Association between Endometriosis and Delivery Outcomes: A Systematic Review and Meta-Analysis

Endometriosis is a common gynecological disorder that can have an impact on delivery outcomes, including an increased risk of instrumental delivery and cesarean section. It may also be associated with a higher rate of postpartum hemorrhage and perioperative complications during cesarean section.
Endometriosis is a common benign gynecological disorder; however, delivery outcomes concerning pregnancies with endometriosis remain understudied. This study aimed to assess the effect of endometriosis on delivery outcomes, including the rate of instrumental delivery, cesarean delivery (CD), postpartum hemorrhage (PPH), and perioperative complications during CD. A systematic literature review was conducted using multiple computerized databases, and 28 studies met the inclusion criteria. Pooled analysis showed that histologically diagnosed endometriosis was associated with an increased rate of instrumental delivery (odds ratio [OR] 1.26, 95% confidence interval [CI] 1.02-1.56) and an increased risk of CD (adjusted OR 2.59, 95%CI 1.32-5.07). In our analysis, histologically diagnosed endometriosis was not associated with an increased rate of PPH; however, one retrospective study reported that endometriosis increased the rate of PPH during CD (adjusted OR 1.7, 95%CI 1.5-2.0). Two studies examined perioperative complications during CD, and women with deep endometriosis had a higher rate of bowel resection or bladder injury than those without endometriosis. Our findings suggest that endometriosis is a significant risk factor for instrumental delivery and CD and may be associated with a higher rate of PPH and intraoperative complications during CD.

Overview of laparoscopic surgery for cervical cancer in Japan: Updates after the laparoscopic approach to cervical cancer trial

Summary: Laparoscopic radical hysterectomy (LRH) has been shown to have similar oncologic outcome as abdominal radical hysterectomy (ARH) for cervical cancer in retrospective studies. However, recent research indicates that minimally invasive approaches are associated with a higher rate of recurrence and death. The reasons for the poorer prognosis of LRH compared to ARH are currently unclear.

Trends and future projections of cervical cancer-related outcomes in Japan: What happens if the HPV vaccine program is not implemented?

Summary: Contrary to other developed countries, in Japan, recent years have seen increases in cervical cancer incidence and mortality among young people. However, the human papillomavirus (HPV) vaccine program, a key measure for avoiding cervical cancer, has been virtually suspended. This study aimed to determine the current status and future trends of cervical cancer and precancerous lesions in Japan. The findings revealed an increasing cervical disease burden among reproductive age females in Japan, and projections show that without effective implementation of HPV vaccine programs and screening, there will be further increases in cervical cancer incidence and mortality.

Summary: Therapeutic options for rare cancers are limited, necessitating the development of novel strategies. Our clinical study showed that a biweekly WT1 Trio peptide vaccine induced stronger immune responses against WT1 compared to a weekly WT1-CTL peptide vaccine. The vaccine was confirmed to be safe, with only one patient experiencing severe adverse events. Fifteen out of 45 patients achieved stable disease after 3 months of treatment, suggesting that WT1-targeted immunotherapy may be a potential strategy for rare cancers.

Summary: This study aimed to review the current evidence on type III vasa previa. A systematic review showed that type III vasa previa accounts for 5.7% of vasa previa cases. The characteristics and obstetric outcomes of type III vasa previa were similar to type I vasa previa. Further studies are warranted to obtain more evidence on type III vasa previa.

Summary: The purpose of this study was to investigate the clinical practices of diagnosing multicystic cervical lesions in order to develop a more appropriate diagnostic algorithm for gastric-type adenocarcinoma (GAS) and its precursors. Clinical information for 159 surgically treated patients was collected and reviewed, and the combination of cytology and MRI findings significantly improved the diagnostic accuracy for differentiating benign disease from premalignancy and malignancy.

Disparities among Japanese municipalities in recommendations for routine and catch-up HPV vaccinations

Summary: In November 2021, the Japanese government reversed its 2013 decision to suspend the proactive recommendation for HPV vaccination. However, the implementation of routine and catch-up vaccinations for young girls did not go as planned. A nationwide questionnaire survey conducted in September 2022 revealed disparities among municipalities in their HPV vaccine notification processes. Monitoring HPV vaccination rates and incidence rates of cervical cancer and precancerous lesions in each municipality will be important for future research.

Summary: This study assesses the feasibility of minimally invasive surgery (MIS) for well-selected epithelial ovarian cancer (EOC) patients. Data from a single center from 2017 to 2022 were reviewed, and a meta-analysis of similar studies comparing laparoscopy and laparotomy outcomes was performed. The results support the possibility of conducting MIS for EOC in well-selected patients.

Summary: A population-based study was conducted to investigate the efficacy of minimally invasive surgery in the treatment of uterine-confined and node-negative cervical cancer in Japan. The results showed that there was no difference in survival outcomes between minimally invasive surgery and open surgery in Japanese patients with FIGO 2018 stage I cervical cancer.

This study reviewed 48 studies published between 1974 and 2022 to determine the outcomes and prevalence of patients with abnormal placentas. The cumulative prevalence of different types of abnormal placentas was reported, along with their associations with adverse pregnancy outcomes.

Population-Based Survival Analysis of Stage IVB Small-Cell Neuroendocrine Carcinoma in Comparison to Major Histological Subtypes of Cervical Cancer

Summary: The aim of this study was to compare the survival outcomes of different histological subtypes of cervical cancer in stage IVB. The results indicated that patients with small-cell neuroendocrine carcinoma (SCNEC) had the shortest survival. Furthermore, SCNEC was associated with significantly decreased survival compared to other histological subtypes, except for those treated with surgery.
