The antibiotics of choice for the treatment of melioidosis in Indian set up

Therapeutic options for the treatment of melioidosis caused by Burkholderia pseudomallei are limited due to the inherent resistance conferred by this pathogen to various groups of antibiotics. Witnessing an increase in the number of microbiological culture-confirmed cases of melioidosis at our settings in the past few years, we undertook this study to estimate the minimum inhibitory concentrations of clinical isolates of B. pseudomallei against the four commonly employed antimicrobial agents in the patient management at our settings, namely, ceftazidime, meropenem, trimethoprim-sulfamethoxazole and doxycycline. All isolates were susceptible to the antibiotics tested, except for one isolate which showed resistance to doxycycline (minimum inhibitory concentration [MIC]: 32 mg/ml). MIC50 and 90 for all the four antibiotics were estimated. From this study, we conclude that the clinical isolates of B. pseudomallei from the southern part of India are well susceptible to the commonly employed antimicrobial agents for therapy.

Epidemiology of sepsis and risk factors for mortality in intensive care unit: a hospital based prospective study in South India

Summary: The study aimed to elucidate the epidemiology of sepsis in a South Indian university hospital, focusing on common bacterial causes, MDR infection proportions, and risk factors for 28-day mortality. Male gender, hypertension, SOFA > 9, and elevated creatinine values were identified as predictors of mortality. Nearly half of cases did not have a detected infectious cause using routine microbiology culture methods, highlighting the need for increased awareness of early sepsis diagnosis in community settings.

Adjuvant therapy with vitamin D can help manage infections caused by biofilm-producing microbes due to its immunomodulatory role in inducing antimicrobial peptides and reducing proinflammatory cytokines.

Antimicrobial Property of Silver Nanoparticles: Effects of Concentration and Temperature on Bacterial Isolates

Summary: The importance of the hospital environment in infection prevention and control has been widely recognized. Inappropriate antibiotic use has led to the emergence of drug-resistant strains that are difficult to treat. Nanotechnology has enhanced the antimicrobial properties of nanoparticles, overcoming the limitations of common antimicrobials. This study investigated the antimicrobial role of silver nanoparticles (Ag-NPs) against clinically important bacterial strains and observed the effects of varying storage temperatures on Ag-NPs' antimicrobial activity.

Recurrent bacteremia with Enterococcus faecalis, the clinical findings predicting endocarditis, and genomic characterization of the isolates: a retrospective cohort study

Summary: This study aimed to investigate the clinical presentation of patients with Enterococcus faecalis bacteremia (EfsB), focusing on the risk of recurrent infection and infective endocarditis (IE), as well as identifying potential improvements in management. Retrospective analysis of a cohort of patients with monomicrobial EfsB episodes revealed 69 patients with IE and 43 with recurrent infections. Factors significantly associated with IE included prolonged symptoms, growth in all blood cultures, unknown origin of infection, heart murmur, and predisposition for IE. Whole genome sequencing and multilocus sequence typing of E. faecalis isolates from multiple episodes in the same patient showed identical sequence types in 28 of 31 patients. Episodes of EfsB in patients later diagnosed with IE displayed characteristics of IE already in the first episodes and were likely true relapses. Risk factor analysis should guide the use of echocardiography.

Molecular detection of antimalarial resistance in Plasmodium vivax isolates from a tertiary care setting in Puducherry

Summary: The study aimed to detect the mutation patterns in drug targets in Plasmodium vivax that develop resistance to common antimalarial agents in India. By analyzing 27 Plasmodium vivax isolates, the study found an increase in the proportion of double mutants in pvmdr1 and pvdhfr over time. Cases with double mutant pvmdr1 gene had a prolonged hospital stay compared to those without, indicating reduced clinical response to chloroquine.
