anticoagulation treatment. Larger randomized trials are recommended.

Mechanical Tricuspid Valves Have Higher Rate of Reintervention: A Single Center Experience

HAS-BLED Score for Prediction of Bleeding and Mortality After Transcatheter Aortic Valve Replacement

Summary: The study evaluated the predictive value of the HAS-BLED score for in-hospital bleeding and mortality after transcatheter aortic valve replacement (TAVR) in a retrospective analysis of 574 patients. The results showed that the HAS-BLED score could be a good predictor of in-hospital mortality after TAVR but had a poor predictive value for bleeding. However, the predictive value for bleeding improved when procedure-specific factors were added and when a creatinine clearance-based definition of renal impairment was used.

Comparison of Outcomes After Transcatheter Versus Surgical Repeat Mitral Valve Replacement

Summary: This study compared the outcomes of repeat transcatheter mitral valve replacement (rTMVR) with surgical mitral valve replacement (rSMVR) in high-risk patients.
