The US National Cancer Institute estimates that cardiotoxicity (CTX) from target therapy refers mostly to four groups of drugs: epidermal growth factor receptor 2 inhibitors, angiogenic inhibitors, directed Abelson murine leukemia viral oncogene homolog inhibitors, and proteasome inhibitors. The main cardiotoxic side-effects related to antiepidermal growth factor receptor 2 therapy are left ventricular systolic dysfunction and heart failure. Angiogenesis inhibitors are associated with hypertension, left ventricular dysfunction/heart failure, myocardial ischemia, QT prolongation, and thrombosis. Moreover, other agents may be related to CTX induced by treatment. In this study, we review the guidelines for a practical approach for the management of CTX in patients under anticancer target therapy.

Prognostic impact of diabetes in chronic and acute heart failure

Summary: There is a strong bidirectional relationship between diabetes mellitus (DM) and heart failure (HF), with DM activating multiple molecular and pathophysiological mechanisms that can damage the heart in the long term. However, the prognostic role of DM in chronic and acute HF is still unclear, with gaps in evidence due to the heterogeneity of patient phenotypes and the necessity to better characterize individual patients and their comorbidities. This review aims to summarize the negative impact of DM on HF and analyze available data on the prognostic implications in both chronic and acute HF.

Right ventricular failure in left heart disease: from pathophysiology to clinical manifestations and prognosis

Summary: Right heart failure is a clinical syndrome characterized by symptoms and signs caused by dysfunction and/or overload of the right heart structures, mainly the right ventricle, resulting in systemic venous hypertension, peripheral edema, and decreased tissue perfusion ability of the right heart. RHF can be caused by various cardiac diseases, including coronary artery disease, hypertension, valvular heart disease, cardiomyopathies, and myocarditis. Early recognition and identification of the underlying cause of RHF are crucial for effective treatment and prognosis improvement.

Summary: The aim of this study is to develop quality indicators (QIs) for evaluating the prevention and management of cancer therapy-related cardiovascular toxicity. The study followed the ESC methodology and involved identifying key domains of care, conducting a literature review, and selecting the final set of indicators. The study was conducted in collaboration with various associations and resulted in the selection of 5 main and 9 secondary QIs across five domains of care. Overall, this study is of great importance in improving clinical practice and patient outcomes.

Gender Differences and Cardiometabolic Risk: The Importance of the Risk Factors

Summary: Metabolic syndrome is a clinical condition characterized by the aggregation of major risk factors for cardiovascular disease and type 2 diabetes, including dyslipidemia, high blood pressure, abnormal blood sugar, and abdominal obesity. These risk factors have an independent effect but become synergistic when combined, leading to a doubled risk of cardiovascular diseases and a 1.5-fold increase in all-cause mortality. This article explores gender differences in the epidemiology, etiology, pathophysiology, and clinical expression of metabolic syndrome components, as well as gender differences in new biochemical markers of metabolic syndrome and cardiovascular risk.

Summary: Episodes of worsening symptoms and signs are common in patients with chronic heart failure and are associated with negative outcomes. Diuretic therapy is often necessary, along with additional treatments such as guideline-recommended medical therapy. Prevention of worsening heart failure is crucial, and early and rapid administration of appropriate treatments can help achieve this goal. The clinical consensus statement by the Heart Failure Association of the European Society of Cardiology provides an update on the management and prevention of worsening heart failure in clinical practice.

Summary: We conducted a systematic review and meta-analysis to investigate the association between pre-operative vitamin D levels and time-to-outcome in stage III colorectal cancer patients. The results showed that patients with lower levels of vitamin D had an increased risk of death and recurrence. These findings suggest that a low vitamin D concentration negatively impacts the time-to-outcome in stage III colorectal cancer.

Non-invasive evaluation of pulmonary capillary wedge pressure using the left atrial expansion index in mitral valve stenosis, prosthesis and repair

Summary: This study aimed to assess the left atrial expansion index (LAEI) measured through transthoracic echocardiography as a novel parameter for estimating pulmonary capillary wedge pressure (PCWP) in patients with mitral valve stenosis, prosthesis, and surgical repair. The results showed that LAEI correlated logarithmically with PCWP, and the log-transformed LAEI (lnLAEI) had a good linear association with PCWP. lnLAEI was found to be an independent determinant of PCWP and provided added predictive value over conventional clinical and echocardiographic variables.

Left atrial expansion index measured with cardiovascular magnetic resonance estimates pulmonary capillary wedge pressure in dilated cardiomyopathy

Summary: This study aimed to evaluate the feasibility of using the left atrial expansion index (LAEI) calculated from echocardiography as a non-invasive parameter for estimating pulmonary capillary wedge pressure (PCWP) in patients with dilated cardiomyopathy (DCM). The study found a strong linear correlation between LAEI and PCWP, and demonstrated that LAEI can accurately identify patients with high PCWP.

Antineoplastic drugs inducing cardiac and vascular toxicity - An update

Summary: With the improvement in cancer prognosis, there has been an increase in survival rate among cancer survivors. However, the use of anti-neoplastic drugs has led to an increase in morbidity among these survivors due to cardiovascular complications. These complications include arrhythmias, myocardial ischemia, heart failure, arterial hypertension, thromboembolic events, and accelerated atherosclerosis. The drugs not only target cancer cells but also affect normal cells within the cardiovascular system, leading to toxicity.
