HF and iSGLT2 have been key topics in diabetology, with a focus on T2D, but research on HF in T1D is lacking. Understanding the characteristics, risk factors, and pathophysiology of HF in T1D is crucial for developing effective prevention strategies and therapies. The association between T1D and HF has a worse prognosis compared to T2D, highlighting the need for further exploration in this area.
Heart failure (HF) has been a hot topic in diabetology in the last few years, mainly due to the central role of sodium-glucose cotransporter 2 inhibitors (iSGLT2) in the prevention and treatment of cardiovascular disease and heart failure. It is well known that HF is a common complication in diabetes. However, most of the knowledge about it and the evidence of cardiovascular safety trials with antidiabetic drugs refer to type 2 diabetes (T2D). The epidemiology, etiology, and pathophysiology of HF in type 1 diabetes (T1D) is still not well studied, though there are emerging data about it since life expectancy for T1D has increased in the last decades and there are more elderly patients with T1D. The association of T1D and HF confers a worse prognosis than in T2D, thus it is important to investigate the characteristics, risk factors, and pathophysiology of this disease in order to effectively design prevention strategies and therapeutic tools.

Summary: This study aimed to evaluate the effect of a very-low-calorie ketogenic diet (VLCKD) compared to a standard, balanced hypocaloric diet (LCD) or bariatric surgery (BS) on the inflammatory and oxidative stress profile in patients with obesity. The results showed that VLCKD induced more notable changes in cytokines and lipid peroxides, especially during the ketosis phase of the intervention.

Summary: Obesity-related glomerulopathy (ORG) is a silent comorbidity associated with serious health consequences. Despite the pathogenic mechanisms not fully understood, factors such as glomerular hemodynamic changes and RAAS overactivation are believed to play a major role. Early diagnosis and intervention are crucial for improving the prognosis of ORG.

A 96-week, double-blind, randomized controlled trial comparing bexagliflozin to glimepiride as an adjunct to metformin for the treatment of type 2 diabetes in adults

Summary: This study compared the effects of bexagliflozin tablets and glimepiride on the treatment of patients with type 2 diabetes. It was found that bexagliflozin tablets were noninferior to glimepiride in lowering HbA1c levels, and were superior in reducing body mass and systolic blood pressure. Additionally, bexagliflozin tablets were associated with fewer hypoglycemic events and had a favorable effect on estimated glomerular filtration rate (eGFR).

An overview of vitamins as epidrugs for colorectal cancer prevention

Summary: Gene expression can be regulated by epigenomic modifications such as DNA methylation, histone modification, and chromosome remodeling. Vitamins, as natural bioactive compounds, have been found to modify epigenetic patterns and potentially impact the risk of cancer, including colorectal cancer (CRC). This review examines the relationship between vitamins and CRC risk through epidemiological and observational studies and discusses the potential of using vitamins as epigenetic modifiers for CRC treatment. Although more studies with better designs are needed, understanding the molecular mechanisms of vitamin-mediated epigenetic regulation can help identify effective therapeutic targets for CRC prevention and treatment.
