Summary: This study investigated the clinical characteristics and cytokines/chemokines profiles among respiratory syncytial virus (RSV), rhinovirus (RV), and their dual infection in Taiwanese children with viral bronchiolitis. The findings revealed that RV bronchiolitis had comparable severity to RSV but was more common in older children with a history of recurrent wheezing and blood eosinophilia. Different viral infections elicited characteristic clinical presentations and immune profiles in bronchiolitis. The study also highlighted the role of the IL-33/IL-31 axis in the immunopathogenesis of bronchiolitis.

Summary: Acute dyspnea is a common symptom that often leads to emergency room evaluation, and its prognosis is important and should be carefully evaluated. The differential diagnosis of dyspnea is challenging due to its lack of specificity and the loose relationship between its intensity and the severity of the underlying condition. Recent evidence suggests that lung ultrasound and echocardiography are valuable diagnostic tools for assessing heart, lung, or extra-pulmonary involvement in patients with dyspnea. Non-invasive respiratory support modalities like high-flow nasal oxygen and continuous positive airway pressure have gained considerable interest for effectively treating dyspnea without invasive mechanical ventilation. This clinical review focuses on the pathophysiology, clinical presentation, evaluation, and non-invasive respiratory support options for acute dyspnea patients.

Summary: The study aimed to evaluate the association between potential emergency department-based modifiable risk factors and subsequent development of delirium among hospitalized older adults free of delirium at the time of ED stay. The findings showed that the length of stay and opioid use in the ED were not associated with an increased risk of incident delirium. However, urinary catheterization in the ED was found to be associated with a higher risk of subsequent delirium.
