Summary: This study aimed to analyze the distribution of functional capacity in older adults with aortic stenosis, its association with decision-making regarding valve disease, and its impact on one-year mortality rate. The results showed that increased functional capacity was associated with a tendency for more invasive treatment, and the Functional Continuum Scale (FCS) was identified as a useful predictive tool for clinical decision-making.

Summary: This study aimed to investigate the association of the multidimensional prognostic index (MPI) based on comprehensive geriatric assessment (CGA) with survival in elderly patients undergoing TAVI. The results showed that patients in the MPI-1 group had a higher overall survival rate compared to those in the MPI-2-3 group, suggesting that the MPI tool could be useful for assessing frailty and predicting which patients may benefit more from TAVI procedures.

Summary: This study investigates the association between frailty and mortality. It finds that frailty is associated with higher mortality compared to pre-frailty. Variables such as sex, age, weight, and the use of proton-pump inhibitors are significantly associated with mortality. In the frail cohort, readmission, SPPB sit and stand, heart failure, and the use of certain medications also increase the risk of mortality. In the pre-frail cohort, factors such as Barthel's score, Pfeiffer test, and various comorbidities contribute to higher mortality risk.

Summary: The study compared short and long-term outcomes of nursing home residents and community dwelling older adults undergoing surgery for pelvic organ prolapse. Despite matching on several characteristics, nursing home residents showed increased risk of longer hospital stay, complications, and mortality compared to community dwelling older adults, suggesting additional vulnerabilities contributing to surgical risk in this population.

Summary: Current evidence is insufficient to support different hypertension treatment targets in older adults. This study evaluated whether frailty modifies the association between systolic blood pressure (SBP) and 8-year all-cause mortality in community-dwelling older adults. The results suggest that frailty may have a protective effect on all-cause mortality in frail older adults with respect to elevated SBP.

Summary: The study found that actigraphy-measured sleep quality was significantly associated with risk of frailty and mortality over a 5-year period among community-dwelling older adults. Sleep fragmentation index, wake after sleep onset, and percent sleep were found to be significantly correlated with frailty and mortality.

Summary: Social characteristics are important predictors of mortality, but current prediction models mainly focus on demographics, medical comorbidities, and function. There is a lack of efficient methods to incorporate social factors into risk prediction models. This study developed and validated a comprehensive measure of social risk, which showed better predictive capacity than traditional risk models.

Summary: This study discovered that the severity of baseline frailty in older adults before receiving ICU care is associated with survival, with an increased risk of mortality as frailty worsens. The survival prediction models adjusted for frailty measures were more discriminative compared to reference models that only considered age, sex, and other factors.

Summary: This paper analyzes the frailty in older adults suffering from diabetes and the combined effect of diabetes and frailty on mortality risk. It is found that frailty among older adults with diabetes increases the risk of poor health outcomes such as death. Therefore, screening and assessing frailty should be emphasized when diagnosing, treating, and managing older adults with diabetes to identify it early and take appropriate intervention measures to avoid or delay adverse effects.

Summary: The study evaluated the use of the EFT in patients undergoing coronary artery bypass grafting procedures. It was found that the EFT could predict patient survival and had a significant impact on the survival rates. After adjustment, each incremental EFT point was associated with a higher risk of mortality.

Summary: This study aimed to evaluate outcomes in older severe aortic valve stenosis patients and select treatment according to frailty levels. The results showed that patients with early frailty levels were the most suitable candidates for surgical or transcatheter aortic valve replacement, while advanced frailty would make each treatment futile or palliative.

Summary: This study aimed to determine the appropriate time and frailty index (FI) threshold for frailty intervention in Chinese community-dwelling older adults. Using the FI to evaluate frailty and define frailty status, the researchers compared the relationships of FI and frailty status with mortality to determine the best time point and FI score for frailty intervention. The results showed that using FI as a continuous variable to predict death is more accurate than frailty status, and early interventions for mortality risk reduction are more beneficial in pre-frail patients. An FI score of 0.5 was found to be the threshold for mortality prediction using the FI.

Summary: Postinfarction ventricular septal rupture carries a high mortality risk, but there is a trend towards younger patients over the past decade. In the second time period, there was a significant decrease in one-year mortality and an increase in the use of venoarterial extracorporeal membrane oxygenation, leading to higher survival rates.

Summary: Mechanical complications are common in myocardial infarction patients, with a high in-hospital mortality rate. There has been no significant improvement in survival rates of these complications over the past few years. Advanced age, cardiogenic shock, and cardio-respiratory failure are the most important risk factors for in-hospital mortality. Patients have better outcomes in cardiac intensive care units with more experience and specialization.

Summary: The study found that patients with left-sided infective endocarditis complicated by congestive heart failure tend to be older, have more comorbidities, and more severe valve damage, leading to significantly higher 30-day and 1-year mortality rates. Summary: Low-flow low-gradient AS affects 10.5% of severe AS patients, with a lower need for AVR compared to the HG group and similar to the NFLG group, with no differences in mortality. AVR has a lower impact on LFLG AS compared with HG AS. The study shows LFLG AS to have an intermediate clinical risk profile between the HG and NFHG groups.
