Summary: Rheumatologic diseases can cause inflammation and scarring of the endocardium, leading to valvular thickening and dysfunction. The progression of valve disease is slow but can result in significant complications. The presence of antiphospholipid antibodies increases the risk of valvular disease and thrombotic complications. Management of valvular disease requires a multidisciplinary approach, with medical therapy optimized and consideration of high-risk valve surgery if necessary.

Summary: This study evaluated the impact of incorporating cardiac biomarkers in reclassifying heart failure (HF) and assessed the prognosis of HF using these biomarkers. The results showed that approximately 1 in 5 older adults without prevalent HF were reclassified as Stage B HF, and the measurement of cardiac biomarkers can help identify individuals at higher HF risk.

Summary: Positron emission tomography myocardial perfusion imaging (PET MPI) is a noninvasive diagnostic test that can detect coronary artery disease, structural heart disease, and myocardial flow reserve (MFR). This study aimed to determine the prognostic utility of PET MPI in predicting major adverse cardiac events (MACE) after liver transplant (LT). Reduced global MFR and reduced resting left ventricular ejection fraction on pre-LT PET MPI were associated with an increased risk of post-LT MACE. Awareness of these PET-MPI parameters may help improve cardiac risk stratification of LT candidates if confirmed in future studies.

Summary: This study assessed the association between change in NT-proBNP and the risk for incident heart failure and death. The results showed that change in NT-proBNP was positively correlated with the risk of heart failure and death. Additionally, changes in blood pressure, cholesterol, triglyceride levels, body mass index, and estimated glomerular filtration rate were associated with change in NT-proBNP.

Ongoing initiatives and proposed actions aim to address these barriers and improve patient outcomes.

Summary: This study describes novel uses of implantable invasive hemodynamic monitoring (IHM) in patients with subaortic right ventricles (sRV), which may minimize the need for serial catheterizations and improve management in a population where pulmonary hypertension (PH) is prevalent. The findings suggest that IHM can be used for medical therapy titration, medical management after ventricular assist device, and serial monitoring of pulmonary artery pressures. There were low rates of heart failure hospitalization and device-related complications, indicating that IHM is a safe and effective tool in this patient population.
