Impaired Endothelial Function in Convalescent Phase of COVID-19: A 3 Month Follow Up Observational Prospective Study

This study evaluated the impact of COVID-19 severity on endothelial function three months after the acute phase. The findings showed that hospitalized COVID-19 patients had impaired endothelial function after three months, which was correlated with pulmonary function impairment.
Background: Endothelial dysfunction has a role in acute COVID-19, contributing to systemic inflammatory syndrome, acute respiratory distress syndrome, and vascular events. Evidence regarding COVID-19 middle- and long-term consequences on endothelium are still lacking. Our study aimed to evaluate if COVID-19 severity could significantly affect the endothelial function after three months from the acute phase. Methods: We assessed endothelial function in outpatients with previous COVID-19 three months after negative SARS-CoV-2 molecular test by measuring flow-mediated dilation (FMD) in patients categorized according to a four-variable COVID-19 severity scale (home care; hospital, no oxygen; hospital, oxygen; hospital requiring high-flow nasal canula, non-invasive ventilation, invasive mechanical ventilation, or extracorporeal membrane oxygenation). FMD difference among COVID-19 severity categories was assessed with analysis of variance; we further clarified the relationship between FMD and previous COVID-19 severity with multivariate logistic models. Results: Among 658 consecutive COVID-19 subjects, we observed a significant linear trend of FMD reduction with the increase of the COVID-19 category (p < 0.0001). The presence of endothelial dysfunction was more frequent among hospitalized patients (78.3%) with respect to home-care patients (21.7%; p < 0.0001). COVID-19 severity was associated with increased endothelial dysfunction risk (OR: 1.354; 95% CI: 1.06-1.71; p = 0.011) at multivariate binary logistic analysis. FMD showed a significant direct correlation with PaO2 (p = 0.004), P/F ratio (p = 0.004), FEV1 (p = 0.008), and 6MWT (p = 0.0001). Conclusions: Hospitalized COVID-19 subjects showed an impaired endothelial function three months after the acute phase that correlated with pulmonary function impairment. Further studies are needed to evaluate if these subjects are at higher risk of developing pulmonary disease or future cardiovascular events.

Summary: This retrospective study evaluated the prevalence of SE-IgE sensitization in a multicentric Italian cohort of severe asthmatic patients. SEB-IgE sensitization was found to be associated with female gender and comorbidities such as CRS and CRSwNP. No significant associations were observed between SEB-IgE sensitization and atopy, exacerbations, and corticosteroid dosages. Patients with SEB-IgE sensitization were younger and had an earlier disease onset, suggesting that it may be an independent risk factor for developing asthma.

Identification and management of acute alcohol intoxication

Summary: Acute alcohol intoxication (AAI) is a harmful and potentially life-threatening condition caused by the intake of large amounts of alcohol. It mainly manifests as behavioral and neurological symptoms, and can affect multiple organs and systems. Blood alcohol concentration (BAC) testing is useful in confirming intoxication. Patient stabilization and vital function support are the first steps in managing AAI, and Metadoxine is a useful drug for enhancing alcohol metabolism and elimination. It is important to screen patients with acute intoxication for underlying alcohol use disorders and refer them for multidisciplinary treatment for long-term abstinence. This review focuses on the clinical features, diagnostic criteria, and treatment strategies of AAI.

Radiomics could predict surgery at 10 years in Crohn's disease

Summary: This pilot study aimed to evaluate the use of radiomics in predicting 10-year surgery for Crohn's disease patients. A cohort of patients with CT scan enterographies and a 10-year follow-up was selected. Radiomic features were extracted and a logistic regression model was developed, achieving a successful prediction of 10-year surgery. Radiomics could serve as a helpful tool in identifying patients at high risk for surgery and requiring stricter monitoring.

Changes in Lymphocyte Subpopulations after Remdesivir Therapy for COVID-19: A Brief Report

Summary: This brief report aimed to assess the correlation between Remdesivir (RDV) therapy and variation in lymphocyte subpopulations. The results showed significant changes in lymphocyte subpopulations after RDV therapy, with an increase in B lymphocytes and plasmablasts, and a reduction in cytotoxic T lymphocytes. There was also a decrease in regulatory T cells and an increase in circulating monocytes. Patients with different disease severity showed different lymphocyte count changes after RDV treatment. These findings may suggest a potential regulatory role of RDV treatment in the immune response against the virus and the cytokine storm observed in patients with more severe disease.

Summary: Coronavirus disease 2019 (COVID-19) presents with a variety of extrapulmonary symptoms, with gastrointestinal (GI) symptoms being among the most frequent. Elderly patients are particularly affected, experiencing severe consequences. The impact of COVID-19 on individuals with pre-existing digestive diseases, especially in the older population, is still not fully understood. This review focuses on investigating the effects of COVID-19 on the GI tract, liver, and pancreas in individuals with and without previous digestive diseases, with a specific emphasis on the elderly population. Finally, the effectiveness and adverse events of anti-COVID-19 vaccination in patients with digestive disorders, as well as the unique characteristics observed in the elderly, are discussed.

This review explores the role of nutrients, particularly vitamins and micronutrients, in the relationship between the gut microbiota and the immune response during SARS-CoV-2 infection, as well as the potential benefits of different diets.
