Summary: This study evaluated the impact of pregnancy physiology and medication non-adherence on the pharmacokinetics and exposure-response of hydroxychloroquine (HCQ) in patients with systemic lupus erythematosus (SLE). The results showed that medication non-adherence had a more significant effect on HCQ exposure than physiological changes during pregnancy. Non-adherent HCQ concentrations in pregnant women were associated with higher rates of preterm birth. Optimizing adherence may be more clinically meaningful than adjusting HCQ dosage to account for physiological changes in pregnancy. Serum HCQ concentrations <=100 ng/mL may indicate non-adherence and help identify pregnancies at risk for poor outcomes.

Summary: Recent guidelines recommend using a lower dose of hydroxychloroquine (HCQ) to reduce the risk of retinopathy in SLE patients. To evaluate the impact of dose reduction, we compared flare incidence in SLE patients treated with different oral HCQ dosages and analyzed HCQ blood levels. The study found that higher HCQ blood levels and older age were protective against flares, while concomitant immunosuppressant therapy increased the risk.
