Antiphospholipid syndrome (APS) and its severe manifestation Catastrophic APS (CAPS) may be affected in patients with COVID-19 infection, especially in critically ill cases. COVID-19 may exacerbate the pathological effects of APS, leading to the onset of vascular diseases.
Antiphospholipid syndrome (APS) is an autoimmune disorder characterized by multiple episodes of venous and arterial thromboses or recurrent fetal losses in the presence of antiphospholipid antibodies against beta(2)GP1, frequently accompanied by moderate thrombocytopenia. Catastrophic APS (CAPS) is a severe manifestation of APS. COVID-19 may have an intense hypercoagulable state in critically ill patients. SARS-CoV2 may potentiate pathogenic APS effects, including the activation of endothelial cells, monocytes, platelets, and complement, resulting in a proinflammatory state and prothrombotic events. The endothelial tropism of SARS-CoV2 may also modify the clinical presentation of COVID-19 in susceptible individuals and trigger flares of underlying vascular diseases. We report a case of a 64-year-old woman with a history of triple-positive APS who had multiple thrombotic and bleeding episodes after being found to have a COVID-19 infection temporally associated with CAPS development that was successfully treated with eculizumab, preventing further macro- and microvascular thrombotic events at 1 month follow-up. Our case highlights the need for more research regarding the mechanism by which COVID-19 may potentiate APS and lead to the development of CAPS.

Summary: The physical function of patients with idiopathic inflammatory myopathies (IIMs) is significantly poorer compared to those with non-IIM autoimmune rheumatic diseases (AIRDs) or healthy controls (HCs). Even in patients with inactive disease, physical function remains impaired. Factors such as older age, female gender, longer disease duration, and specific diagnoses were identified as independent factors contributing to lower PROMIS PF-10a scores.

Summary: In this study, hesitancy in patients with autoimmune rheumatic diseases (AIRDs) towards COVID-19 vaccines was investigated. The results showed a decrease in hesitancy, but an increase in concerns over the long-term safety of the vaccine. Patients with idiopathic inflammatory myopathies (IIMs) expressed more skepticism towards vaccine science and had more concerns over long-term safety.

Summary: Using global surveys, this study investigated the incidence, predictors, and patterns of flares following COVID-19 vaccination in individuals with autoimmune rheumatic diseases (AIRDs). The results showed that flares occurred in nearly 1 in 10 AIRDs patients after vaccination, with higher vulnerability in patients with comorbidities, mental health disorders, and those receiving the Moderna vaccine. The presence of multiple autoimmune diseases and mental health disorders, as well as receiving the Moderna vaccine, were predictive of flares, while the use of MMF and glucocorticoids were protective. Further research and optimization of vaccine strategies are needed.

Summary: This study investigated the safety of COVID-19 vaccines in pregnant and breastfeeding women with autoimmune diseases. The results showed that adverse events were more frequent in pregnant women compared to non-pregnant patients, but there was no significant difference compared to pregnant healthy controls. Summary: Limited evidence on long-term COVID-19 vaccine safety in patients with idiopathic inflammatory myopathies (IIMs) contributes to vaccine hesitancy. This study analyzed delayed-onset vaccine adverse events (AEs) in patients with IIMs, other systemic autoimmune and inflammatory disorders (SAIDs), and healthy controls (HCs). The findings showed that vaccination is safe in individuals with IIMs, with AEs and hospitalization rates comparable to SAIDs, and mostly limited to those with autoimmune multimorbidity and active disease. These observations can inform guidelines for identifying high-risk patients in the post-vaccination period.

Summary: The light-scattering effect of hypertriglyceridemia can interfere with the photometric analysis of electrolytes, leading to errors in laboratory values. We present a case of erroneously low bicarbonate levels due to severe hypertriglyceridemia. Understanding conditions like hypertriglyceridemia that can interfere with electrolyte measurements is important in everyday clinical medicine.

Summary: This study analyzed delayed adverse events (ADEs) related to COVID-19 vaccination in patients with systemic sclerosis (SSc) and other systemic autoimmune and inflammatory disorders (SAIDs). The results showed that the risk of delayed ADEs after vaccination was not higher in SSc patients compared to other SAIDs and healthy controls. However, SSc patients with diffuse cutaneous phenotype and coexisting autoimmune conditions such as myositis and thyroid disease may be more prone to experiencing minor ADEs.
