Sarcopenia is an age-related muscle disorder with unclear mechanisms and effective treatments. This article reviews the evidence regarding the definition, diagnosis, risk factors, and efficacy of plant-derived natural products for sarcopenia. Some plant-derived natural products, including curcumin, resveratrol, catechin, soy protein, and ginseng, have been shown to have beneficial effects on sarcopenia without significant side effects.
Sarcopenia is an age-related muscle disorder typically associated with a poor quality of life. Its definition has evolved over time, and several underlying causes of sarcopenia in the elderly have been proposed. However, the exact mechanisms involved in sarcopenia, as well as effective treatments for this condition, are not fully understood. The purpose of this article was to conduct a comprehensive review of previous evidence regarding the definition, diagnosis, risk factors, and efficacy of plant-derived natural products for sarcopenia. The methodological approach for the current narrative review was performed using PubMed, Scopus, and Web of Science databases, as well as Google Scholar (up to March 2021) in order to satisfy our objectives. The substantial beneficial effects along with the safety of some plant-derived natural products including curcumin, resveratrol, catechin, soy protein, and ginseng on sarcopenia are reported in this review. Based on clinical studies, nutraceuticals and functional foods may have beneficial effects on physical performance, including handgrip and knee-extension strength, weight-lifting capacity, time or distance travelled before feeling fatigued, mitochondrial function, muscle fatigue, mean muscle fibre area, and total number of myonuclei. In preclinical studies, supplementation with herbs and natural bioactive compounds resulted in beneficial effects including increased plantaris mass, skeletal muscle mass and strength production, increased expression of anabolic factors myogenin, Myf5 and MyoD, enhanced mitochondrial capacity, and inhibition of muscle atrophy and sarcopenia. We found that several risk factors such as nutritional status, physical inactivity, inflammation, oxidative stress, endocrine system dysfunction, insulin resistance, history of chronic disease, mental health, and genetic factors are linked or associated with sarcopenia. The substantial beneficial effects of some nutraceuticals and functional foods on sarcopenia, including curcumin, resveratrol, catechin, soy protein, and ginseng, without any significant side effects, are reported in this review. Plant-derived natural products might have a beneficial effect on various components of sarcopenia. Nevertheless, due to limited human trials, the clinical benefits of plant-derived natural products remain inconclusive. It is suggested that comprehensive longitudinal clinical studies to better understand risk factors over time, as well as identifying a treatment strategy for sarcopenia that is based on its pathophysiology, be undertaken in future investigations.

Menopause and women?s cardiovascular health: is it really an obvious relationship?

Kamila Ryczkowska, Weronika Adach, Kamil Janikowski, Maciej Banach, Agata Bielecka-Dabrowa

Summary: Cardiovascular diseases are common for both men and women, but there are differences in terms of symptoms, pathophysiology, and treatment response. Women often have a lower perception of the risk, leading to delayed diagnosis and recognition of symptoms. The risk of cardiovascular diseases and the importance of monitoring women's health increase after menopause due to the loss of estrogen's protective effect.

Coffee or tea: Anti-inflammatory properties in the context of atherosclerotic cardiovascular disease prevention

Stanislaw Surma, Amirhossein Sahebkar, Maciej Banach

Summary: Atherosclerotic cardiovascular disease (ASCVD) is the leading cause of premature death worldwide. Inflammation, along with other risk factors such as hypertension, lipid disorders, and diabetes, may contribute to the residual risk of cardiovascular disease (CVD). While modern lipid-lowering treatments are not fully effective in reducing inflammation, selected nutraceuticals with anti-inflammatory properties, including tea and coffee, may have a significant impact on reducing cardiovascular risk and mortality. However, the clinical significance of the anti-inflammatory effects of tea and coffee remains uncertain due to inconsistent results from studies and confounding factors.

Rhabdomyolysis or Severe Acute Hepatitis Associated with the Use of Red Yeast Rice Extracts: an Update from the Adverse Event Reporting Systems

Maciej Banach, Giuseppe Danilo Norata

Summary: Elevated levels of LDL-C increase the risk of atherosclerotic cardiovascular disease (ASCVD), and reducing LDL-C levels can lower the risk of cardiovascular adverse events. Red yeast rice (RYR) has cholesterol-lowering effects due to its bioactive components (monacolins) that inhibit the activity of HMG-CoA reductase. However, the use of RYR as a food supplement at a dose of 10 mg/day raises significant safety concerns, and serious adverse effects have been reported at even lower doses.

Summary: The COVID-19 pandemic, which started in Wuhan, China, in early 2020, has caused a global health crisis. The disease mainly affects the respiratory system but can also have an impact on other systems, including the cardiovascular system. The relationship between atherosclerosis and COVID-19 is bidirectional, with both conditions exacerbating each other. The pandemic has also led to reduced access to healthcare amenities, an increase in sedentary lifestyles and unhealthy eating habits, resulting in a high prevalence of overweight and obese individuals. The long-term healthcare challenges posed by COVID-19 can be mitigated by the lessons learned and the advancements made in patient care during this crisis.
