Summary: The prophylactic administration of tranexamic acid (TXA) before cesarean section significantly reduces intraoperative, 2-hour postpartum, and total blood loss, as well as the decline in hemoglobin. However, it does not significantly affect blood loss at 6-hour postpartum. Therefore, prophylactic intravenous TXA is helpful in preventing perioperative bleeding in women undergoing cesarean section.

The study found substantial differences in the incidence of emergency peripartum hysterectomy across income settings, with higher rates observed in lower-income settings. Women in lower-income settings also experienced higher morbidity and mortality related to the procedure.

Summary: A total of 14 studies including 11,944 patients were analyzed to evaluate the effectiveness of pharmacologic VTE prophylaxis in postpartum patients. The results suggested that there was no significant difference in the risk of VTE between patients who were exposed to postpartum pharmacologic VTE prophylaxis and those who were unexposed. However, due to the rarity of VTE events, the current literature provided an insufficient sample size to draw a conclusion.

Summary: This study aimed to identify and quantify risk factors for atonic postpartum hemorrhage through a systematic review of English language studies. Out of 47 potential risk factors identified, 15 were classified as definite or likely risk factors, while the remaining 32 showed no association or had conflicting evidence with atonic postpartum hemorrhage. Many established risk factors were confirmed, with additional novel factors such as hypertension, diabetes, and ethnicity identified.

Additionally, IOCS use reduced the incidence of adverse events related to allogeneic blood transfusion.

Summary: This study aimed to identify trends in pregnancy outcomes, especially delivery mode, among pregnant patients older than 45 years. A total of 24 retrospective, observational studies were included in the review. The results showed that pregnant patients aged >= 45 years had a significantly higher cesarean delivery rate and a lower emergency cesarean delivery rate compared to those aged <45 years. In addition, older pregnant patients had a lower assisted delivery rate and a higher risk of preeclampsia, gestational diabetes mellitus, placenta previa, placental abruption, postpartum hemorrhage, and preterm birth.

Summary: Contrary to other developed countries, in Japan, recent years have seen increases in cervical cancer incidence and mortality among young people. However, the human papillomavirus (HPV) vaccine program, a key measure for avoiding cervical cancer, has been virtually suspended. This study aimed to determine the current status and future trends of cervical cancer and precancerous lesions in Japan. The findings revealed an increasing cervical disease burden among reproductive age females in Japan, and projections show that without effective implementation of HPV vaccine programs and screening, there will be further increases in cervical cancer incidence and mortality.

Summary: Therapeutic options for rare cancers are limited, necessitating the development of novel strategies. Our clinical study showed that a biweekly WT1 Trio peptide vaccine induced stronger immune responses against WT1 compared to a weekly WT1-CTL peptide vaccine. The vaccine was confirmed to be safe, with only one patient experiencing severe adverse events. Fifteen out of 45 patients achieved stable disease after 3 months of treatment, suggesting that WT1-targeted immunotherapy may be a potential strategy for rare cancers.

Summary: This study aimed to review the current evidence on type III vasa previa. A systematic review showed that type III vasa previa accounts for 5.7% of vasa previa cases. The characteristics and obstetric outcomes of type III vasa previa were similar to type I vasa previa. Further studies are warranted to obtain more evidence on type III vasa previa.

Summary: The purpose of this study was to investigate the clinical practices of diagnosing multicystic cervical lesions in order to develop a more appropriate diagnostic algorithm for gastric-type adenocarcinoma (GAS) and its precursors. Clinical information for 159 surgically treated patients was collected and reviewed, and the combination of cytology and MRI findings significantly improved the diagnostic accuracy for differentiating benign disease from premalignancy and malignancy.

Summary: In November 2021, the Japanese government reversed its 2013 decision to suspend the proactive recommendation for HPV vaccination. However, the implementation of routine and catch-up vaccinations for young girls did not go as planned. A nationwide questionnaire survey conducted in September 2022 revealed disparities among municipalities in their HPV vaccine notification processes. Monitoring HPV vaccination rates and incidence rates of cervical cancer and precancerous lesions in each municipality will be important for future research.

Summary: This study assesses the feasibility of minimally invasive surgery (MIS) for well-selected epithelial ovarian cancer (EOC) patients. Data from a single center from 2017 to 2022 were reviewed, and a meta-analysis of similar studies comparing laparoscopy and laparotomy outcomes was performed. The results support the possibility of conducting MIS for EOC in well-selected patients.

Summary: The aim of this study was to compare the survival outcomes of different histological subtypes of cervical cancer in stage IVB. The results indicated that patients with small-cell neuroendocrine carcinoma (SCNEC) had the shortest survival. Furthermore, SCNEC was associated with significantly decreased survival compared to other histological subtypes, except for those treated with surgery.
