Summary: This study examined the impact of COVID-19 on solid organ waiting list mortality in the United States, finding a significant increase in kidney waiting list mortality after the national emergency declaration. Liver, pancreas, lung, and heart waiting list mortality did not significantly change, with kidney candidates showing variability across donation service areas and an increase in mortality for Black candidates compared to White candidates. The first 10 weeks after the declaration of a national emergency had a heterogeneous effect on waitlist mortality rate, varying by geography and ethnicity, complicating comparisons of transplant program performance during COVID-19.
