Low Adherence to the Mediterranean Diet Is Associated with Poor Socioeconomic Status and Younger Age: A Cross-Sectional Analysis of the EpiDoC Cohort

The Mediterranean diet is recognized as one of the healthiest dietary patterns, but its adherence has been decreasing in Portugal, especially among low socioeconomic groups. This study found that younger individuals with lower income and education level were more likely to have a lower adherence to the Mediterranean diet.
The Mediterranean diet (MD) is recognized as one of the healthiest dietary patterns as it has been consistently associated with several beneficial health outcomes. Adherence to the MD pattern has been decreasing in southern European countries for the last decades, especially among low socioeconomic groups. The aim of this study was to assess the adherence to the MD in Portugal, to evaluate regional differences, and explore associated factors (sociodemographic, economic, and lifestyles behaviors). This study used the third data collection wave of the Epidemiology of Chronic Diseases Cohort Study (EpiDoC 3). MD adherence was assessed using the Portuguese-validated MD adherence score (MEDAS) questionnaire. Non-adjusted and adjusted logistic regression models were used to assess the risk factors for low MD adherence and individual MEDAS items. In this cross-sectional evaluation of the EpiDoC 3 cohort study (n = 5647), 28.8% of the Portuguese population had low adherence to a MD. Azores and Madeira had lower adherence to the MD than the rest of the country. Younger individuals in lower income categories (e.g., ORfinding it very difficult = 1.48; 95% CI 1.16-1.91) and with a lower educational level (e.g., OR0-4 years = 2.63; 95% CI 2.09-3.32) had higher odds of having a lower adherence to the MD. Portuguese adults have a high prevalence of low adherence to the MD, especially among those who are younger and have lower socioeconomic status. Public health policies to promote adherence to the MD should pay special attention to these groups.

Health-related quality of life among spondyloarthritis and chronic low back pain patients: results from a nationwide population-based survey

Summary: The study found that both spondyloarthritis and chronic low back pain have similar impacts on health-related quality of life (HRQoL), including mental, physical, and social domains. Patients with spondyloarthritis and chronic low back pain reported significantly lower HRQoL compared to those without rheumatic diseases. Modifiable factors such as multimorbidity, active disease, obesity, and regular physical exercise were associated with HRQoL in these conditions.

Zgli: A Pipeline for Clustering by Compression with Application to Patient Stratification in Spondyloarthritis

Summary: This paper introduces a similarity measure method called normalized compression distance (NCD), which is based on compression and can be used to construct the initial distance matrix for clustering methods. The authors propose a Python module called Zgli, which allows users to compute the NCD between files in a given folder. The results demonstrate that compression by column can achieve better clustering results for tabular data, and the categorical encoder can enhance the processing of categorical data.

Summary: Portugal has a chronic problem of underfunding in scientific research. Despite increasing national investment in recent years, it is still significantly insufficient. Clinical medicine, immunology, and multicenter epidemiological and cohort studies, which have huge potential for return on investment in clinical trials, are not given priority in the National Health Plan in Portugal, and researcher initiated studies in these fields are underfunded.

Clinical courses, impact and prognostic indicators for a persistent course of low back pain: Results from a population-based cohort study

Summary: This study aimed to investigate the courses of chronic low back pain (CLBP), its cumulative impact, and the indicators for the persistence of pain. The results showed that patients with CLBP could follow a persistent or relapsing course of pain in the long-term. Being female, presenting depressive symptoms, having a manual job, and higher disability at baseline were predictors of a persistent course of LBP.

Long non-coding RNA H19 regulates matrisome signature and impacts cell behavior on MSC-engineered extracellular matrices

Summary: This study decodes the regulatory network of the lncRNA H19 on the extracellular matrix (ECM) and reveals its impact on bone regeneration. Inhibition of H19 expression results in decreased ECM density and collagen content, and promotes adipogenic differentiation of bone marrow-derived stem cells. Furthermore, H19 is targeted by miR-29c, which is downregulated in osteoporotic bone tissue and affects MSC proliferation and collagen production. These findings suggest that H19 is a potential therapeutic target for controlling cell behavior and engineering the bone ECM.

Trajectories and determinants of ageing in Portugal: insights from EpiDoC, a nationwide population-based cohort

Summary: This study examines the health trajectories of Portuguese older adults over a 10-year period and identifies associated sociodemographic, lifestyle factors and multimorbidity status. The findings show that physical disability increased and health-related quality of life declined with advancing age. Women had worse health at baseline but exhibited similar rates of change compared to men. Higher education and regular exercise were associated with better health outcomes, while multimorbidity and excess weight were associated with worse outcomes. This research provides valuable insights for resource allocation, healthcare improvement, and public health policymaking in Portugal.

Disease activity and physical function were found to be major factors influencing HRQoL and GH across different types of SpA. Clinical characteristics and sociodemographic factors also play an important role, highlighting the importance of a holistic approach for individual patients.

Association of physical activity with physical function and quality of life in people with hip and knee osteoarthritis: longitudinal analysis of a population-based cohort

These findings emphasize the importance of maintaining regular physical activity to optimize quality of life and physical function.
