Summary: This study assessed the outcomes and characteristics of postpancreatectomy hemorrhage (PPH) in over 1000 patients who underwent pancreatoduodenectomy (PD) at a high-volume hepatopancreaticobiliary center. The results showed that PPH occurred in 3.0% of patients who underwent PD, with an in-hospital mortality rate of 1.0% and a 90-day mortality rate of 0%. Early and late PPH affected 9 and 24 patients, respectively, with 16 patients experiencing late-extraluminal PPH. Standardized management of PPH based on the onset and source of hemorrhage can minimize serious deterioration and mortality.

Summary: A predictive model for conversion surgery was developed and the long-term outcomes of patients with technically unresectable colorectal liver metastases were evaluated. Patients who underwent conversion surgery had significantly higher median overall survival compared to those who did not. The predictive model for conversion surgery was constructed using several predictive factors. Patients were graded according to their scores, and those in Grade A had significantly better overall survival than Grade B and C patients.

The study found that an initial positive proximal hepatic duct margin was associated with poor long-term outcomes for patients with resectable PHCC. Additional resection after achieving a negative margin had minimal impact on survival.

The average age of surgical patients is steadily increasing, with more than 20% of patients aged 80 years or older. Overall, Japanese gastroenterological surgeries have shown good safety.
