Summary: Vertebral hemangioma resection is a challenging procedure for spine surgeons due to the risk of massive bleeding. Preoperative transarterial embolization is recommended to reduce bleeding risk. In this article, the authors discuss an unusual clinical and radiologic presentation following an endovascular procedure and provide an explanation for the findings.

Summary: COVID-19 in immunocompromised patients is difficult to treat, and the understanding of SARS-CoV-2 interaction with the host immune system and therapy role is limited. The use of monoclonal antibodies and antiviral combinations for viral replication and disease progression is lacking data. Here, we report successful treatment of two COVID-19 patients with positive SARS-CoV-2 RNAemia, who had undergone rituximab treatment for non-Hodgkin lymphoma and granulomatosis with polyangiitis, respectively, using a salvage combination therapy with sotrovimab, remdesivir, and nirmatrelvir/ritonavir.

Summary: This study assessed the effect of remdesivir on COVID-19 mortality among elderly individuals using real-world data. The results showed that remdesivir reduced mortality in patients requiring oxygen support or non-invasive mechanical ventilation, but no association was found among patients necessitating invasive mechanical ventilation.

Monkeypox outbreak in Genoa, Italy: Clinical, laboratory, histopathologic features, management, and outcome of the infected patients

Summary: Since May 2022, there have been multiple cases of human Monkeypox reported in nonendemic countries, primarily among men who have sex with men. This study focuses on the characterization, clinical course, management, and outcome of Monkeypox cases diagnosed in a hospital in Italy. The cases showed typical symptoms including genital skin/mucosal lesions and systemic signs. Histopathology revealed consistent features of epidermal ulceration and dermal inflammatory infiltrate. Concurrent sexually transmitted infections, especially anal high-risk human papillomavirus (HR-HPV) infection, were frequently observed. The overall prognosis was favorable with minimal complications.

Efficacy of Dolutegravir versus Darunavir in Antiretroviral First-Line Regimens According to Resistance Mutations and Viral Subtype

Summary: This study compared the efficacy of darunavir-based regimens and dolutegravir-based regimens in clinical practice, with a focus on pre-treatment drug resistance mutations and HIV-1 subtype. The results showed that dolutegravir-based regimens had a superior efficacy in terms of time to virological failure compared to darunavir-based regimens, and pre-treatment drug resistance mutations and HIV-1 subtype influenced the treatment outcomes.

Ultra-rare RTEL1 gene variants associate with acute severity of COVID-19 and evolution to pulmonary fibrosis as a specific long COVID disorder

Summary: This study found that up to 8.6% of severe COVID-19 patients carry ultra-rare variants of the RTEL1 gene and demonstrated how to recognize this subgroup. These patients show higher liver function indices, increased CRP and inflammatory markers, and a higher prevalence of autoimmune disorders. Additionally, their decreased diffusion lung capacity for carbon monoxide after six months of COVID-19 suggests that RTEL1 variants contribute to the development of SARS-CoV-2-induced lung fibrosis.

Summary: Monoclonal antibodies have been shown to be effective in the treatment of COVID-19, but real-world data during the Omicron variant surge are still lacking. This multicentre retrospective study evaluated the effectiveness of sotrovimab and casirivimab-imdevimab in mild SARS-CoV-2 infection patients and found that supplemental oxygen therapy before infection and solid malignant tumors are risk factors for treatment failure.

Real-life safety of Emtricitabine/Tenofovir Alafenamide/Bictegravir

Summary: The study aimed to investigate the metabolic and hepatic safety of FTC/TAF/BIC in a real-life setting. The results showed that switching to FTC/TAF/BIC significantly improved lipid profile in ART-experienced PLWH, with slight variations in blood glucose and ALT levels without clinical relevance. In ART-naive PLWH, blood lipids increased without worsening lipid profile, and there was a trend towards normalization of liver enzymes. FTC/TAF/BIC demonstrated good tolerability in the real-life setting.
