MicroRNA-7 (miR-7) has been characterized as an anti-oncogenic microRNA (miRNA) in several cancers, including hepatocellular carcinoma (HCC). However, the mechanism for the regulation of miR-7 production in tumors remains unclear. Here, we identified nuclear factor 90 (NF90) and NF45 complex (NF90-NF45) as negative regulators of miR-7 processing in HCC. Expression of NF90 and NF45 was significantly elevated in primary HCC tissues compared with adjacent non-tumor tissues. To examine which miRNAs are controlled by NF90-NF45, we performed an miRNA microarray and quantitative RT-PCR analyses of HCC cell lines. Depletion of NF90 resulted in elevated levels of mature miR-7, whereas the expression of primary miR-7-1 (pri-miR-7-1) was decreased in cells following knockdown of NF90. Conversely, the levels of mature miR-7 were reduced in cells overexpressing NF90 and NF45, although primiR- 7-1 was accumulated in the same cells. Furthermore, NF90-NF45 was found to bind pri-miR-7-1 in vitro. These results suggest that NF90-NF45 inhibits the pri-miR-7-1 processing step through the binding of NF90-NF45 to pri-miR-7-1. We also found that levels of the EGF receptor, an oncogenic factor that is a direct target of miR-7, and phosphorylation of AKT were significantly decreased in HCC cell lines depleted of NF90 or NF45. Of note, knockdown of NF90 or NF45 caused a reduction in the proliferation rate ofHCCcells. Taken together, NF90-NF45 stimulates an elevation of EGF receptor levels via the suppression of miR-7 biogenesis, resulting in the promotion of cell proliferation in HCC.

Summary: This study proposed a new method for evaluating the accuracy and efficacy of real-time navigation for anatomic liver resection using the Medical Imaging Projection System (MIPS), which continuously demonstrates the transection plane with ICG fluorescence. The results suggest that real-time navigation with MIPS may facilitate the performance of anatomic liver resection.

Summary: This study evaluated the future liver remnant (FLR) hypertrophy rate in patients undergoing portal vein embolization before left trisectionectomy, finding that PVE is effective in achieving FLR hypertrophy. The study also showed that PVE before left trisectionectomy was equally effective in patients with left portal vein stenosis or occlusion as compared to those without, with acceptable complication rates.

Impact of the aberrant right hepatic artery on local recurrence of pancreatic ductal adenocarcinoma after pancreaticoduodenectomy

Takayoshi Nakajima, Shinichi Ikuta, Ikuo Nakamura, Tsukasa Aihara, Meidai Kasai, Hideaki Iwama, Yasuhiro Fujimoto, Etsuro Hatano, Naoki Yamanaka

Summary: This study aimed to analyze the impact of an aberrant right hepatic artery on local recurrence after pancreatoduodenectomy for pancreatic ductal adenocarcinoma. The results showed that the incidence of local recurrence was higher in the aberrant right hepatic artery group compared to the normal right hepatic artery group, and the aberrant right hepatic artery was identified as an independent risk factor for local recurrence. Combined resection of the aberrant right hepatic artery may reduce local recurrence, especially for tumors near the root of the aberrant right hepatic artery.

Summary: This prospective study investigated the changes in gut microbiota and associated intestinal environment during the early period after liver transplantation. The recipients showed dysbiosis and depletion of organic acid before transplantation, which were related to the severity of liver disease. The transplantation procedure caused minimal changes in gut microbiota and organic acid concentrations in the donors, while the recipients exhibited delayed recovery in these factors.

Current status of liver transplantation for non-B non-C liver cirrhosis and hepatocellular carcinoma

Takahiro Nishio, Takashi Ito, Koichiro Hata, Kojiro Taura, Etsuro Hatano

Summary: Non-B non-C chronic liver diseases, including alcoholic liver disease and nonalcoholic fatty liver disease/nonalcoholic steatohepatitis, have increased significantly worldwide. Liver transplantation is an effective treatment for hepatocellular carcinoma and decompensated liver cirrhosis. Efforts to unify the indications for eligibility of HCC patients for liver transplantation have been made in Japan, leading to the development of 5-5-500 criteria. The management of patients with various comorbidities during the peri-transplant period and risk assessment for liver transplantation are crucial for improving post-transplant outcomes.

Summary: The present study describes an efficient five-step training protocol for rat liver transplantation (LT), which can be mastered within 3 months through continuous training. The training includes watching videos, performing actual operations, and achieving specific goals. Systematizing the training increases its efficiency, and determining the specific number of operations in advance helps maintain motivation.
